Are general surgery residents adequately prepared for hepatopancreatobiliary fellowships? A questionnaire‐based study
ConclusionsOverall, HPB fellows felt themselves to be adequately prepared for fellowship. Advanced laparoscopic procedures and the perioperative management of complex patients are two of the challenges facing HPB fellows. General surgery residents who plan to pursue an HPB fellowship may benefit from spending extra rotations on certain subspecialties. Focus on perioperative workup and management should be an integral part of residency and fellowship training. (Source: HPB: official journal of the International Hepato Pancreat Biliary Association)

Is local resection adequate for T1 stage ampullary cancer?
ConclusionPatients with T1 AC have a high risk for nodal metastases especially if they are higher‐grade lesions. Nodal clearance with a lymphadenectomy or a PD is essential for long‐term survival in these patients. (Source: HPB: official journal of the International Hepato Pancreat Biliary Association)

A case‐matched comparison and meta‐analysis comparing pylorus‐resecting pancreaticoduodenectomy with pylorus‐preserving pancreaticoduodenectomy for the incidence of postoperative delayed gastric emptying
ConclusionsPylorus‐resecting pancreaticoduodenectomy is a safe procedure associated with less severe and less frequent postoperative DGE than PpPD. (Source: HPB: official journal of the International Hepato Pancreat Biliary Association)

Mortality after a cholecystectomy: a population‐based study
DiscussionMortality after cholecystectomy is low. Co‐morbidity and peri‐operative complications may, however, increase mortality substantially. The increased mortality risk associated with open cholecystectomy could be explained by confounding factors influencing the decision to perform open surgery. Clinical classification of Caroli's disease: an analysis of 30 patients
ConclusionsNo typical symptoms, signs or laboratory indicators are able to distinguish CD from other conditions. Both MRCP and CT were most valuable in diagnosis. The two types of CD may be differentiated by age of onset and routine blood tests. (Source: HPB: official journal of the International Hepato Pancreat Biliary Association)

Morbid obesity in liver transplant recipients adversely affects longterm graft and patient survival in a single‐institution analysis
ConclusionsObesity increasingly impacts outcomes in liver transplantation. Although the present data are limited by the fact that they were sourced from a single institution, they suggest that morbid obesity adversely affects longterm outcomes despite providing similar short‐term results. Further analysis is indicated to identify risk factors for poor outcomes in morbidly obese patients. (Source: HPB: official journal of the International Hepato Pancreat Biliary Association)

Laparoscopic versus open liver resection for hepatocellular carcinoma at a North‐American Centre: a 2‐to‐1 matched pair analysis
DiscussionLLR confers the widely‐accepted benefits of laparoscopic surgery, namely severity of complication, 30‐day readmission rate, LOS and blood loss. Further studies are required to examine intra‐ and extra‐hepatic recurrence after LLR. LLR for HCC should be considered for appropriately selected patients in centres with requisite volume and expertise. The inhomogeneous distribution of liver function: possible impact on the prediction of post‐operative remnant liver function
DiscussionThe present study underlines the necessity of a segmental liver function test able to compensate for the non‐homogeneous nature of liver function, if the prediction of post‐operative remnant liver function is to be improved.
