Complete Unbalanced Atrioventricular Septal Defect With a Common Arterial Trunk in an Uncontrolled Type 1 Diabetic Mother: A Case of Diabetic Embryopathy
When performed by a trained sonographer, fetal echocardiography can be instrumental in identifying cardiac pathology and abnormal cardiac physiology before a baby is born. Even though the diagnostic accuracy of a fetal echocardiogram can be limited by maternal body habitus, gestational age, and fetal position, sequential exams allow for detailed analysis of the cardiac structures. Early identification and diagnosis of intrauterine cardiac anomalies and subsequent pregnancy management and delivery planning are essential to providing the best possible outcome for fetuses born with heart defects. In this case study, we descri...

Sonographic Detection of Ovarian Ectopic Pregnancy: A Case Study
This report documents one such uncommon case of ovarian pregnancy in a woman presenting with light vaginal bleeding in early pregnancy, with subsequent rupture and laparotomy. Histopathologic analysis confirmed the diagnosis of ovarian ectopic pregnancy. (Source: Journal of Diagnostic Medical Sonography)

Congenital Pulmonary Airway Malformation: A Case Study and Case Comparison
Congenital pulmonary airway malformation (CPAM) is a rare congenital lung mass of the fetus that can present as solid or cystic. This is often diagnosed prenatally with sonography and routinely followed through the term of the pregnancy. Congenital pulmonary airway malformation is now classified into five different types that all originate from different areas of the lung and can vary in appearance. CPAM does present with a few differentials that need to be assessed, including bronchopulmonary sequestration, bronchogenic cyst, and congenital diaphragmatic hernia. It is vital to fully assess the fetal thorax and determine i...

Decidualized Endometriosis: Case Study of a Rare Condition in a Rare Location
Decidualized endometriosis is a benign condition associated with ectopic endometrial tissue that has undergone a decidual reaction during pregnancy. A case is presented of a 31-year-old female who presented to an imaging center for a follow-up sonogram after experiencing one episode of vaginal bleeding during pregnancy. A solid, highly vascular mass, with irregular borders, was seen that was not present in the sonogram two weeks prior. The mass was later revealed to be decidualized endometriosis. This is a rare condition that lacks extensive reporting in the literature. The case demonstrates a particularly odd location for...

Sonographic Assessment of Uterine Dehiscence During Pregnancy in Women With a History of Cesarean Section: A Case Series
Uterine scar dehiscence is a complication seen in pregnancy commonly from previous cesarean scars. It increases the risk for uterine rupture, which carries a high incidence for maternal and neonatal morbidity and mortality. Ultrasound is an effective method to monitor the defect closely throughout pregnancy to help predict uterine rupture or neonate complications. Repair of a dehiscence may be possible, allowing for future pregnancies. (Source: Journal of Diagnostic Medical Sonography)

Sonographic Evaluation of a Pilonidal Cyst: A Case Study
Pilonidal cysts are collections of hair or skin debris located most commonly on or near the natal cleft of the buttocks. These cysts can become infected and cause swelling and bleeding at the site. Once infected, the condition is then termed a pilonidal abscess, and treatment should be followed. Several options are available for treating a pilonidal abscess, depending on the extent of the condition and the patient’s response to antibiotic therapy. This case study of a pilonidal abscess demonstrates the important information that sonography adds to the diagnosis and treatment of this condition. (Source: Journal of Dia...

Situs Inversus With Liver Metastases: Diagnosis With Sonography and Computed Tomography
Situs inversus totalis is a rare condition of the organs being transposed within the abdominal and chest cavities. The cause of situs inversus is unknown, but it is a recessive trait. Complete situs inversus may be associated with other malformations, such as levocardia, dextrocardia, and Kartagener syndrome; congenital heart disease may be present in some cases. The case presented describes a patient with situs inversus totalis with detection of liver metastases diagnosed by 2D sonography, computed tomography, and biopsy. Intrahepatic Cholangiocarcinoma (ICC) Detected by Sonography
Intrahepatic cholangiocarcinoma (ICC) is a malignant disease process that develops within the small branches of the biliary system within the liver. It is one of two types of cholangiocarcinoma (CCA). The patient in this case study experienced general abdominal and pelvic discomfort when further sonographic evidence proved there to be a moderate amount of fluid within the pelvic and abdominal cavities, suggesting a malignant process. Three types of growth processes related to ICC require different methods of treatment, thus making it imperative for the sonographer to identify the origin of the disease process. Appropriate ...

Combined Sonography: Surgery Clinic for Patients Referred With Thyroid Nodules
We report the results of a joint thyroid clinic with a consultant surgeon and consultant radiologist to offer "triple assessment" for patients on a single visit. All new referrals from September 2011 to December 2013 were reviewed. Among 228 patients referred, 163 patients underwent ultrasonography (US) on the same visit and 66 fine needle aspiration cytology (FNAC) studies (57 under US guidance) were done. One hundred ninety-eight (87%) patients made a single visit to the clinic for diagnosis. One hundred twenty-five were discharged or referred elsewhere after the visit and 21 underwent surgery; the remaining 52 patients ...

Assessment of the Fetal Heart During Routine Obstetrical Screening, a Standardized Method
Assessment of the fetal heart during routine obstetrical screening remains a challenge for sonographers and physicians. Reliance on still images and nonstandard methods of acquiring images and assessing the fetal heart contribute to the relatively low rate of identification of congenital heart disease (CHD). A standardized assessment of the fetal heart using two cine-loop sweeps has been shown to address some of these challenges. Image acquisition using two cine-loop sweeps combined with a standardized five-step assessment is proposed to address the limitations of the nonstandard approach in place at most screening centers...

Incidental Finding: LAD to RV Fistula
Congenital coronary arteriovenous fistula is a rare anomaly that is usually found incidentally by echocardiogram or by angiography. A case is presented of an asymptomatic male infant with a continuous heart murmur that was referred to pediatric cardiology. Echocardiographic findings of a coronary arteriovenous fistula are shown and treatment discussed. (Source: Journal of Diagnostic Medical Sonography)

Simultaneous Lung Point and Lung Pulse in Pneumothorax
Sonography of the lung has been shown to be a useful bedside diagnostic tool in critical care units for pneumothorax (PNX). A case of PNX is reported in which all the standard sonographic findings were present but coexisting with the lung pulse sonographic sign. Descriptions of the sonographic findings of PNX, the lung point sign, and the lung pulse sign are given and discussed, with the characteristics that should be used to avoid misdiagnosis.
