Comparison of general anesthesia and combined spinal-epidural anesthesia for retrograde intrarenal surgery: a prospective randomized trial in one center.
CONCLUSIONS: Combined spinal-epidural anesthesia, which produces favorable outcomes in the intraoperative and postoperative periods, will become an alternative to general anesthesia. Also, the costs associated with these two anesthesia methods were calculated, it was found that the total cost of anesthesia medications and materials per operation was similar both methods. Impact of Enhanced Recovery After Surgery (ERAS) protocols versus standard of care on perioperative outcomes of radical cystectomy: a systematic review and meta-analysis of comparative studies.
CONCLUSIONS: This systematic review with meta-analysis of comparative studies showed that ERAS protocols applied to patients undergoing RC enabled a faster recovery of bowel function, a faster return to regular diet and a shorter hospital stay with no increase in major complication or readmission rate compared to standard perioperative care. RC with ERAS protocols should be considered the new standard of care. PMID: 31241271 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Body Mass Index and prostatic specific antigen are independent predictors of multiple prostate cancer lymph node metastases in Caucasian patients undergoing robot assisted radical prostatectomy and extended pelvic lymph node dissection.
CONCLUSIONS: The risk of multiple LNI was independently predicted by PSA and BMI with fair discrimination power. PMID: 31241272 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Study of phosphodiesterase 5 inhibitors and α-adrenoceptor antagonists used alone or in combination for the treatment of lower urinary tract symptoms due to benign prostatic hyperplasia.
CONCLUSIONS: Combination treatment is more effective than PDE5i or Abs alone, though the side effects are not significantly different. PMID: 31241273 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Robotic pyelolithotomy for the treatment of large renal stones: a single centre experience.
CONCLUSIONS: Our findings suggest that RPL is a safe, reproducible and minimally invasive approach as treatment of large renal stones when endoscopic treatment failed or was not available. RPL permits to achieve an excellent stone free status, in a single definitive procedure. PMID: 31241274 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Factors predictive of SWL failure for ureteral stones: why we need to hurry.
CONCLUSIONS: We report a large series of patients undergone primary SWL for ureteric stones. Our data demonstrated the role of hydronephrosis, time elapsed from obstruction onset to treatment and stone density as independent predictive factors of SWL failure. Identifying tumor-related risk factors for simultaneous adrenalectomy in patients with cT1 - cT2 kidney cancer during robotic assisted laparoscopic radical nephrectomy.
CONCLUSIONS: Our evaluated tumor-related characteristics did not show to impact the incidence of simultaneous adrenalectomy. Adrenal gland resection T does not provide significant benefit in recurrence-free survival. We consider that RALRN with adrenalectomy should be reserved only for patients with adrenal compromise as stated previously regardless that it has shown to be a safe procedure. When should we use urodynamic testing? Recommendations of the Italian society of urodynamics (SIUD). Part 2 - Male and neurogical population.
CONCLUSIONS: This review provides a summary of the most effective utilization of urodynamic studies for the global evaluation of patients with lower urinary tract symptoms, and how to use them when really needed, avoiding unnecessary costs and patient inconveniences. Can preoperative multiparametric MRI avoid unnecessary prostate biopsies before Holmium laser enucleation of the prostate? Preliminary results of a multicentric cohort of patients.
CONCLUSIONS: Including a mpMRI in clinical evaluation of patients eligible to HoLEP with a preoperative PCa suspicion leads to low the rates of iPCa and might avoid unnecessary TRUS-GB. PMID: 31166103 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Pain and discomfort after Retzius-sparing robot-assisted radical prostatectomy. A comparative study between suprapubic cistostomy and urethral catheter as urinary drainage.
CONCLUSIONS: We demonstrated suprapubic tube to be more comfortable than transurethral catheter after RARP, with a possible advantage concerning anastomotical postoperative problems. PMID: 31144484 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Predictive factors for progression of patients with carcinoma in situ of the bladder at long-term follow-up: pure versus non-pure Cis.
CONCLUSIONS: Pure Cis and non-pure Cis are efficiently treated by BCG therapy combined with transurethral resection and/or radical cystectomy, with relatively low rate of progression. Cis type was the only significant predictor of progression. PMID: 31144485 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Patterns of positive surgical margins after open radical prostatectomy and their association with clinical recurrence.
CONCLUSIONS: Men with bilateral PSMs had higher risk to experience CR, suggesting that the different patterns of PSMs, should be considered during patients counseling to guide postoperative treatments. Retrospective nature of the study and restricted number of patients included consist of main limitations. PMID: 31144486 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Relationship between cellular and exosomal miRNAs targeting NOD-like receptors in bladder cancer: preliminary results.
CONCLUSIONS: Our work demonstrated the inverse correlation between exosomal and cellular miRNAs in patients with bladder cancer. The fact that these miRNAs were higher in exosomal than cellular fraction allowed us to hypothesize their active compartimentalization during cancer progression, suggesting their potential role as cancer messengers. The effect of using Ureteral Access Sheath (UAS) for treatment of impacted ureteral stones at mid-upper part with flexible ureterorenoscopy: a randomized prospective study.
CONCLUSIONS: In the treatment of mid-upper impacted ureteral stones, using UAS during the application of FURS is an advantageous procedure due to shorter operation time, fever intraoperative stone migrations and the need for additional surgery, less postoperative infection. Smoking reduces PSA accuracy for detection of prostate cancer: results from an Italian cross-sectional study.
CONCLUSIONS: Ina cohort of Italian men undergoing prostate biopsy, the performance accuracy of PSA, as a predictor of prostate cancer, is lower in smokers. Although the molecular link behind our findings is still unknown, our study firstly showed that the PSA accuracy for PCa diagnosis is significantly influential by smoking.
