Combined variants in reading epilepsy; coexisting anterior and posterior variants camouflaged as heat cramps where the patient finds his own diagnosis searching the internet
We present a case with suggestible evidence of both conditions coexisting within the same patient, a finding that, to our knowledge, has not been previously reported. The diagnosis in this specific case was contributed to by the patient searching the internet. (Source: Epilepsy and Behavior Case Reports)

ACTH therapy for West syndrome with severe hemophilia A
Publication date: Available online 10 May 2016 Source:Epilepsy & Behavior Case Reports Author(s): Kazuo Kubota, Yoriko Kinomura, Takahiro Yamamoto, Michio Ozeki, Minako Kawamoto, Norio Kawamoto, Toshiyuki Fukao Hemophilia A is an X-linked recessive disorder caused by factor VIII deficiency, which is an important factor in the coagulation system. Here, we describe a 1-year-old boy with hemophilia A who developed West syndrome (WS). Recombinant factor VIII was administered during adrenocorticotropic hormone (ACTH) therapy to prevent intracranial hemorrhage. Infusion of factor VIII at fixed intervals...

Occipital seizures and persistent Homonymous Hemianopia with T2 hypointensity on MRI in Non-Ketotic hyperglycemia
Conclusion Occipital seizures with visual field deficits can occur in hyperglycemic states. These can be associated with specific MRI brain and EEG changes. The HH is reversible with apt treatment primarily including glycemic control with or without anti-seizure medication. (Source: Epilepsy and Behavior Case Reports)

Relationship between High Frequency Oscillations and Spikes in a Case of Temporal Lobe Epilepsy
Conclusions These results agree that ripples are a marker for SOZ and co-occur with spikes. Seizure onset was heralded by an increase in ripples and spikes with ripples, without an increase in spikes without ripples. We hypothesize that spikes associated with ripples may have a somewhat different pathophysiological mechanism than spikes not associated with ripples, differences that may be relevant for the timing of seizure onset. Atypical Benign Partial Epilepsy of Childhood with Acquired Neurocognitive, Lexical Semantic and Autistic Spectrum Disorder
We describe the phenotype–genotype study of a boy with ABPE with follow up from age 5 to 13years showing acquired oromotor and later, a specific lexical-semantic and pervasive developmental disorder. Exome sequencing identified mutations in SCN9A, CPA6, and SCNM1. A direct role of the epilepsy in the pathogenesis of the oromotor and neurocognitive deficits is apparent. Long-term accelerometry-triggered video monitoring and detection of tonic–clonic and clonic seizures in a home environment: Pilot study
Conclusion While the algorithms were previously validated in a controlled video/EEG monitoring unit with wired sensors, we now show the first results of long-term, wireless testing in a home environment. (Source: Epilepsy and Behavior Case Reports)

A unique epilepsy case with cinematographic visual hallucinations
Conclusion The present study demonstrates focal seizures of temporal lobe origin producing complex visual hallucinations without a corresponding lesion on MRI brain imaging. (Source: Epilepsy and Behavior Case Reports)

Mitigating bit flips or single event upsets in epilepsy neurostimulators
Conclusions Cosmic radiation can threaten RAM and settings of Neurostimulators, neuromodulation teams and device designers need to take this threat into account when designing multifunctional neuromodulation systems. (Source: Epilepsy and Behavior Case Reports)

Dynamics of epileptic activity in a peculiar case of childhood absence epilepsy and correlation with thalamic levels of GABA
Publication date: Available online 6 April 2016 Source:Epilepsy & Behavior Case Reports Author(s): Alberto Leal, José P. Vieira, Ricardo Lopes, Rita G. Nunes, Sónia I. Gonçalves, Fernando Lopes da Silva, Patrícia Figueiredo Objectives Childhood Absence Epilepsy (CAE) is a syndrome with well-defined electro-clinical features, but unknown pathological basis. An increased thalamic tonic GABA inhibition has recently been discovered on animal models (Cope, 2009) but its relevance for human CAE is unproven. Methods We studied an 11-year old boy, presenting the typical clinical features of CAE, but...

H-Coil Repetitive Transcranial Magnetic Stimulation for Treatment of Temporal Lobe Epilepsy: A Case Report
We report the results of rTMS in a young adult with pharmacoresistant bilateral TLE who underwent three courses (of 10, 15 and 30 daily sessions) of unilateral rTMS over the hemisphere from which seizures originated most often. Seizure frequency was assessed before and after each block of rTMS sessions, as was the tolerability of the procedure. Seizure frequency declined significantly, by 50 to 70 % following each rTMS course. All sessions were well-tolerated. (Source: Epilepsy and Behavior Case Reports)

Intraoperative Mapping of Expressive Language Cortex Using Passive Real-Time Electrocorticography
Publication date: Available online 16 March 2016 Source:Epilepsy & Behavior Case Reports Author(s): AmiLyn M. Taplin, Adriana de Pesters, Peter Brunner, Dora Hermes, John C. Dalfino, Matthew A. Adamo, Anthony L. Ritaccio, Gerwin Schalk In this case report, we investigate the utility and practicality of passive intraoperative functional mapping of expressive language cortex using high-resolution electrocorticography (ECoG). The patient presented here experienced new-onset seizures caused by a medium-grade tumor in very close proximity to expressive language regions. In preparation of tumor resect...

Phenytoin-induced isolated chronic, nocturnal dry cough
We report a 72-year-old man with a four-year history of dyscognitive seizures (with occasional secondary generalization) who developed isolated, nocturnal dry cough immediately after being starting on PO phenytoin. The cough was not accompanied with any other symptom or sign – as his physical exam was completely normal. Further investigation with chest CT and spirometry was unremarkable. This symptom persisted for six months, and did not resolve until we weaned him off of phenytoin. According to the Naranjo Adverse Drug Reaction Probability Scale, his cough was classified as being probably (score +6) related to the use o...

Electrocorticographic mapping of expressive language function without requiring the patient to speak: A report of three cases
Conclusions The results of this study provide encouraging preliminary evidence that it may be possible that a brief and practical protocol can identify expressive language areas without requiring the patient to speak. This protocol could provide the clinician with a map of expressive language cortex within a few minutes. This may be useful as an adjunct to ECS interrogation, or as an alternative to mapping using functional magnetic resonance imaging (fMRI). In conclusion, with further development and validation in more subjects, the approach presented here could help identifying expressive language areas in situations wher...
