Robot-assisted laparoscopic single-site hysterectomy: our experience and multicentric comparison with single-port laparoscopy.
CONCLUSIONS: This study confirms the safety and reproducibility of the RSS. The comparison between RSS and LESS techniques has shown that RSS has important advantages over the LESS, like deeper learning curve, lower OT not correlated with age and uterus size. Comparison of patients' reported pain following office hysteroscopy with and without endometrial biopsy: a prospective study.
CONCLUSIONS: Patients undergoing both OH and EMB appear to report similar pain scores as those undergoing OH alone after controlling for confounding variables. The presence of fibroids was found to contribute to higher reported pain in the patients having OH in combination with EMB. [New methods for prediction of preterm birth: the PAMG-1 Test].
CONCLUSIONS: The inaccuracy of traditional methods, including biophysical tests, biochemical markers, or the combination of both, for assessing the risk of spontaneous preterm birth in patients presenting with symptoms of PTL often leads to unnecessary admission and administration of corticosteroids and tocolytics. The PAMG-1 test demonstrated high efficacy in identifying women at risk of imminent PTL within 7 days of testing, despite being performed immediately after TVUS. High NPV can prevent improper admission and unnecessary therapies for mother and fetus. PMID: 29856188 [PubMed - as supplied by publisher] (Sou...

Art in women of advanced fertility age.
Authors: Artini PG, Obino ME, Vergine F, Sergiampietri C, Papini F, Cela V Abstract Over the past four decades, the postponement of childbearing has continued to increase, significantly impacting the age of first-time mothers. Female age is the most significant factor influencing clinical outcome in IVF. Overcome the limits of the age-related decline of fertility is a challenge for fertility experts. In the decrease of fertility in late reproductive age, a lot of factors related with advancing women's age and general health can be considered, however the dominant regulator of this age- dependent loss of fe...

Age-adjusted treatment of cervical intraepithelial 1 lesions by superficial or regular loop excision of transformation zone.
CONCLUSIONS: Among women of childbearing age, the performance of a LEEP procedure with smaller cone height appeared equivocal in achieving correct pathological diagnoses and low recurrence. To avoid increasing the risk of premature delivery, the excisional depth should be adjusted to age and fertility status. PMID: 29845824 [PubMed - in process] (Source: Minerva Ginecologica)

Managing fertile women under lithium treatment: the challenge of a Teratology Information Service.
CONCLUSIONS: Lithium treatment in fertile women is a very delicate topic, where risks and benefits of discontinuing therapy when women plan to become pregnant should be accurately evaluated. Thorough peri-conceptional counselling is crucial for the outcome of pregnancy and for maternal health status during preconception, gestation and breastfeeding. Successful pregnancy in a woman previously suffering from β-thalassaemia following transplantation of ovarian tissue cryopreserved before puberty.
Authors: Matthews SJ, Picton HM, Ernst E, Andersen CY Abstract Ovarian tissue removed from a nine year old girl suffering from thalassemia was kept deep- frozen for 14 years before being transplanted back to the now adult woman. Subsequently, she conceived following IVF treatment and delivered a healthy baby where the oocyte derived from the transplanted tissue. This is the first woman worldwide to have a baby where the ovarian tissue was frozen pre-pubertally and expand the therapeutic options for rare genetic diseases like thalassemia and girls who suffer from childhood cancer. Challenges of fertility preservation in leukemia patients.
Authors: Shapira M, Raanani H, Derech Chaim S, Meirow D Abstract An ongoing increase in survival rates among young leukemia patients is accompanied by a growing attention to possible long-term complications such as chemotherapy induced ovarian insufficiency and infertility. Therefore, an important element of the management of these patients is fertility preservation, which is challenged by considerations specific for leukemia. Such considerations include prepuberty, poor medical condition on presentation and the need for urgent chemotherapy, all of which may preclude the use of conventional assisted reprod...

Techniques for ovarian tissue transplantation and results.
Authors: Donnez J, Manavella DD, Dolmans MM Abstract Fertility preservation has emerged as a field of growing interest thanks to increasingly effective cancer treatments in women. For young women with good prospects of surviving cancer, fertility preservation counseling before gonadotoxic treatment is imperative to offer them a potential chance of future childbearing. Ovarian tissue cryopreservation and transplantation rises as a strategy for fertility preservation, especially for prepubertal girls, adolescents and women in whom cancer treatment cannot be delayed. Numerous surgical techniques have been rep...

Fertility preservation in women with ovarian endometriosis.
Authors: Donnez J, García-Solares J, Dolmans MM Abstract Endometriosis is a frequently encountered benign gynecological disease that may be responsible for infertility. Treatment of endometriosis-associated infertility has been investigated using both medical and surgical therapeutic modalities. Therapy has essentially three main objectives: (i) to preserve and improve fertility, (ii) to reduce pain, and (iii) to delay recurrence. The aim of this paper is to focus on fertility preservation in women with severe endometriosis. In ovarian endometriosis-associated infertility, a medico-surgical approach remai...

Pregnancy complications in infertile patients with polycystic ovary syndrome: updated evidences.
Authors: Palomba S, Falbo A, Daolio J, Battaglia FA, LA Sala GB Abstract Polycystic ovary syndrome (PCOS) is a common female disorder with a pathogenesis still today not completely known. To the present, PCOS is considered more than just a reproductive disorder since several metabolic consequences that could affect women's health during different stages of reproductive and post-reproductive life were reported. The aim of the current review was to evaluate present evidence-based data regarding the pregnancy complications in infertile patients with PCOS. An extensive literature search until February 2018 was...

Indications for fertility preservation in women from malignant diseases to benign conditions to age-related fertility decline.
Authors: Dolmans MM, Donnez J Abstract Oocyte vitrification and ovarian tissue cryopreservation are 2 common fertility preservation options. Oocyte vitrification provides the highest yield for women with benign diseases or those seeking fertility preservation for age-related fertility decline. This technique is also indicated for cancer women if cancer treatment can be post-poned. Ovarian tissue cryopreservation has some specific indications. There is indeed no other alternative for fertility preservation in prepubertal girls, adolescents and cancer women in whom cancer treatment cannot be postponed. This ...

Risk of transplanting malignant cells in cryopreserved ovarian tissue.
Authors: Dolmans MM, Masciangelo R Abstract Improvements in cancer treatments have increased the chances of survival of young cancer patients, but have given rise to other issues, like premature ovarian insufficiency and infertility. Preservation and subsequent restoration of ovarian function in these patients is now possible thanks to ovarian tissue cryopreservation and transplantation. However, safety concerns about the possible presence of cancerous cells in the tissue to be cryopreserved must be addressed. Indeed, reimplantation of malignant cell-contaminated ovarian tissue could potentially lead to re...
