Recognition and management of hemostatic disorders in critically ill patients needing to undergo an invasive procedure
Abnormal laboratory coagulation tests are frequently documented in critically ill patients, and these patients often also need to undergo invasive procedures. Clinicians have an understandable desire to minimize any perceived heightened risk of bleeding complications in those patients who require invasive procedures. In this setting prophylactic administration of platelets or plasma is commonplace. While the transfusion-transmission (TT) risk associated with the major transfusion-relevant viruses such as HIV is now very low, during the last 20 years there has been a growing awareness of the threat to blood safety from emerging infectious diseases, a number of which are known to be, or are potentially, transfusion transmissible. Two published models for estimating the transfusion-transmission risk from EIDs, referred to as the Biggerstaff-Petersen model and the European Upfront Risk Assessment Tool (EUFRAT), respectively, have been applied to several EIDs in outbreak situations. Management of platelet disorders and platelet transfusions in ICU patients
Thrombocytopenia or receipt of anti-platelet drugs, with or without bleeding, are common indications for platelet transfusions in the ICU. However there is almost no evidence base for these practices other than expert opinion. Also common is use of platelet transfusions prior to invasive procedures or surgery in patients with thrombocytopenia. Adjuncts to blood component therapies for the treatment of bleeding in the intensive care unit
Patients who are critically ill following surgical or traumatic injury often present with coagulopathy as a component of the complex multi-system dysfunction that clinicians must rapidly diagnose and treat in the intensive care environment. Failure to recognize coagulopathy while volume resuscitation with crystalloid or colloid takes place, or an unbalanced transfusion strategy focused on packed red blood cell transfusion, can all significantly worsen coagulopathy leading to increased transfusion requirements and poor outcomes. Intersecting Worlds of Transfusion and Transplantation Medicine: An International Symposium organized by the Canadian Blood Services Centre for Innovation
The principal theme of the symposium was centred on how the world of regenerative medicine intersects with that of transfusion medicine, with a particular focus on hematopoietic stem cells (HSCs) and stem cell therapies. The symposium highlighted several exciting developments and identified areas where additional research is needed. A revised map of human hematopoietic hierarchy was presented based on the functional and phenotypic analysis of thousands of single stem and progenitor cells from adult bone marrow and fetal liver.
