Neurocognitive Improvement After Carotid Artery Stenting in Patients With Chronic Internal Carotid Artery Occlusion: A Prospective, Controlled, Single-Center Study
Symptomatic internal carotid artery (ICA) occlusion with hemodynamic impairment remains a dismal disease when untreated. In this prospective, single-center, controlled study, we investigated the feasibility, safety, and long-term outcome of stenting by endovascular recanalization for patients with chronic ICA occlusion. Forty patients with symptomatic chronically occluded ICA were assigned to receive endovascular recanalization (group A, n = 18) or conservative management (group B, n = 22). The primary end point was 100% complete recanalization of the primary occlusion at 60 minutes, and secondary end points were improveme...

Effect of Heparin-Derived Oligosaccharide on bFGFR1 and bFGFR2 in Vascular Smooth Muscle Cells
Our purpose is to investigate the inhibitory effect and mechanisms of heparin-derived oligosaccharide (HDO) on proliferation of vascular smooth muscle cells (VSMCs) induced by basic fibroblast growth factor (bFGF). Proliferation of VSMCs was measured by 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyl tetrazolium bromide; cell cycle distribution was analyzed by flow cytometry; bFGF receptor 1 and receptor 2 (bFGFR1 and bFGFR2) messenger RNA (mRNA) expression levels were determined by reverse transcription–polymerase chain reaction; and its protein expression levels were detected by Western blotting and immunocytochemical me...

Endovascular Aneurysm Repair for Multiple Aneurysms as a Sequel of Hypereosinophilic Syndrome
This case represents the first report of multiple arterial aneurysms including aortic, iliac, visceral, and coronary aneurysms associated with hypereosinophilic syndrome. It presents an interesting case of epinephrine abuse and the unfortunate sequelae. This case illustrates novel approaches in emergency repair of internal iliac artery aneurysm rupture and the management of visceral artery aneurysms and exemplifies how multiple endovascular technologies can be utilized even in the high-risk polymorbid patient. Symptomatic Popliteal Vein Aneurysm
We report a case of PVA in a previously healthy 58-year-old female with a history of pain in her right popliteal fossa for the past 3 years. Patient had no history of trauma or claudication of the right leg. Following a preoperative venogram to confirm the diagnosis, the PVA was dissected circumferentially through a posterior incision. The aneurysm was resected and repaired with lateral venorrhaphy. Patient had an uneventful recovery. Due to the possibility of severe consequences, if left untreated, early surgical repair is highly recommended whether the patient has symptoms or not. (Source: Vascular and Endovascular Surgery)

Endovascular Repair of an Ductus Arteriosus Aneurysm Causing Ortner Syndrome
A ductus arteriosus aneurysm (DAA) is a rare but potentially fatal condition in adults. In the past, open surgery was the only available option for treatment. Nowadays, endovascular repair has emerged as a safe and less invasive treatment option for aortic aneurysms. However, there is little experience with DAA and its anatomic location can complicate endovascular repair. Here, we describe the case of a 69-year-old patient who presented with Ortner syndrome caused by DAA, which was successfully treated using a solely endovascular approach and a standard stent graft. Metastatic Endometrial Sarcoma With Inferior Vena Caval and Cardiac Involvement: A Combined Surgical Approach
Low-grade endometrial stromal sarcoma with intracaval and intracardiac extension represents a complex and often lethal condition. A case of a 40-year-old woman, who underwent hysterectomy for endometrial stromal sarcoma 4 years previously and developed recurrence with intracaval and intracardiac metastases, is presented. Operative management of resection of the intracaval and intracardiac tumor is described. The literature is reviewed in order to highlight the data on diagnosis and management of recurrent endometrial stromal sarcoma with vascular involvement, including the multimodality approach required in treating this d...

Successful Coil Embolization of Circumflex Iliac Artery Pseudoaneurysms Following Paracentesis
We report 2 patients who presented with abdominal wall pseudoaneurysms following abdominal paracentesis for the evaluation and treatment of their hepatic dysfunction. We subsequently review the treatment modalities and interventions performed in each case. (Source: Vascular and Endovascular Surgery)

Surgical Salvage of Acute Renal Artery Occlusion in the Setting of a Solitary Kidney
We report a unique case of a patient that had a 24-hour onset of anuria and acute renal failure upon arrival to the hospital. Nuclear imaging showed trace uptake of the right kidney, without evidence of excretion. Conventional digital subtraction angiography was performed; however, evidence of nephrogram or distal filling of the renal artery was not demonstrated. Secondary to conflicting studies, a computed tomography of the abdomen and pelvis with intravenous contrast revealed only minimal cortical perfusion despite complete occlusion of the previously grafted right renal artery. Patient was taken for urgent hepatorenal b...

Congenital Carotid-Jugular Fistula in a Child: A Case Report
Congenital carotid–jugular (CJ) fistula of the neck is a very rare clinical entity that has various causes. The CJ fistulas are particularly prone to complications unlike other peripheral arteriovenous fistulas. The aim of this report is to present a case of a CJ fistula between the external carotid and the external jugular vein, which was successfully closed with detachable balloon by an endovascular approach. A 14-year-old child was admitted to our clinic with a pulsatile neck swelling. There was no previous history of trauma. A high-flow fistula between the external carotid and the external jugular vein was determ...

Endovascular Stent Graft Repair of Suture-Line Pseudoaneurysm Following Ascending Aorta Replacement
Ascending aorta pseudoaneurysm (AAPA) is an uncommon complication following replacement of the ascending aorta with a prosthetic graft, carry a high risk of rupture, and warrant urgent intervention. The open surgical procedure "gold standard" of care is not always favorable, as the reoperations are technically more difficult or patient’s general condition doesn’t allow proceeding. Case discussed is an 80-year-old male patient who presented with worsening cough and hemoptysis. He underwent ascending aorta replacement 10 years ago. Computed tomography (CT) scan revealed a contrast-filled mediastinal mass communic...

Endovascular Management of a Mobile Thoracic Aortic Thrombus Following Recurrent Distal Thromboembolism: A Case Report and Literature Review
Sources of lower extremity and visceral arterial thromboembolism most commonly include the heart and proximal aneurysmal disease. Infrequently, further workup of "cryptogenic" emboli will reveal a mural atheroma or thrombus of the descending thoracic aorta. Without prospective data, anticoagulation and open surgical thrombectomy with or without aortic replacement have been the standard approach. Presented is a case of a floating mural thrombus in the setting of superior mesenteric and femoral arterial thromboembolism that was effectively treated with endovascular stent graft exclusion. Conclusion: The frequent postoperative circulating aortic FC can be explained by the persistence of the primary entry tear, the presence of iatrogenic tears, and/or a retrograde filling in the SAVs. (Source: Vascular and Endovascular Surgery)
