Update on the Inaugural Sydney Diabetic Foot Conference 2013
The first International Diabetic Foot Conference in Australia was hosted at Liverpool Hospital in Sydney during May 30-31, 2013. In response to the growing diabetes epidemic globally and more locally to Australia, the conference provided the perfect bridge for interaction between the multidisciplinary team members involved in diabetes care and the opportunity to assimilate the most up-to-date evidence-based medicine from some of the most respected researchers in the field. Secondary Prevention of Diabetic Foot Infections in a Caribbean Nation: A Call for Improved Patient Education
Most countries have instituted measures to limit the complications of diabetes. We evaluate secondary prevention strategies for diabetic foot infections in a Caribbean country. We performed a prospective questionnaire study evaluating all patients admitted to tertiary care hospitals across Trinidad and Tobago from July 2011 to June 2012. Primary study end points were the number of patient-detected injuries and the time interval between injury and presentation. Secondary end points included the practice of regular foot inspection (≥2 foot examinations per week) for early detection and the use of self-directed nonmedical ...

Shewanella putrefaciens, a Rare Cause of Osteomyelitis
Shewanella putrefaciens is a Gram-negative bacillus with a distinguishable characteristic of hydrogen sulfide production and routinely found in a marine environment. This organism has been cultured as a pathogen in a small number of soft tissue infections, but has rarely been the causative agent in osteomyelitis. This case report details calcaneal osteomyelitis due to S putrefaciens in a 77-year-old male with bilateral heel ulcerations and peripheral vascular disease. Heel Ulcer and Blood Flow: The Importance of the Angiosome Concept
A young female diabetic patient is reported, who presented with a double foot lesion. She presented with a first metatarsal head exposure concomitant with a heel wet gangrene. Magnetic resonance demonstrated osteomyelitis of the rear portion of the calcaneus. Transmetatarsal amputation was performed and a wide debridement was required to remove all gangrenous tissue from the heel wound. The pedal artery was palpable; the posterior tibial pulse was present, but weak.Transcutaneous oximetry (TcPO2) at the dorsum of the foot was TcPO2 = 56 mmHg despite significant oedema. Nevertheless, TcPO2 on the perilesional area of the he...

Case Series of Isolated Primary Persistent Sciatic Vein
This study concluded that in this persistent sciatic vein, the associated non-hypoplastic femoral vein is not uncommon, and care must be taken about this condition during ultrasonographic examination. Unusual causes of chronic venous insufficiency and other venous anomalies should not be overlooked. (Source: The International Journal of Lower Extremity Wounds)

Motorcycle Muffler-Induced "Pillion Burns" of Distal Leg: A Persistent Problem of Increasing Proportions in Indian Settings
Motorcycles have emerged as a viable mode of transport for millions in the third world. Mufflers (exhaust pipes in some countries) remain a potential "Achilles’ tendon" or a designing flaw in the mass-produced, economical motorcycles of the developing world. Owing to the excessive temperature they attain while the hot exhaust gases pass through them and their proximity to the lower limbs while riding a motorcycle, they can lead to burns of varying nature in the lower leg. This is a descriptive retrospective study of muffler-induced lower leg burns treated at our hospital from January 2008 to December 2012. Various pa...

Umbilical Cord Mesenchymal Stem Cell Transplantation Ameliorates Burn-Induced Acute Kidney Injury in Rats
In this study, UC-MSCs or ulinastatin was administered intravenously in the rats with burn trauma, and the therapeutic effects of UC-MSCs on the survival of severe burn-induced AKI rats and functional protection of kidney were analyzed. Results showed that UC-MSCs promoted the survival and prevented commitment to apoptosis of resident kidney cells and reduced organ microscopic damage in kidneys after thermal trauma. Thus, our study demonstrates that intravenously delivered UC-MSCs protected the host from death caused by kidney injury subsequent to severe burn, identifying UC-MSCs transplantation may be an attractive candid...

A Functional Single Nucleotide Polymorphism -1562C>T in the Matrix Metalloproteinase-9 Promoter Is Associated With Type 2 Diabetes and Diabetic Foot Ulcers
Impaired neovascularization is the hallmark of type 2 diabetes, which results in various macro- and microvascular complications and the development of foot ulcerations later in life. Matrix metalloproteinases (MMPs) are the key enzymes which influence matrix remodeling. Here, we aim to investigate that whether single nucleotide polymorphism (SNP -1562C>T) (rs3918242) in the promoter region of MMP-9 gene, which alters the transcriptional activity of MMP-9 is associated with type 2 diabetes and diabetic foot ulcers (DFUs). This case–control study was composed of 730 individuals, out of which 463 patients were with t...

The Clinical Effectiveness of Two Compression Stocking Treatments on Venous Leg Ulcer Recurrence: A Randomized Controlled Trial
Venous leg ulcers affect up to 3% of people aged 65 years and older, present significant pain and suffering, and are associated with long episodes of health care. These chronic wounds recur at rates of up to 69%. A double blind randomized controlled trial was conducted in a home nursing setting in Victoria, Australia to compare the effectiveness of a 23 to 32 mm Hg (moderate) and a 34 to 46 mm Hg (high) compression stocking treatment on venous ulcer recurrence. Participants (n = 100) were monitored for 26 weeks. Study wound recurrence was low (11.8%), and the average time to recurrence was 77.91 days. Adherence to treatmen...
