Analysis of TRALI and possible TRALI reported to the French Hemovigilance Network from 2007 to 2013
Using the French Hemovigilance Network database from 2007 to 2013, we provide information on demographics, incidence, and risk factors of reported TRALI and possible TRALI; analyse TRALI mitigation efforts for fresh frozen plasma and platelet concentrates; and consider the impact of platelet additive solutions on TRALI incidence. We applied the Toronto consensus conference definitions for TRALI and possible TRALI. Two TRALI subgroups were considered: “Antibody positive” when a donor has HLA (class I or II) and/or HNA antibodies and the recipient has cognate antigen, and “Antibody negative” when immunological invest...

Acute management of hemostasis in patients with neurological injury
Neurological injuries can be divided into those with traumatic and non-traumatic causes. The largest groups are traumatic brain injury (TBI) and non-traumatic stroke. TBI patients may present with intracranial hemorrhages (contusions, subdural-, or epidural hematomas). Strokes are ischemic or hemorrhagic. In all these disorders thrombosis and hemostasis play a major role. Treatment aims to either cease bleeding and/or restore perfusion. We reviewed hemostatic and thrombolytic therapies in patients with neurological injuries by MEDLINE and EMBASE search using various key words for neurological disorders and hemostatic thera...

Optimal dose, timing and ratio of blood products in massive transfusion: Results from a systematic review
Optimal dose, timing and ratio to red blood cells (RBC) of blood component therapy (fresh frozen plasma [FFP], platelets, cryoprecipitate or fibrinogen concentrate) to reduce morbidity and mortality in critically bleeding patients requiring massive transfusion is unknown. We performed a systematic review for randomised controlled trials (RCT) in MEDLINE, The Cochrane Library, Embase, CINAHL, PubMed and the Transfusion Evidence Library using multiple clinical trials registries was undertaken to 21st February 2017. A Call to Arms: Wartime Blood Donor Recruitment
To ensure an adequate blood supply, blood collection agencies must design campaigns to recruit and maintain an active donor pool. Such campaigns generally appeal to altruism and humanitarianism, which donors most commonly cite as their reasons for donating. However, large donor registries, and the widespread recruitment campaigns that sustain them, did not become a necessity until the technology for the collection, storage and transfusion of blood had advanced to a point that enabled the establishment of transfusion services that could provide large amounts of stored blood to meet high demands. Management of hemostatic disorders in patients with advanced liver disease admitted to an intensive care unit
Patients with liver diseases frequently acquire complex changes in their hemostatic system. Traditionally, bleeding complications in patients with liver disease were ascribed to these hemostatic changes, and liver diseases were considered as an acquired bleeding disorder. Nowadays, it is increasingly acknowledged that patients with liver diseases are in ‘hemostatic rebalance’ due to a commensurate decline in pro- and anticoagulant drivers. Indeed, both thrombosis and bleeding may complicate liver disease. (Source: Transfusion Medicine Reviews)

Evidence-based red blood cell transfusion practices in cardiac surgery
Cardiac surgical patients are amongst the highest consumers of allogeneic red blood cells (RBC) due to the prevalence of anaemia and bleeding. Up until recently, there was a paucity of high quality evidence informing transfusion decisions in this patient group which led to wide-variability in transfusion decision making. The article reviews and critically analyses the available evidence for RBC transfusion in cardiac surgery, focusing on trials of transfusion triggers and age of blood, and provides suggestions for future research. Autologous Blood Collection in Canada
Background: The Choosing Wisely Canada campaign released the eighth CSTM recommendation in June 2015: Do not routinely order perioperative autologous and directed blood collection. This recommendation and change in transfusion practice prompted a review of the current state of the Canadian autologous blood collections. (Source: Transfusion Medicine Reviews)

Residual Risk of Bacterial Contamination of Platelets: 6 Years of Experience with Sterility Testing at Canadian Blood Services
Background: Canadian Blood Services screens platelet concentrates for bacterial contamination with the BacT/ALERT system within 24 to 30 hours post-phlebotomy. Quality Control (QC) sterility testing of 1% outdated platelets (or minimum 10 units) is performed on a monthly basis. Data of routine screening, QC testing, and septic reactions obtained from 2010 to 2016 are presented herein. (Source: Transfusion Medicine Reviews)

Improving Appropriateness of RBC Transfusion for Iron Deficiency Anemia Patients Presenting to the Emergency Department
Background: Patients presenting to the Emergency Department (ED) with iron deficiency anemia (IDA) are under recognized and over transfused. A 3-month audit of RBC transfusions at Sunnybrook Health Sciences Centre (SHSC) ED conducted in 2013 suggested that 58% of those transfused for IDA were appropriate. Interference of Anti-CD38 in Multiple Myeloma Patients
Background: Darzalex (Daratumumab), the first therapeutic CD38 monoclonal antibody to treat multiple myeloma, was approved by Health Canada in June 2016. The CD38 transmembrane protein is highly expressed in malignant myeloma cells. This human CD38-directed (IgG1) monoclonal antibody reacts with glycoprotein found on the surface of many types of cells including red blood cells. It binds to CD38 expressed on RBCs and may result in positive indirect antiglobulin tests (IAT) performed to identify clinically significant alloantibodies. (Source: Transfusion Medicine Reviews)
