Red blood cell distribution width [RDW] and long-term mortality after community-acquired pneumonia. A comparison with proadrenomedullin
Proadrenomedullin (proADM), a cardiovascular biomarker, has shown high prognostic power for community-acquired pneumonia (CAP) outcomes. Red-blood-cell distribution width (RDW), linked to cardiovascular disorders, has been associated with short-term and medium-term mortality after CAP. Our objective was to assess the accuracy of both biomarkers for CAP long-term mortality (>90 days).Adults hospitalized with CAP underwent blood proADM, RDW, C-reactive protein (CRP) and procalcitonin (PCT) measurements at admission, and were evaluated after 30, 90, and 180 days, and 1, 2, and 3 years, until either death or 5 years of follow-...

The Xpert MTB/RIF assay in routine diagnosis of pulmonary tuberculosis: a multicentre study in Lithuania
Drug-resistant tuberculosis (TB) is an important public health problem in Lithuania with MDR rates in new cases reaching 11% in 2012. Currently available diagnostic tools are not fully adequate for an accurate and rapid result for diagnosis of TB and MDR-TB. (Source: Respiratory Medicine CME)

Exhaled nitric oxide and carbon monoxide in lung transplanted patients
Exhaled nitric oxide (eNO) and carbon monoxide (eCO) are markers of pulmonary inflammation associated with acute graft rejection and lung infections in lung transplant (LTX) recipients. Regarding eNO and eCO levels in LTX patients affected by bronchiolitis obliterans syndrome (BOS), published data are discordant. (Source: Respiratory Medicine CME)

Serum Cytokine Profiles of Patients with Interstitial Lung Disease Associated with anti-CADM-140/MDA5 antibody positive Amyopathic Dermatomyositis
Patients with amyopathic dermatomyositis (ADM) sometimes develop rapidly progressive interstitial lung disease (ILD) predominantly in Asia. Although anti-CADM-140/MDA5 antibody titer could correlate with disease activity and predict the course of ILD associated with ADM, it is not clear how this antibody is involved in the pathogenesis of ILD in ADM. Home non-invasive mechanical ventilation use following acute hypercapnic respiratory failure in COPD. A solid protective factor
Currently, home NPPV programs in COPD are still on debate and have conflictive results1,2, so it is of utmost relevance to conduct studies clarifying this important issue. Galli JA et al. described positive effects of long term outcome in two acute exacerbation COPD cohorts in favour of NPPV use3. These results generate updated controversy regarding influence of NPPV after discharge; however, a more accurate assessment is needed for appropriate clinical extrapolations. Causes of Chronic Productive Cough: An approach to management
A chronic ‘productive’ or ‘wet’ cough is a common presenting complaint for patients attending the adult respiratory clinic. Most reviews and guidelines suggest that the causes of a productive cough are the same as those of a non-productive cough and as such the same diagnostic pathway should be followed. We suggest a different diagnostic approach for patients with a productive cough, focusing on the conditions that are the most likely causes of this problem. This review is intended to briefly summarise the epidemiology, clinical features, pathophysiology and treatment of a number of conditions which are often associa...

Bone mineral density in patients with idiopathic pulmonary fibrosis
Decreased bone mineral density (BMD) has been reported in patients with interstitial lung disease. However, BMD has not been evaluated in steroid-naïve patients with idiopathic pulmonary fibrosis (IPF). We aimed to measure vertebral BMD and investigate its relationship with clinical features in steroid-naïve patients with IPF. (Source: Respiratory Medicine CME)

Diagnostic value and safety of medical thoracoscopy in tuberculous pleural effusion
Differentiating tuberculous pleural effusion from other lymphocytic pleural effusions is often challenging. This retrospective study aimed to assess the efficacy and safety of medical thoracoscopy in patients with suspected tuberculous pleural effusion. (Source: Respiratory Medicine CME)

Prognostic value of variables derived from the six-minute walk test in patients with COPD: results from the ECLIPSE study
In addition to the six-min walk distance (6MWD), other six-min walk test (6MWT) derived variables, such as mean walk-speed (6MWSpeed), 6-min walk-work (6MWW), distance-saturation product (DSP), exercise-induced oxygen desaturation (EID), and unintended stops may be useful for the prediction of mortality and hospitalization in patients with chronic obstructive pulmonary disease (COPD). We studied the association between 6MWT-derived variables and mortality as well as hospitalization in COPD patients and compared it with the BODE index. (Source: Respiratory Medicine CME)

The effect of bronchodilators on forced vital capacity measurement in patients with idiopathic pulmonary fibrosis
Forced vital capacity (FVC) is a key measure of disease severity in patients with idiopathic pulmonary fibrosis (IPF) and is an important clinical trial endpoint. We hypothesize that reversible airflow limitation co-exists in a subgroup of patients with IPF, and that bronchodilator use will improve the performance characteristics of FVC. Lung ultrasound in the diagnosis and monitoring of community acquired pneumonia in children
Lung ultrasound (LUS) is as an easily accessible, radiation-free imaging technique that might be used as a diagnostic tool in community-acquired pneumonia (CAP).The aim of the study was to evaluate the usefulness and accuracy of LUS in the diagnosis and monitoring of childhood CAP.One hundred six consecutive children aged between 1 and 213 (median 52.5) months referred to the hospital with suspicion of CAP were enrolled. All patients underwent LUS on the day of admission, followed by chest radiograph (CXR). (Source: Respiratory Medicine CME)
