The value of etiological tests and skin biopsy in the management of prurigo simplex subacuta
It has been emphasized that prurigo simplex subacuta may be associatedwith various disorders and may be clinically confused with dermatitis herpetiformis.Aim of the study was to evaluate the value of our set of tests for findingpossible associations and for the aforementioned differential diagnosis. Rates ofimplementations and abnormal results in our set of tests, including skin prick test, skin biopsy, complete blood count, blood chemistry panel, and psychiatric evaluationwere analyzed retrospectively between the years of 2010 and 2015 in 130patients clinically diagnosed with prurigo simplex subacuta. The set of tests wasi...

Minoxidil acts as an antiandrogen: a study of 5 α-reductase zype 2 gene expression in a human keratinocyte cell line
Although more than three decades have passed since the first useof minoxidil in androgenetic alopecia (AGA), its mechanisms of action havestill not been comprehensively understood. 5 α-reductase (5α-R) has an activerole as the predominant enzyme in both AGA and female pattern hair loss(FPHL), which are also the main therapeutic indications of topical minoxidil. But there is insufficient literature data regarding the interaction of minoxidiland the enzyme 5α-R. Herein, we studi ed the in vitro expression levels of 5α-Rtype 2 (5α-R2) in a minoxidil-treated human keratinocyte cell line (HaCaT) inorder to elucidate the rel...

The role of CD247 polymorphisms in Bulgarian patients with systemic lupus erythematosus
Decreased expression of the TCR ζ-chain has been reportedin several autoimmune and inflammatory diseases. Recent evidence suggeststhat this deficiency may be due to polymorphisms in the CD247gene. A total 52 patients with systemic lupus erythematosus (SLE) and95 healthy controls of Bulgarian ethnicity were genotyped for 837C>G,rs1052230, 844A>T, and rs1052231 using a TaqMan genotyping assay.None of the two polymorphisms appeared associated with the diseases. On the other hand, we have found that the -837GG genotype and the Gallele were associated with hematological disease. The -844AA genotypeand the A allele appeare...

Bevacizumab-associated sudden onset of multiple monomorphic comedones on the scalp successfully treated with 30% salicylic acid peels.
Bevacizumab is a humanized monoclonal antibody against vascular endothelial factor (VEGF) that targets tumor cell angiogenesis and proliferation. Although it is usually well tolerated, many side-effects have been reported. These include hypertension, bleeding, and thromboembolic events among others. Drug-associated cutaneous adverse effects are less common and include itching, exfoliative dermatitis, and acneiform eruptions. A man with bevacizumab-associated monomorphic skin eruption successfully was treated with 30% salicylic acid peels. To the author ’s knowledge, this is the first report of open comedones with no furt...

Long lasting allergic patch test reactions: a literature review
Long-lasting allergic patch test reactions (LLAPTR) are those in which the clinical features of palpableerythema are still present at the site of a positive allergic patch test reaction 14 or more days after application of the allergen. LLAPTR have been described for a wide range of contact allergens, many of these included in the baseline patch test series. LLAPTR are far from uncommon; they occur in consecutive patients with positive patch tests to baseline allergens with frequency up to 17.9% of the total reactions. Patch test reactions persisting for a very long time (up to several months after the test) have been desc...

Aquagenic urticaria: A perplexing physical phenomenon
Aquagenic urticaria (AQ) is a rare physical urticaria induced by contact with water. It may be distinctive clinically, evident as 1-2 mm folliculopapular urticaria, a pattern also characteristic of cholinergic urticaria. AQ has a truncal and upper extremity distribution within 20-30 minutes after contact with water, regardless of its temperature or source. AQ is usually symptomatic with mild to severe pruritus and a burning sensation. The mechanism by which water produces mast cell degranulation and histamine release remains unclear. We review its clinical presentation, diagnostic parameters, differential diagnosis, and tr...

Bilateral sacroiliitis confirmed with magnetic resonance imaging during isotretinoin treatment: Assessment of 11 patients and a review of the literature
In recent years, several cases pointing at sacroiliitis due to isotretinoin treatment have been reported, but a causal association remains unproven. The aim of this study was to assess the characteristics of patients in whom bilateral sacroiliitis was detected while using isotretinoin treatment for acne and to review previous sacroiliitis cases treated with isotretinoin. In total, 11 patients who were diagnosed with sacroiliitis during isotretinoin treatment were identified, and patient characteristics were noted. Patients were classified according to magnetic resonance imaging (MRI) findings. The 11 patients (3 men and 8 ...

Real-time investigation of skin blood flow changes induced by topical capsaicin
In this study, we investigated real-time skin blood flow changes that occur in neurogenic inflammation induced by topical capsaicin by means ofin vivo reflectance confocal microscopy. 27 healthy subjects (15 women and 12 men, mean age ± Standard Deviation: 22.62±4.47) were administered topical capsaicin solution (Capsaicin group) or immersion oil (Control group) on the dorsal side of their non-dominant hand. At different time intervals during administration (0, 10, 25, and 40 minutes), cutaneous blood flow was evaluated using r eflectance confocal microscopy and compared between the two groups. Blood flow values were hig...
