Screening of obstructive sleep apnea by STOP-BANG questionnaire in patients with epilepsy
I read with great interest the manuscript entitled “The STOP-BANG questionnaire improves the detection of epilepsy patients at risk for obstructive sleep apnea,” published recently in Epilepsy Research (Sharma et al., 2017). The authors conducted a screening of epilepsy patients for obstructive sleep apnea (OSA) using the STOP-BANG questionnaire (Chung et al., 2008). Of 269/647 patients (41.6%) who were screened for OSA, 84 met the criteria for elevated OSA risk with STOP-BANG score ≥3. During the subsequent 3 month period, 12 of the 33 screening-positive patients visited the sleep clinic. (Source: Epilepsy Research)

Commentary: Forced thinking about hypercognitive seizures
In this issue, Stephani and Koubeissi provide an interesting historical look at the very uncommon ictal phenomenon of forced thinking. The literature consists primarily of case reports and small series of patients who describe stereotypic unbidden thoughts that occur at the onset of a focal seizure. While the current term is quite descriptive of the intrusive thoughts that patients experience during these events, the authors note that the term “forced thinking” may be conceptually ambiguous, referring to symptoms as varied as forced actions, depersonalization, and auditory verbal hallucinations. We greatly appreciate the comments about our article “The STOP-BANG questionnaire improves the detection of epilepsy patients at risk for obstructive sleep apnea.” Given the potential for obstructive sleep apnea (OSA) to negatively influence seizure control and overall quality of life in people living with epilepsy, we felt it was important to per form this quality improvement project at our institution. As we describe in the article, our study was not without limitations. Given that this represented a voluntary quality improvement project, we could not force medical assistants nor physicians to administer the STOP-BAN...

Generic antiepileptic drug equivalence: The journey from anecdotes to evidence
Generic antiepileptic drug (AED) substitution has been a controversial topic for many years. Every day several million tablets of generic AEDs are taken by people with epilepsy. Generic drugs can be an important weapon to combat healthcare costs –the US Food and Drug Administration (FDA) estimated $230 billion per year was saved by generic substitution in all therapeutic categories. However, advocacy groups have criticized widespread generic substitution in people with epilepsy because the FDA rules allow too much variability across diffe rent products.
