Sonography of Isolated Internal Jugular Vein Impingement and Thrombosis
This case illustrates incidental findings of internal jugular vein (IJV) impingement and thrombosis caused by a metastatic tumor. Sonography of an asymptomatic adult female demonstrated nonocclusive deep venous thrombosis in the left IJV and an impinging vascular mass. Contrast-enhanced computed tomography confirmed compression, displacement, and obstruction of the IJV and metastasis. This case highlights the utility of sonography to identify unsuspected pathologies. (Source: Journal of Diagnostic Medical Sonography)

Common Carotid Artery Occlusion With Retrograde Flow in the Internal Carotid Artery: A Case Report
In most cases of common carotid artery (CCA) occlusion, the internal carotid artery (ICA) is also occluded. This case presents a patient with a patent ICA distal to a thrombotic CCA occlusion, likely secondary to cardiac embolization related to chronic atrial fibrillation, with retrograde filling of the extracranial ICA via intracranial collateral flows. The Impact of Education: Has the Failure to Standardize Musculoskeletal Sonography Undermined Its Value?
Sonography has made substantial technological improvements over the years, removing many of the barriers to its application in evaluation of the musculoskeletal (MSK) system. Sonography offers the advantages of a dynamic, real-time evaluation that routinely incorporates patient movement, evaluation of MSK structures in symptomatic positions with real-time feedback from patients, and a patient-friendly, noninvasive imaging modality. However, educational standards and efficacy of musculoskeletal ultrasonography (MSUS) have demonstrated as much variance as published reports of MSUS accuracy and interobserver agreement. This r...

Imaging of Urethral Bulking Agents: A Sonographer's Perspective
Urethral bulking is an office procedure for treatment of stress urinary incontinence that has increased in popularity in recent years. Numerous types of urethral bulking agents have been used since its introduction in the 1930s. Typically, these agents are imaged using specialty 3D 360° field-of-view transducers; however, a sonographer, during a pelvic or renal examination, may inadvertently find and image an echogenic structure near the urethrovesical junction in patients with a history of urethral bulking. Familiarizing oneself with the sonographic appearance of urethral bulking agents and optimizing techniques to ob...

Variation of Pinch and Grip Force Between Different Size Transducers: A Preliminary Study
Work-related musculoskeletal disorders affect a large percentage of diagnostic medical sonographers. Ultrasound transducer design has been identified as a major contributing factor to hand and wrist strains. The purpose of this study was to determine if the design of the ultrasound transducer affects the amount of pressure exerted by the hands of the sonographer to achieve a quality scan. The study also aimed to determine if the amount of pressure exerted on the transducer relates to the subjective pain level reported by the sonographer. The study found that the average maximum pressure and average force exerted was higher...

Evaluation of Rex Shunts: What Sonographers Need to Know
The purpose of the Rex shunt is to restore normal blood flow to a hemodynamically compromised liver that is otherwise normal. It has proven to be an effective treatment for children with extrahepatic portal vein thrombosis as in the case presented. The shunt allows blood from the superior mesenteric vein to bypass the obstructed extrahepatic portal vein and enter the liver through the still patent left portal vein. The operation is successful in more than 90% of patients. The name "Rex shunt" is derived from the location in the liver in which the shunt is placed, the area between segments three and four of the left lobe of...

Internal Iliac Artery Aneurysm Detected by Sonography
Internal iliac artery aneurysms are rare and seldom imaged with sonography in an acute situation. This case study illustrates a ruptured internal iliac artery aneurysm and shows the correlation of sonographic images with computed tomographic images in an emergent setting. Partial Testicular Torsion and Torsion-Detorsion Syndrome
Testicular torsion (TT) is a common cause of acute scrotal pain in young men and accounts for as many as 26% of cases of acute scrotum. Accurate diagnosis and differentiation of TT from acute epididymo-orchitis is essential because TT is treated surgically and epididymitis with or without orchitis is treated medically. Distinguishing TT from epididymo-orchitis is generally a clinical dilemma. Sonography remains the best imaging modality for these entities, but the sonographic findings can be complex and can mimic complicated epididymo-orchitis when dealing with partial TT and torsion-detorsion syndrome. This case report il...

Examination of Intracranial Translucency as a Marker for Early Detection of Open Spina Bifida
This article focuses on the sonographic examination technique for evaluation of intracranial translucencies during the first trimester scan. Appropriate knowledge of the sonographic landmarks is the most important factor for evaluating normal as well as abnormal brain structure, and this examination may enable detection of open spina bifida at an early stage of fetal development.
