Commentary: Forced thinking about hypercognitive seizures

In this issue, Stephani and Koubeissi provide an interesting historical look at the very uncommon ictal phenomenon of forced thinking. The literature consists primarily of case reports and small series of patients who describe stereotypic unbidden thoughts that occur at the onset of a focal seizure. While the current term is quite descriptive of the intrusive thoughts that patients experience during these events, the authors note that the term “forced thinking” may be conceptually ambiguous, referring to symptoms as varied as forced actions, depersonalization, and auditory verbal hallucinations.
