Celiac Artery Compression Syndrome: A Literature Review
Celiac artery compression syndrome, also known as median arcuate ligament syndrome and Dunbar syndrome, is a rare cause of chronic mesenteric ischemia. Ischemia results from external compression of the proximal celiac artery during respiration, typically during expiration, by the median arcuate ligament just below the diaphragm. The author reviews the current literature to highlight the most common signs and symptoms, the imaging features, and the treatment options associated with celiac artery compression syndrome. Evaluation of Shear Stress in the Popliteal Artery in Patients With Type 2 Diabetes Mellitus Using Vascular Sonography
The aim of this study was to use vascular sonography to evaluate the dynamic forces acting on the wall of the popliteal artery in patients with type 2 diabetes mellitus (T2DM) compared to a control group without diabetes. Forty patients with T2DM and 20 control subjects were enrolled in the study. Patients were divided according to their measured Ankle-Brachial Index (ABI) into two groups, those with evidence of peripheral vascular disease (PVD) based on an ABI < 0.9 (n = 18) and those without peripheral vascular disease (N-PVD, ABI ≥ 0.9, n = 22). The systolic and diastolic internal diameter (IDs, IDd) and the end-d...

Comparison and Accuracy of Carotid Plaque Analysis Between Two- and Three-Dimensional Ultrasound Imaging
Plaque characterization using traditional two-dimensional (2D) imaging and/or three-dimensional (3D) ultrasonographic (US) techniques is a new method for evaluating artery wall morphology and plaque risk stratification. The purpose of this study was to assess and compare 2D and 3D US, measuring the interobservation differences for specific plaque-imaging analyses. Phantoms that simulated various types of atherosclerotic plaque pathology were imaged and findings reported by three experienced sonographers. Interobservation agreement and subanalyses were created. For each type of plaque pathology, agreement was moderate; howe...

Health Care Students Who Frequently Use Facebook Are Unaware of the Risks for Violating HIPAA Standards: A Pilot Study
Social networking creates easy opportunities to violate HIPAA (Health Insurance Portability and Accountability Act). The purpose of this study is to determine if students who frequently update their Facebook statuses have the ability to identify certain Facebook postings as HIPAA violations. An anonymous survey was distributed to students on a university campus of a health sciences center, containing questions related to how often Facebook was used or accessed, how often students updated their Facebook statuses, and whether they could identify if specific online postings constituted HIPAA violations. Students’ HIPAA ...

Incidental Findings in Stress Echocardiography: Embolization of an Iliofemoral Stent
Sonographic follow-up of surgical or other invasive procedures may lead to incidental findings, and knowledge of a patient’s treatment history is an important factor in properly interpreting these results. The treatment regimen for a patient with deep vein thrombosis and its sequelae can include a combination of anticoagulation, thrombolytic therapy, compression stocking, venous bypass, and stenting of the veins. This case presents an incidental finding during a stress echo examination of embolization into the right ventricle of a previously placed iliofemoral venous stent that required surgical removal. It provides ...

Sonographic Detection of a Heterotopic Pregnancy in Natural Conception
Visualization of a first trimester pregnancy, whether normal or abnormal, is best done using sonography. Heterotopic pregnancies, the combination of an intrauterine and an extrauterine pregnancy, provide a difficult diagnosis as the intrauterine pregnancy gives a sense of false assurance. This case study presents a spontaneous heterotopic pregnancy with one viable intrauterine pregnancy and one ruptured ectopic pregnancy and discusses the role that sonography plays in the detection and management of this problem. Prenatal Diagnosis of Bladder Exstrophy
Bladder exstrophy is a rare congenital abnormality where the urinary bladder, anterior pelvis, and genitalia fail to close in the midline, resulting in the bladder being located externally along the anterior pelvic wall. The incidence is 1 per 40 000 live births and has a male predominance. Common prenatal sonographic findings include an absent urinary bladder, normal amniotic fluid, and an inferior abdominal mass. The absence of the bladder in the second trimester with normal amniotic fluid volumes should prompt evaluation for this abnormality. Advances in ultrasound technology including the use of 3D and 4D imaging have ...

Longitudinal Analysis of Median Nerve Perfusion Using Spectral Doppler in a Working Animal Model
The objective of this study was to longitudinally quantify the amount of perfusion of the median nerve using spectral Doppler in a working animal model, as the cohort prospectively increased their risk of MMN through a repetitive pinching task. The results of the study provided low-level evidence that the use of spectral Doppler to accurately detect longitudinal changes in vascularity within the median nerve may be diagnostically inadequate. As the subjects in the study moved through the working and nonworking stages of study, spectral Doppler alone proved to lack the level of sensitivity needed to detect physiologic chang...

Upper Extremity Kinematics in Sonographers During Kidney Scanning
To understand the mechanisms involved that lead to the high prevalence of work-related musculoskeletal disorders in sonographers, this motion capture study evaluated upper extremity kinematics during kidney scans. The purpose of this study was to provide an evaluation of joint ranges of motion during scanning, which could be helpful in assessing risk of injury. A Vicon MX motion capture system recorded reflective marker positions while four sonographers scanned a volunteer’s kidneys. The results indicated that sonographers were scanning with their shoulders in flexion, abduction, and external rotation. The elbows wer...

A Simple Effective Protocol to Increase Prenatal Detection of Critical Congenital Heart Disease
Prenatal diagnosis of congenital heart disease (CHD) during routine obstetric sonography has been aptly named the sonographer’s Achilles heel. Although CHD occurs more commonly than any other major congenital abnormality, the detection rate remains low. The goal of this study was to improve the prenatal diagnosis of CHD during routine obstetric sonography through the development and implementation of a simple and effective screening protocol.
