The Australian experience with the Bethesda classification system for thyroid fine needle aspiration biopsies
We present our experience of the Bethesda system in 2076 thyroid nodules from 1410 patients. Categories were as follows: 266 (12.8%) were category 1 (B1) non-diagnostic, 1551 (74.7%) category 2 (B2) benign, 97 (4.7%) category 3 (B3) atypia of uncertain significance, 98 (4.7%) category 4 (B4) suspicious for follicular neoplasm, 16 (0.8%) category 5 (B5) suspicious for malignancy and 48 (2.3%) category 6 (B6) malignant.Surgery was performed on 425 nodules from 315 patients. Malignancy rates in the target nodules were B1 4.2%, B2 0.26%, B3 9.3%, B4 15.3%, B5 79% and B6 100%. Twelve patients with B3 nodules underwent repeat FN...

In situ hybridisation for Epstein–Barr virus as a differential diagnostic tool for T- and natural killer/T-cell lymphomas in non-immunocompromised patients
SummaryEpstein–Barr virus (EBV) is a ubiquitous human herpesvirus infecting over 90% of humans and the infection persists for life. Although most people are asymptomatic, EBV infection may cause a continuous range of symptoms from transient to severe and protracted diseases depending on the immunological response of the individuals. EBV infects primarily B lymphocytes and rarely T and natural killer (NK) cells. It is implicated in around 1% of human tumours with the majority being haematological malignancies including Hodgkin lymphoma, B- and T-cell non-Hodgkin lymphomas, and immunodeficiency-associated lymphoproliferati...

Effect of uraemic status on immunofluorescence and enzyme-linked immunosorbent assays for detection of human herpesvirus type 8 antibodies
This study aimed to investigate whether this discrepancy is due to the effect of uraemic status.Plasma samples from 162 ESRD patients, taken immediately before and after haemodialysis, and 162 age and sex matched healthy controls were analysed for HHV-8 antibodies using both IFA and ELISA.HHV-8 seropositivities based on IFA and ELISA, both before and after haemodialysis, were significantly greater in ESRD patients than in healthy controls (p  (Source: Pathology)

The interactions of TGF-beta signalling pathway and Jagged2/Notch1 pathway induce acanthosis in lingual epithelia
SummaryThe aims of this study were to distinguish between the primary and secondary effects of TGF-β signalling disruption by Dox treatment in NTPDase2+ cells; and to investigate the interactions between TGF-β signalling and Jagged2/Notch1 pathway in regulating the expansion of tongue epithelia stem cells.Transgenic mice expressing rtTA from the mouse NTPDase2 promoter or K14 promoter were used to generate an inducible dominant negative TGF-β receptor type II (Tgfbr2) mutant model.Disruption of TGF-β signalling in NTPDase2+ cells initially inhibited the formation of filiform papillae but led to their regeneration over ...

Immunochromatographic antigen testing alone is sufficient to identify asymptomatic refugees at risk of severe malaria presenting to a single health service in Victoria
SummaryCurrent screening guidelines for malaria in new refugees include a combination of thick and thin film examination and immunochromatographic antigen test (ICT). However, as the prevalence of malaria in our population has decreased due to changing refugee demographics, we sought to determine if an ICT alone can reliably exclude malaria in our asymptomatic refugee population.A retrospective analysis was conducted of all investigations for malaria performed from 1 August 2011 to 31 July 2013, including thick and thin blood film examination, BinaxNOW ICT, and external morphological and polymerase chain reaction (PCR) val...

Evaluation of Enterococcus faecalis clinical isolates with ‘penicillin-resistant, ampicillin-susceptible’ phenotype as reported by Vitek-2 Compact system
SummaryIt has been recently reported that ampicillin susceptibility cannot accurately predict piperacillin and imipenem susceptibilities in penicillin-resistant, ampicillin-susceptible (Pen-R, Amp-S) Enterococcus faecalis isolates, contrary to the current Clinical and Laboratory Standards Institute (CLSI) recommendations. This has important therapeutic implications. Such isolates were noted after the use of Vitek-2 Compact system AST-GP67 susceptibility cards in a Singapore general hospital and they were increasing in numbers. The primary aim of this study was to evaluate these clinical isolates against microbroth dilution...

A survey of Australian haematology reference intervals
This study was designed to create a snapshot of Australian haematology reference intervals (RIs) in use, in particular red cell parameters. We present an analysis of survey results conducted across Australian laboratories between November 2012 and January 2013.All Australian laboratories enrolled in the Royal College of Pathologists of Australasia Quality Assurance Program (RCPA QAP) were invited to participate in the December 2012 Survey Monkey survey, with a response from 85 laboratories (17%) received. The scope included laboratory demographics (location, size/throughput, and network), RIs in use for the full blood coun...

A comparison of high resolution melting, allele-specific priming and Sanger sequencing for the detection of BRAFV600E mutation in hairy cell leukaemia from different haematological specimens
SummaryThe BRAFV600E mutation is a highly sensitive and specific marker for the diagnosis of hairy cell leukaemia (HCL) and a potential therapeutic target. We assessed the performance of high resolution melting (HRM), allele-specific priming (ASP) and Sanger sequencing (SS) for BRAFV600E detection in 17 unenriched samples from 15 HCL patients: blood (n = 7), marrow aspirate (n = 3), ethylenediaminetetraacetic acid (EDTA)-decalcified trephine biopsy (n = 2), formic acid (FA)-decalcified trephine biopsy (n = 5). Our results showed that for blood and marrow aspirate samples, both HRM and ASP had a very high an...
