A Novel Echocardiographic Marker (PDAM) Incorporating the Diameter of the Patent Foramen Ovale to Assess Hemodynamically Significant Patent Ductus Arteriosus
This report proposes a new composite marker to predict a hemodynamically significant PDA, termed the PDA marker (PDAM) score, which incorporates the diameter of the patent foramen ovale (PFO) in addition to the ratio of the diameter of the left atrium to the diameter of the root of the aorta (LA/Ao) and ratio of the size of the patent ductus arteriosus to the left pulmonary artery (PDA/LPA) diameter. PDA was suspected in 191 infants between 2009 and 2013. Analysis of the data indicated that there was no significant difference between the accuracy of the PDA diameter and the PDAM score as markers of either a hemodynamically...

Echocardiographic Markers for the Prediction of Nonclosure of the Patent Ductus Arteriosus in Premature Neonates
Four echocardiographic markers—left ventricular ejection fraction (LVEF), the ratio of the left atrial diameter to the aortic root diameter (LA/Ao), the ratio of the patent ductus arteriosus (PDA) diameter to the left pulmonary arterial diameter (PDA/LPA), and the patent foramen ovale (PFO) diameter—were explored to predict a PDA in 221 premature neonates (53% male). There were 182 cases with PDA (mean diameter = 2.49 mm); 39 control subjects were observed with a closed ductus arteriosus. Although 104 of the cases (57.1 %) were diagnosed when 0 to 3 days old (median age = 1 day), 78 (42.9%) were diagnosed when ...

An Absent Right Umbilical Artery Versus Absent Lt and the Prognostic Implications for the Fetus
A single umbilical artery (SUA) is one of the most common malformations in a fetus, with approximately a 1% occurrence rate. A retrospective study of 108 fetuses with a SUA was done to determine if the laterality of the SUA made a difference in fetal prognosis. An absent right umbilical artery was found in 42 (38.9%) of cases and an absent left umbilical artery in 66 (61.1%) cases. When correlated with fetal outcome, there was a closer association between an absent right umbilical artery with genitourinary anomalies, cardiac defects, and chromosomal abnormalities. There was a nine times greater likelihood (95% confidence i...

Role of Socioeconomic Status in Predicting Sonographically Detected Breast Cancer Compared With Family History, Age, or Ethnicity
To determine if socioeconomic status is another risk factor in the development and treatment of breast cancer, the records of 250 women, equally divided between those with and without breast cancer, were studied at an outpatient clinic. The frequency of sonographically detected breast cancer increased systematically with respect to age, from 0% at age <30 years to 70.5% at age ≥50 years. The frequency of detection was higher among white (non-Hispanic) women, 69.4%, compared with nonwhite (Hispanic, Black, Asian) women, 42.3%. Breast cancer detection was higher (55.3%) among the lowest income group (<$40,345) than ...

Incidental Finding of Cor Triatriatum Sinistrum in an Adult
Cor triatriatum is a congenital heart defect often diagnosed in infancy or childhood. This case study presents an adult with metastatic breast cancer, anemia, and an incidental finding of cor triatriatum sinistrum. On the basis of clinical presentation, the patient was treated conservatively and discharged. Sonographic findings of cor triatriatum sinistrum, along with clinical signs, the significance of other imaging modalities, and potential treatments, are presented. The Incidental Finding of Hepatocellular Carcinoma by Echocardiography
Hepatocellular carcinoma (HCC) is the most common type of primary hepatic malignancy. Most cases of primary hepatic carcinoma are secondary to either viral hepatitis infections (hepatitis C or hepatitis B) or cirrhosis, with chronic alcohol abuse being the most common cause of hepatic cirrhosis. Echocardiography is done primarily to evaluate the overall function of the heart; the condition of the heart muscle, heart valves, and chamber sizes; and the risk for heart disease. It is also used to evaluate the effectiveness of medical or surgical treatment over time. The case presented emphasizes the importance of performing a ...

Sonographic Evaluation of a Cavoportal Hemitransposition Liver Transplant: A Case Series
Extensive portal vein thrombosis is no longer considered a contraindication for liver transplantation. With cavoportal hemitransposition (portal diversion) or other surgical techniques, patients with portal vein thrombosis who would otherwise succumb to their liver disease are able to receive a transplant. A series of three cases is presented describing the evaluation and liver transplant procedures in patients with portal vein thrombosis. Sonography is shown to be an important tool both for diagnosing portal vein thrombosis and evaluating the portal system following transplantation. (Source: Journal of Diagnostic Medical Sonography)

Duplex Ultrasound Findings of Popliteal Artery Aneurysms With Acute Limb Ischemia
Popliteal artery aneurysms are rare, but they have the potential to be limb threatening as a result of thrombosis and distal embolization. Duplex sonography may be used to effectively detect popliteal artery aneurysms and determine patency of the runoff arteries. Findings may include dilatation of the popliteal artery during gray-scale imaging with intraluminal heterogeneous echoes indicating the presence of mural thrombus. Abnormal or absent Doppler signals in the more distal arteries with intraluminal heterogeneous echoes may indicate the presence of thrombus, likely from embolization. This case presents the duplex sonog...
