Minilaparoscopy in gynecology: applications, benefits and limitations.
CONCLUSIONS: Minilaparoscopy in gynecologic surgery represents an option for selected patients, for both benign and malignant indications. The aesthetic outcome represents the main benefit of this approach. Surgeons have to be aware of few limitations of this technique such as severe obesity and endometriosis. Endometriosis-associated infertility: surgery or IVF?
Authors: Muzii L, Di Tucci C, Galati G, Mattei G, Chiné A, Cascialli G, Palaia I, Benedetti Panici P Abstract Endometriosis is a chronic disease frequently associated with female infertility. The choice of treatment in case of endometriosis is one of the most discussed topics in Reproductive Medicine. The approach to the patient with endometriosis and infertility should be tailored based on different parameters. The localization of the disease, the severity of symptoms and the age of the patient are just some of them. Management options include surgery, in-vitro fertilization (IVF), or a combination of bo...

High grade cervical intraepithelial neoplasia positive biopsy: the importance of accurate pre-operative workup.
CONCLUSIONS: These findings suggest implementing the protocol for the management of women with high risk precancer lesions, with a further HPV test before surgical treatment. The evaluation of HPV presence and genotype before conization might represent a useful tool in reducing or postpone the conization treatment. Efficacy and safety of a novel vaginal medical device in recurrent bacterial vaginosis: a multicenter clinical trial.
CONCLUSIONS: Our research showed that 3 monthly cycles of Polybactum® ovules administered after one course of metronidazole vaginal therapy can reduce the rate of Bacterial vaginosis recurrence and improve the vaginal milieu, favoring the growth of vaginal lactobacillus species. Cesarean delivery: an evidence-based review on of the technique.
Authors: Carbone L, Saccone G, Conforti A, Maruotti GM, Berghella V Abstract The cesarean section is utilized to deliver babies since the late 19th century. Nowadays, the frequency of cesarean section is enormously increased, mainly because of the low rate of complications and for the increasing demand from future mothers, scared by the idea of painful labor. Although the technique to perform cesarean section has been refined over time, infections, hemorrhage, pain and other consequences still represent matter of debate. To try to reduce the incidence of these complications many trials, randomized or not, ...

Approaching ovarian endometrioma with medical therapy.
Authors: Vignali M, Solima E, Barbera V, Becherini C, Belloni GM Abstract Endometriosis is a chronic inflammatory gynecological disorder associated with pelvic pain symptoms and infertility. Ovarian cysts (endometriomas) are the most common localization of endometriosis in the pelvis. Considering non-invasive methods, transvaginal ultrasound has high sensitivity and specificity for endometrioma diagnosis. Laparoscopic removal of endometrioma is related to a damage to the ovarian reserve and should be limited to patients with suspicious cysts or unresponsive to medical treatment. The main goal of medical th...

Update on new imaging technologies in sentinel node detection.
Authors: Pelligra S, Scaletta G, Cianci S, Alletti SG, Restaino S, Fagotti A, Scambia G, Fanfani F Abstract In most cancers, lymph node status is the most critical factor impacting the evolution of the disease and the overall survival. Identifying potential nodal metastasis allows the oncologist to adjust the stage and, consequently, the patient's treatment. For this reason, a precise evaluation of the regional nodes is mandatory. In gynecological cancers, pelvic, paraaortic, and inguinal nodes are the region most frequently interested by metastasis. In the past years, comprehensive lymphadenectomy was the...

Fertility and pregnancy outcomes following hysteroscopic metroplasty of uterine septa: an update.
Authors: Carugno J, Alonso L Abstract The septate uterus is still a matter of debate and discussion today in different medical forums. The controversy is divided between 2 different quite divergent opinions, on the one hand that of those who promote its surgical correction and on the other that of those who advocate expectant management with no recommended surgical intervention. It is extremely difficult to determine the association between septum and infertility. Infertility is a multifactorial condition, furthermore, the mechanism by which the septate uterus is associated with infertility is still unknow...

Minimally invasive surgery in cervical cancer.
Authors: Bogani G, Di Donato V, Chiappa V, Lopez S, Monti M, Muzii L, Benedetti Panici P, Ditto A, Raspagliesi F Abstract In recent years, minimally invasive surgery has replaced open surgery for almost all surgical indications in gynecological practice. Recently, the results of the Laparoscopic Approach to Cervical Cancer (LACC) trial questioned the role of minimally invasive surgery for patients affected by earlystage cervical cancer. In the present paper, we discussed the current evidence regarding the adoption of minimally invasive surgery for patients with cervical cancer. We evaluated the current evi...

The complex relationship between female age and embryo euploidy.
CONCLUSIONS: Female age is the strongest predictor of embryo chromosomal abnormalities and has a non-linear relationship with the blastocyst euploidy rate. Other factors related to both the male and female subjects may only minimally affect this outcome. PMID: 33306288 [PubMed - as supplied by publisher] (Source: Minerva Ginecologica)

Reducing the radicality of surgery for vulvar cancer: are smaller margins safe?
CONCLUSIONS: A surgical resection margin of 2-3mm does not appear to be associated with a higher rate of local recurrence than the widely used limit of 8mm. As such the traditional practice of reexcision or adjuvant radiotherapy on the basis of 'close' surgical margins alone needs to be closely evaluated, since the attendant morbidity associated with these procedures may not be outweighed by oncological benefit.
