Sonographic Assessment of Ovarian Embryonal Cell Carcinoma
Ovarian embryonal cell carcinoma is an uncommon ovarian germ cell tumor. This case study is a report of a patient referred for sonography based on abdominal pain and abnormal levels of alpha-fetoprotein (AFP) and β-human chorionic gonadotropin (β-hCG). A large heterogeneous pelvic mass was characterized by sonography and was surgically proven to be a malignant ovarian embryonal cell carcinoma. (Source: Journal of Diagnostic Medical Sonography)

First Trimester Diagnosis of Omphalocele: Differentiating Between Omphalocele and Normal Physiologic Gut Herniation
Normal physiologic herniation of the fetal bowel is often seen on first trimester sonogram. An omphalocele is an abdominal wall defect that occurs as a result of herniation into the umbilical cord and is often associated with chromosomal anomalies. This case report presents an omphalocele seen in the first trimester and discusses how to differentiate this finding from normal physiologic bowel herniation. Sonographic Detection of Starry Sky Liver in a Pregnancy With Twin-Twin Transfusion Syndrome
Twin-twin transfusion syndrome (TTTS) is a severe complication of monochorionic twin pregnancies. A starry sky liver is characterized by sonography as liver parenchyma appearing hypoechoic due to swelling of liver cells, accentuating the periportal collagen. It has been associated with several pathological processes but has only recently been reported with TTTS. This case study is a report of a twin pregnancy that developed TTTS in which a starry sky liver was visualized in the recipient twin. (Source: Journal of Diagnostic Medical Sonography)

Real-Time Elastography in the Follow-Up of Fibromatosis Colli
Fibromatosis colli is a rare disease related to congenital muscular torticollis that responds well to therapy if diagnosed and treated early. Gray-scale ultrasonography is used for diagnosis and follow-up, but with limitations. A novel technique, real-time elastography, might prove helpful for evaluating the response to therapy. A case is presented of fibromatosis colli diagnosed with sonography and real-time elastography in the first month of life. The baby was treated with physiotherapy for 3 months, and therapy was ended on the basis of real-time elastographic findings. Real-time elastography has been shown to be useful...

Sonographic Evaluation of Fournier's Gangrene
Fournier’s gangrene is a rare type of necrotizing fasciitis specific to the superficial fascia and subcutaneous tissues of the external genitalia, perineum, perirectal, and ischiorectal regions. It is usually diagnosed clinically, and imaging is used to support the diagnosis, assess the extent of the necrosis, and aid in surgical planning. Computed tomography (CT) is the primary imaging modality to diagnose Fournier’s gangrene, usually after symptoms are severe. Sonography may be used to diagnose early stages of this disease and to differentiate it from intrascrotal pathology or benign cellulitis. A case of Fou...

Sonographic Evaluation of Mobile Echogenicities Within the Epididymis
Current ultrasound systems have significantly improved spatial and temporal resolution secondary to increased transducer frequencies, increased bandwidth, and complex signal processing such as harmonics and compound scanning. The case presented describes the differential diagnoses of small, linear, mobile echogenic foci visualized by sonography in the left epididymal tail. Differentiation of moving parasites associated with filariasis versus mobile sperm may become more difficult as the resolution of ultrasound equipment continues to improve. Multiple Targetoid Lesions in the Spleen: Sonographic Findings of Chronic Lymphocytic Leukemia and Their Relation to Effective Staging
Chronic lymphocytic leukemia (CLL) is the most prevalent hematologic malignancy of adults in the western hemisphere. Involvement of the spleen is not uncommon, often presenting as diffuse enlargement due to infiltration of neoplastic cells. A case is presented of an 81-year-old woman with a past medical history of CLL who was incidentally found to have multiple, focal splenic infiltrates on sonography. The patient had a normal sized spleen, which is a less common occurrence with splenic invasion by CLL. As current CLL guidelines discourage imaging but rely on evidence of organ involvement, this finding was critical in stag...

The Role of CTA, MRA, and Sonography in Aortic Dissection
Aortic dissection is an emergent condition requiring rapid diagnosis and immediate treatment. Patients with aortic dissection typically present with complaints of a sudden onset of severe abdominal and back pain. Patients with a suspected dissection may undergo a variety of imaging tests such as computed tomography angiography, sonography, and/or magnetic resonance angiography. The role of these diagnostic imaging modalities for aortic dissection is examined in detail in this literature review. These imaging modalities are compared and contrasted by reviewing their advantages, disadvantages, and overall effectiveness for a...

At the Heart of the Matter: An Overview of Adult Echocardiography for the Non-Cardiac Sonographer
The objective of this article is to provide the non–cardiac sonographer with an overview and appreciation of the basic principles and practices of echocardiography. It is not meant as a guide to scanning but rather, as a vehicle to spark the interest and imagination of the reader. (Source: Journal of Diagnostic Medical Sonography)

A Mixed-Method Approach to Evaluating the Association Between Myofascial Trigger Points and Ankle/Foot Pain Using Handheld Sonography Equipment: A Pilot Study
This mixed-method blinded research study used high-frequency diagnostic medical sonography (DMS) to document myofascial trigger points (MTrPs) associated with ankle/foot pain. A total of 17 symptomatic and 8 asymptomatic participants provided 500 MTrP areas for palpation. Forty-nine of these MTrP areas (including 16 tender points, 15 palpable MTrPs, and 18 palpated and imaged MTrPs) were compared with the patient’s ankle/foot pain, collected with the SF-12 Health Survey, Victorian Institute of Sports Assessment–Achilles questionnaire, and a visual analog scale. Qualitative analyses of the participants’ hi...
