Anticoagulation in Patients With Left Ventricular Systolic Dysfunction and Sinus Rhythm: When?
Left ventricular (LV) systolic dysfunction and chronic systolic heart failure (HF) predispose to intraventricular thrombus formation and embolization resulting in stroke. Current guideline recommends the use of oral anticoagulants in patients with atrial fibrillation and history of previous thromboembolism. However, anticoagulant treatment in patients with LV systolic dysfunction with sinus rhythm and without history of previous thromboembolism is still on debate. Recent epidemiologic date has reported increased stroke rate in patients with systolic HF shortly after diagnosis. This review focuses on the possible causes of ...

Monitoring of Unfractionated Heparin Using Activated Partial Thromboplastin Time: An Assessment of the Current Nomogram and Analysis According to Age
Conclusion: Our results indicate a high rate of achieving therapeutic aPTT at 24hous and 48hours, but a low success rate for maintenance within the TR. Most patients had supratherapeutic aPTT of more than 90 seconds. Therefore, the TR of aPTT that matches heparin levels of 0.3 to 0.7 IU/mL measured by antifactor Xa assay should be determined. If not, we should consider adopting a new heparin dosing nomogram. (Source: Clinical and Applied Thrombosis/Hemostasis)

Determinants of Angiographic Thrombus Burden in Patients With ST-Segment Elevation Myocardial Infarction
Conclusion: Present study results demonstrated that high thrombus burden in patients with STEMI was associated with impaired postprocedural epicardial and myocardial perfusion and higher no reflow and distal embolization; and increased RDW values were independent predictors of coronary thrombus burden. Construction Noise Induces Hypercoagulability and Elevated Plasma Corticosteroids in Rats
Although animals housed for research purposes are strictly monitored for lighting, temperature, and humidity, the acoustic environment receives less attention. In a retrospective study, we investigated the effect of construction-induced noise on coagulation using thromboelastography in a group of healthy control rats. Animals were unintentionally exposed to noise due to construction in the vicinity of the animal care facility where these rats were housed. We compared the results to those of age-, gender-matched nonexposed rats. There was a significant shortening of the reaction (R) time (P = .009) and a trend toward an inc...

Lower Incidence of M2/ANXA5 Carriage in Recurrent Pregnancy Loss Patients With Elevated Lipoprotein(a) Levels
This study compared the incidence of M2/ANXA5 haplotype carriage, a documented repeated miscarriage risk factor, in patient groups with normal and elevated lipoprotein(a) (Lp(a)) levels. A total of 138 women with ≥2 consecutive, idiopathic recurrent miscarriages, categorized in patients with elevated (≥30 mg/dL, n = 44) and normal Lp(a) level (<30 mg/dL, n = 94) were recruited at the recurrent pregnancy loss (RPL) clinic of Munich Großhadern University Hospital. A total of 500 fertile women served as controls. All patients were genotyped for ANXA5 promoter haplotypes, genetic frequencies were compared, and o...

SYNTAX Score Predicts the Left Ventricle Thrombus Development in Patients Undergoing Primary Percutaneous Coronary Intervention for First Anterior Myocardial Infarction
Conclusion: High SS which was obtained through diagnostic angiogram of PPCI may be associated with LVT development in patients with first anterior wall STEMI. (Source: Clinical and Applied Thrombosis/Hemostasis)

Effect of Graduated Compression Stockings on Venous Blood Velocity in Supine Resting Hospitalized Patients
In conclusion, thigh-length GCS in hospitalized patients had little or no effect on popliteal or femoral vein blood velocity while supine at rest, irrespective of whether the patients had venous insufficiency. (Source: Clinical and Applied Thrombosis/Hemostasis)

Relation of Red Cell Distribution Width With CHA2DS2-VASc Score in Patients With Nonvalvular Atrial Fibrillation
Conclusion: Our study results indicate that RDW values are significantly correlated with CHA2DS2-VASc score in nonanemic patients with AF, while also being independent predictor of high CHA2DS2-VASc score. (Source: Clinical and Applied Thrombosis/Hemostasis)

Sustained Release of Tissue Factor Following Thrombosis of Lower Limb Trauma
This study was undertaken to provide evidence for the mechanism of venous thromboembolism (VTE) in healthy patients with minor lower limb injury (fracture; Achilles tendon rupture) that was medically managed with plaster cast/brace immobilization. The Plaster Cast clinical trial provided a unique opportunity to identify the natural history of VTE using placebo-controlled patients (n = 183) with validation of the mechanism using the low-molecular-weight heparin (LMWH; reviparin)-treated patients (n = 182). Confirmed VTE in this population was associated with a burst of tissue factor release (and a minor fibrinolytic deficit...

Do Postliver Transplant Patients Need Thromboprophylactic Anticoagulation?
Postoperative thromboprophylactic anticoagulation against Deep Vein Thrombosis (DVT) and Pulmonary Embolism (PE) is standard of care with current evidence-based guidelines. However, majority of liver transplant (LT) patients have thrombocytopenia and/or prolonged INR before surgery. Studies or guidelines regarding role of prophylactic anticoagulation after LT are lacking. There is a need to balance the risk of thrombosis with significant hemorrhage, implying those needing transfusion or return to OR due to bleeding. We conclude that after LT, anticoagulation is not required routinely for DVT/PE prophylaxis. Rather, it is i...

The Risk Factors for Thrombosis in Children With Acute Lymphoblastic Leukemia
We aimed to scrutinize the risk factors for thrombosis in children with acute lymphoblastic leukemia treated with the Berlin-Frankfurt-Münster 95 protocol. The study population was 82 children younger than 16 years of age. The children were followed up for 10 years until January 2007. Thrombosis occurred in 10 (12%) of 82 patients during the treatment course, mainly after the M protocol. The most common risk factor was factor V Leiden (FVL; 15.6%). This was followed by methyleneterahydrofolate reductase (MTHFR; 9.3%), elevated lipoprotein (1.5%), and prothrombin (PT) 20210A (1.5%) in descending order. The risk of thro...

Comparison of Thrombin Generation Assay With Conventional Coagulation Tests in Evaluation of Bleeding Risk in Patients With Rare Bleeding Disorders
Based on the premise that the capacity of plasma to generate thrombin in vitro is a comprehensive and precise functional test of the clotting system, we designed a cross-sectional, single-center study involving 83 patients with rare bleeding disorders (RBDs) to compare the usefulness of the thrombin generation (TG) assay versus conventional tests including prothrombin time (PT) and activated partial thromboplastin time (aPTT) in predicting bleeding risk in patients with RBD in southern Iran. The TG parameters consisted of endogenous thrombin potential, lag time, peak, time to peak (ttPeak), and start tail. The area under t...
