Treatment of Lumbar Spinal Stenosis by Microscopic Unilateral Laminectomy for Bilateral Decompression: A Technical Note
Lumbar spinal stenosis is typically a degenerative condition that leads to compression of the spinal canal and lateral recess, resulting in leg pain and walking disability. Surgical management is indicated after failure of non‐surgical management or rapidly worsening neurological impairment. The traditional approach is a laminectomy with foraminotomy and partial facetectomy but a newer minimally invasive option, unilateral laminectomy for bilateral decompression (ULBD), seems to demonstrate the better postoperative outcomes due to its unilateral exposure. ULBD involves a midline incision, opening the thoracolumbar fascia...

Spinal Metastatic Disease: A Review of the Role of the Multidisciplinary Team
Historically, a simple approach centered on palliation was applicable to the majority of patients with metastatic spinal disease. With advances in diagnosis and treatment, a more complicated algorithm has devolved requiring a multidisciplinary approach with institutional commitment and support. We performed a database review including pertinent articles exploring the multidisciplinary management of spinal metastatic disease. The wide variation in clinical presentation and tumor response to treatment necessitates a multidisciplinary approach that integrates the diagnosis and treatment of the cancer, symptom management, and ...

LncRNA ‐H19 Modulates Wnt/β‐catenin Signaling by Targeting Dkk4 in Hindlimb Unloaded Rat
ConclusionOur findings demonstrated that low expression of H19, induced by mechanical unloading, leads to development of DOP through inhibition of Wnt signaling by promoting Dkk4 expression. (Source: Orthopaedic Surgery)

Thoracic Endoscopic ‐Assisted Mini‐Open Surgery for Thoracic and Thoracolumbar Spinal Cord Compression
Intervertebral disc herniation is a common cause of spinal cord compression, especially for the thoracic and thoracolumbar spinal cord, which has limited buffer space in the spinal canal. Spinal cord compression usually causes decreased sensation and paralysis of limbs below the level of compression, urinary and fecal incontinence, and/or urinary retention, which brings great suffering to the patients and usually requires surgical intervention. Thoracotomy or abdominothoracic surgery is usually performed for the thoracolumbar cord compression caused by hard intervertebral disc herniation. However, there is high risk of tra...

Surgical Technique of Total Knee Arthroplasty without Soft Tissue Balance
In this study, the authors present detailed steps for performing TKA. The attached video demonstrates the TKA procedure. The patient is a 77‐year‐old man who had suffered from knee pain for 12 years. Physical examination showed the Apley test to be positive and that the range of motion (ROM) decreased. An X‐ray filmed at the positive lateral of the knee joint and the double lower extremity revealed a progression of degenerative osteoarthritis with genu varum. The key point of no soft tissue release is to make a rectangular extensional space by osteotomy. In addition, the osteophytes, especially syndesmophytes, shou...

Surgical Technique for Subtrochanteric Fracture of Femur
A 56‐year‐old woman had fell over herself 4 h prior to her presentation to our hospital. The diagnosis was a subtrochanteric fracture of the right femur (AO‐32‐A3). The patient was placed in a supine position was set up on a fracture traction table after general anesthesia. The right leg was placed in abduction while the left leg was positioned so that there was flexion of the knee and hip joint. The C‐arm was placed on the left side. The surgical field was sterilized and draped. Deformity and external rotation are always present for this type of fracture, so the first step was to place a joystick in the proxim...

Arthroscopic Retrieval of over 100 Loose Bodies in Shoulder Synovial Chondromatosis: A Case Report and Review of Literature
We report a rare case of shoulder synovial chondromatosis in which more than 100 loose bodies were successfully retrieved by an arthroscopy in an individual who had an excellent outcome post‐surgery, reaffirming our faith in the procedure. A detailed literature review of arthroscopic procedures is also presented. (Source: Orthopaedic Surgery)

Treatment for Periprosthetic Cyst after Total Hip Arthroplasty: Analysis of Six Cases
The present study investigates the pathogenesis of periprosthetic cysts after total hip replacement, and explores appropriate treatment appoaches. Six patients with periprosthetic cysts after total hip arthroplasty were treated at the First Affiliated Hospital of Nanjing Medical University between 2009 and 2014. During surgery, it was found that all cysts communicated with the hip and the hip prosthesis could be seen after cyst excision. Four patients simply underwent cyst excision, and light red liquid was found in the cyst. Among them, radiological examination revealed that a part of the hip prosthesis projected from the...

N ‐acetylcysteine Attenuates Cobalt Nanoparticle‐Induced Cytotoxic Effects through Inhibition of Cell Death, Reactive Oxygen Species‐related Signaling and Cytokines Expression
ConclusionsThese findings indicated that NAC could reverse CoNP‐induced cytotoxicity by inhibiting ROS‐induced cell death and cytokine expression. To our knowledge, this is the first report that describes how CoNP‐induced cytotoxicity in TCMK‐1 cells could be attenuated by anti‐oxidative agents (NAC), which may function through inhibition of cell death and ROS. Relationship between Patellar Tracking and the “Screw‐home” Mechanism of Tibiofemoral Joint
ConclusionsAs the tibiofemoral joint rotated, the patellofemoral joint became more stable and aligned, which indicates that the screw‐home mechanism plays an important role in regulating patellofemoral joint alignment. (Source: Orthopaedic Surgery)

What Role Does Low Bone Mineral Density Play in the “Killer Turn” Effect after Transtibial Posterior Cruciate Ligament Reconstruction?
ConclusionsThe low BMD group demonstrated an inferior biomechanical outcome to the control group with the transtibial technique. With low BMD, the “killer turn” effect compromises the posterior tibial cortex by enlarging the tunnel inlet. (Source: Orthopaedic Surgery)

Management of Acute Hematogenous Infection Following Total Knee Arthroplasty: A Case Series of 11 Patients
ConclusionsPatients who developed an acute hematogenous infection with non‐staphylococcus species following operative debridement and continuous irrigation with prosthetic retention had satisfactory outcomes, but patients infected with staphylococcal had poor results. To improve the success rate of treatment, patients should be treated as soon as possible and individually according to the bacterial culture results. (Source: Orthopaedic Surgery)
