Updates on the management of placenta accreta spectrum.
Authors: Di Mascio D, Calì G, D'antonio F Abstract Placenta accreta spectrum (PAS) disorders is a multifactorial process that encompasses a heterogeneous group of conditions characterized by an abnormal invasion of trophoblastic tissue through the myometrium and uterine serosa. PAS is associated with a high burden of adverse maternal outcomes including severe life-threatening hemorrhage, need for blood transfusion, damage to adjacent organs, and death. Prenatal screening of PAS is mandatory so that women may be counselled about the severity of this condition to plan management with a multidisciplinary tea...

Cumulative live birth rates in IVF.
CONCLUSIONS: Cumulative LBR are a more suitable are more comprehensive when reporting success in an IVF program. Cumulative LBR increase with the number of oocytes retrieved, suggesting that ovarian stimulation may have a minimal or no detrimental effect on oocyte/embryo quality. Fetoscopic spina bifida repair.
CONCLUSIONS: Continued refinement of a minimally invasive strategy for prenatal treatment of spina bifida is necessary to maximize benefits to the child and further minimize maternal risks and preterm birth. PMID: 30486637 [PubMed - as supplied by publisher] (Source: Minerva Ginecologica)

Uterine transposition technique: update and review.
Authors: Ribeiro R, Baiocchi G, Tsunoda AT, Linhares JC, Pareja R Abstract To review and discuss the uterine transposition technique for preserving fertility in patients who require pelvic radiotherapy. Uterine transposition was developed from adapting well-established surgical techniques with the goal of keeping the uterus and adnexa outside the radiotherapy field in order to preserve their function. The uterine transposition technique is presented step by step, with discussion of the literature that served as the basis for the technique's development. Its most recent modifications, resulting from the aut...

Evaluation of safety and skin tolerability of organic cotton pads in case of irritative vulvitis.
CONCLUSIONS: Results demonstrated that organic cotton pads, in each part, are safe and do not impair any physiological activities of the tissue substrates. PMID: 30465412 [PubMed - in process] (Source: Minerva Ginecologica)

Pearls in clinical obstetrics: challenges in anticoagulation in pregnancy.
Authors: Della Torre M, Digiovanni LM, Sutherland MB Abstract Women are at increased risk for VTE during both pregnancy and in the post-partum period. We have conducted a comprehensive literature review of the use of anticoagulation in pregnancy for pregnant women at increased risk for venous thromboembolism (VTE). Multiple factors, including physiologic and pharmacokinetic changes make the treatment and prevention of VTE complicated in pregnancy. Adequate treatment and prevention of VTE in pregnancy is critically important, yet good quality medical studies continue to be lacking. There is a growing amount...

Update in non invasive prenatal testing.
Authors: D'ambrosio V, Squarcella A, Vena F, Di Mascio D, Corno S, Pajno C, Piccioni MG, Brunelli R, Pizzuti A, Benedetti Panici P, Giancotti A Abstract Non-invasive prenatal test (NIPT) has revolutionized the approach to prenatal diagnosis and, to date, it is the most superior screening method for the common autosomal aneuploidies, mostly trisomy 21. This screening has having a significant population-wide impact on the uptake of conventional screening and diagnostic testing. In recent years, emerging genomic technologies, largely based around next generation sequencing, have expanded the analyses to the s...

Effects of adenomyosis on obstetric outcomes: a literature review.
CONCLUSIONS: The potential consequences of adenomyosis during pregnancy are difficult to evaluate because, now, there are few data available concerning adenomyosis and major obstetrical complications and because the study populations of those studies were small. Our results suggest that women with adenomyosis have needed of a prenatal management in a tertiary center. PMID: 30318871 [PubMed - as supplied by publisher] (Source: Minerva Ginecologica)

Management of endometriosis from diagnosis to treatment. Roadmap for the future.
Authors: Muzii L, Di Tucci C, Di Feliciantonio M, Galati G, Marchetti C, Perniola G, Pecorini F, Benedetti Panici P Abstract Endometriosis, in spite of decades of research on the topic, remains a mysterious and elusive disease. Both in the fields of diagnosis and treatment, many issues remain unresolved, and the scientific community strives in trying to find universal criteria for diagnosis, and algorithms of treatment that may be universally applied. Recently, there has been a shift away from the view of the need of invasive diagnosis and therapy with the universal use of laparoscopy. Today the diagnosis ...

Epigenetics and the vaginal microbiome: influence of the microbiota on the histone deacetylase level in vaginal epithelial cells from pregnant women.
Authors: Witkin SS, Nasioudis D, Leizer J, Minis E, Boester A, Forney LJ Abstract Histone deacetylase (HDAC) influences the acetylation status of histones at gene promotor loci, providing an epigenetic mechanism that regulates gene expression. We determined if variations in the composition of the vaginal microbiome in pregnant women were associated with alterations in the level of HDAC1 in vaginal epithelial cells and whether this influenced the concentration of compounds present in vaginal fluid. Vaginal epithelial cells were obtained from 150 women in their first trimester of pregnancy, lysed and assayed...

Managing fetal growth restriction: surveillance tests and their interpretation.
Authors: Seravalli V, Di Tommaso M, Petraglia F Abstract The main challenges in pregnancies affected by fetal growth restriction consist in identifying signs of disease progression and determining the appropriate timing of delivery. The risk of continuing the pregnancy must be balanced with the risk of prematurity, which depends on gestational age. To allow appropriate monitoring of the growth-restricted fetus, several surveillance tests are available. These include ultrasound Doppler velocimetry of feto-placental vessels, cardiotocography, and amniotic fluid evaluation. It is well known that the combinati...
