Current Treatment Strategies for Acute Type B Aortic Dissection
Acute type B aortic dissection (ATBAD) is a medical emergency that is a common occurrence in patients with atherosclerotic disease. The presentation is usually severe, with tearing pain that radiates to the back, and various levels of end-organ ischemia and malperfusion, even rupture, may occur. Everyone agrees that prompt and aggressive blood pressure control with β-blockers and nitroprusside is imperative, but when to surgically intervene is still not well characterized. However, the advent of minimally invasive stent graft placement has reshaped our thoughts regarding therapeutic intervention for ATBAD. This review...

Less Than Total Excision of Infected Prosthetic PTFE Graft Does Not Increase the Risk of Reinfection
Conclusions: Leaving a well-incorporated small 1-to 5-mm PTFE remnant at the arterial or venous anastomoses can be performed safely with a low risk of complications. (Source: Vascular and Endovascular Surgery)

Lessons Learned in the Surgical Treatment of Neurogenic Thoracic Outlet Syndrome Over 10 Years
Conclusion: Excellent results were seen in this surgical series reported for NTOS. Younger patients with shorter duration of symptoms with less narcotic use led to even better FRRS results in the second 5 years of surgical intervention. An established vascular practice for referrals for NTOS resulted in an increased number of appropriate patients for surgical intervention, requiring fewer lidocaine and/or Botox injections preoperatively. Should Age Limit the Use of Catheter-Directed Thrombolysis: Results of National Survey
Conclusion: Among vascular specialist, there seems to be no consensus on age limitations for TPA infusion. Serious complications do not seem to be age related and thus age alone should not be a contraindication for catheter-directed thrombolysis. (Source: Vascular and Endovascular Surgery)

Percutaneous Embolization of a Postnephrectomy Arteriovenous Fistula With Intervening Pseudoaneurysm Using the Amplatzer Vascular Plug 2
We report a case of a 68-year-old male who presented with a renal AVF and was treated by percutaneous transcatheter embolization using the Amplatzer Vascular Plug 2 (AVP 2; St Jude Medical, Plymouth, Minnesota) through an arterial access. To our knowledge, the use of AVP 2 device in the treatment of renal AVF as a single embolotherapy device through the transarterial route has not been previously reported in the literature. Our technique demonstrates the feasibility and safety of AVP 2 device in the treatment of renal AVF. (Source: Vascular and Endovascular Surgery)

Clopidogrel and 1-Year Freedom From Amputation After Endovascular Lower Extremity Revascularization in the Medicare Population
Conclusion: Utilization of clopidogrel after LER was associated with lower rates of amputation, yet only 38% of the Medicare population was identified as using clopidogrel. Patients with ulceration and tissue loss benefited the most with significantly greater freedom from amputation and overall survival. (Source: Vascular and Endovascular Surgery)

Ruptured Abdominal Aortic Aneurysm Treatment in the Stent Graft Era
Conclusion: We support the idea that better results can be obtained by showing regard to suitable patient, suitable clinical condition, and suitable anatomy together with the correct choice of operation type. (Source: Vascular and Endovascular Surgery)

Bovine Carotid Artery (Artegraft) as a Hemodialysis Access Conduit in Patients Who Are Poor Candidates for Native Arteriovenous Fistulae
Our experience with bovine carotid artery graft (BCAG) for hemodialysis access (Artegraft, North Brunswick, New Jersey) is presented. A review of all patients who underwent placement of BCAG for hemodialysis access at our institution was performed. Between January 2012 and June 2013, 17 BCAGs were placed in 17 patients. Indications included skin compromise, recurrent expanded polytetrafluoroethylene (ePTFE) and catheter infections, immunosuppression, groin placement, and surgeon's choice. Actuarial primary, primary-assisted, and secondary patency rates at 18 months were 73.3%, 67%, and 89%, respectively. One immunosuppress...

A Novel Classification System for Autogenous Arteriovenous Fistula Aneurysms in Renal Access Patients
Conclusion: Type 1 aneurysms are much commoner in patients who have not yet needled their fistula and have a relatively innocuous course although type 1a aneurysms should be monitored for high flow and physiological consequences thereof. Type 2 aneurysms are associated with needling of AVFs. They are at significant risk of rupture and need to be monitored carefully or treated prophylactically. Interventions on the Superficial Venous System for Chronic Venous Insufficiency by Surgeons in the Modern Era: An Analysis of ACS-NSQIP
Conclusions: The OSVV, obesity, and VU increase the odds of sSSI after procedures treating CVI. Patients in these categories should be monitored closely for signs of infection in the perioperative period. Patients undergoing EVA have higher odds of postoperative DVT, suggesting that routine screening after EVA for DVT may be justified. (Source: Vascular and Endovascular Surgery)

Rupture After Endovascular Abdominal Aortic Aneurysm Repair: A Multicenter Study
A total of 22 patients with ruptured abdominal aortic aneurysms (rAAAs) after previous endovascular aortic repair (EVAR; rAAAevar) were presented to 7 referral hospitals in Greece, between January 2006 and April 2012. Type Ia endoleak and endograft migration were identified in 72.7% and 50%, respectively. Compliance to follow-up protocol prior to rupture was 31.8%. In-hospital mortality was 36.4% (9.1% for those treated with secondary EVAR and 63.6% for those treated with open surgical repair, P = .02). An increase in the proportion of patients with rAAAevar among the total number of patients with rAAAs from 1.3% in 2007 t...

Statins and Nitric Oxide Donors Affect Thrombospondin 1-Induced Chemotaxis
Conclusion: Short-term statin pretreatment inhibited TSP-1-induced VSMC chemotaxis, suggesting a pleiotropic effect. High-dose NO reversed statin inhibition of TSP-1-induced chemotaxis, suggesting NO and statin combination therapies warrant further study. (Source: Vascular and Endovascular Surgery)
