Single-site robotic platform in clinical practice: first cases in the USA.
Authors: Kaouk J, Bertolo R Abstract The initial experience with urological robotic interventions performed by using the Da Vinci SP® robotic surgical platform (Intuitive Surgical, Sunnyvale, CA, USA) during the first month (September 28th - October 30th, 2018) after the system was installed at our institution is reported herein. The aim of the study was to determine the feasibility and the safety of major urologic procedures as measured by the rate of conversions and the incidence of perioperative complications. Secondary aims of the study consisted of key perioperative surgical outcomes including operat...

Radiological Wheeler staging system: a retrospective cohort analysis to improve the local staging of prostate cancer with multiparametric Magnetic Resonance Imaging.
CONCLUSIONS: The rW staging system has low accuracy in predicting each single pW class, while accuracy was over 80% for experienced readers in the identification of organ confined (T2 stage class) tumours and non-organ confined cases (T3 stage class). Impact of hospital admissions subsequent to renal transplantation on Italian regional resources: a retrospective study in the region Emilia-Romagna.
CONCLUSIONS: Costs due to admissions of RTRs appeared to gradually increase in the long term probably due to the increasing number of admissions and increasing mean age of admitted RTRs. PMID: 30654602 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Bladder recurrence of primary upper tract urinary carcinoma following nephro-ureterectomy, and risk of upper urinary tract recurrence after ureteral stent positioning in patients with primary bladder cancer: a contemporary non-systematic review of the literature.
Authors: Mistretta FA, Carrion DM, Nazzani S, Vásquez JL, Fiori C, De Cobelli O, Porpiglia F, Esperto F Abstract Upper tract urinary carcinoma (UTUC) and bladder cancer (BCa) share similar biological, histological and pathological characteristics. These resemblances could explain the high rate of synchronous and metachronous tumours affecting both upper urinary tract and bladder. In patients affected by primary UTUC bladder recurrence is quite common and it represents one of the major concerns during the follow-up of patients who underwent radical nephro-ureterectomy. Conversely, UTUC recurrence after pri...

Retroperitoneoscopy in urology: a systematic review.
CONCLUSIONS: Retroperitoneoscopic approach in urological diseases is a valid alternative to the anterior approach. Evidence suggest lower morbidity for the retroperitoneoscopic approach however technical complexity may limit its widespread. The role of vascular clamping during robot-assisted partial nephrectomy for localised renal cancer: rationale and design of the CLOCK randomised phase III study.
Authors: Cindolo L, Antonelli A, Sandri M, Annino F, Celia A, De Concilio B, Giommoni V, Nucciotti R, Sessa F, Porreca A, Veccia A, Schips L, Minervini A, AGILE Group (Italian Group for advanced laparo-endoscopic surgery) Abstract Outline the design and rationale of CLOCK, a large, phase III randomised controlled trial which examines the role vascular clamping during robot-assisted partial nephrectomy (RAPN) for localised renal cancer. To be eligible for study entry, patients must have normal coagulative function, preoperative estimated glomerular filtration rate (eGFR) >60 ml/min, normal contralateral ...

External validation of Imamura nomogram as a tool to predict preoperatively laser semi-rigid ureterolithotripsy outcomes.
CONCLUSIONS: We firstly validated Imamura nomogram in an European cohort study. It proved a reasonable accuracy (AUC 0.67) and a good PPV (83%). Further studies should confirm our results to support the routine clinical use of Imamura nomogram as a tool to predict ULT outcomes. Effect of puboprostatic ligament reconstruction on continence recovery after robot-assisted laparoscopic prostatectomy: our initial experience.
CONCLUSIONS: Despite anterior reconstruction of the puboprostatic ligament showed no significant effect on the overall continence, it showed earlier return to continence up to 6 months, which supports the theory that anterior puboprostatic reconstruction may provide better immediate continence and shorten the time to continence for RALP patients. However, most of the published literature showed better continence rates with the total anatomical reconstruction (combined anterior and posterior). Therefore, we started to offer patients in our center total anatomical reconstruction during RALP. PMID: 30547904 [PubMed - ...

Low dose CT scan in stone detection for stone treatment follow up: is there a relation between stone composition and radiation delivery? Study on a porcine-kidney model.
CONCLUSIONS: Ultra-low-dose CT has a good detection rate for all kinds of stone, even when the fragment size is small. Only small uric acid fragments need higher energy settings in order to be detected. When the stone composition is known after surgery for urolithiasis, the most appropriate CT scan setting could be suggested by the urologist during their follow-up. Oncologic outcomes in prostate cancer patients treated with robot-assisted radical prostatectomy: results from a single institution series with more than 10 years follow up.
CONCLUSIONS: In experienced centres, RARP allows optimal oncologic outcomes at long term follow up. Adverse pathologic characteristics are independent predictors of BCR and CR. PMID: 30547906 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Cytoreductive prostatectomy: what is the evidence? A systematic review.
CONCLUSIONS: Although supported from a biological point of view and albeit encouraging results of population-based studies, cytoreductive remains to date experimental. A true benefit on overall survival of mPCa is not supported by current evidence. The results of prospective trials are eagerly awaited. Small cell carcinoma of the upper urinary tract and factors associated with progress survival: a systematic review and pooled analysis.
CONCLUSIONS: In multivariable analyses, no variables were associated with OS and CSS. UUT-SCC is a rare tumor characterized by an aggressive clinical course. Pathological stage and platinum-based chemotherapy regimen are the most important factors related to OS and CSS.
