Clinical Utility of Biomarkers in Chronic Kidney Disease and Chronic Heart Failure
SUMMARY Biomarkers have an increasingly important clinical role in managing patients with heart failure as well as those with kidney disease, both common conditions with generally poor prognostic outcomes and huge impacts on healthcare economics. For patients with chronic heart failure, biomarkers have become centre place in streamlining diagnostic pathways as well as identifying those with worse prognosis. There is much interest in the role for biomarkers in identifying patients at risk of acute kidney injury, although a number of these currently remain as research tools or are in the early stages of evaluation in clinica...

Quality of life of adult clients on renal replacement therapies in nepal
ConclusionThe QOL score in renal transplant recipients was significantly better than that of clients on HD in three of the four WHOQOL‐BREF domains. The limited resources and facilities for renal transplantation and the post‐transplant follow‐up service in Nepal might have contributed to a poorer outcome on the environmental domain in this group. Emotional effort and perceived support in renal nursing: a comparative interview study
ConclusionsBecause their high levels of emotional engagement with patients are mostly positive, renal nurses are less prepared than other nurses to manage difficult emotional situations. As co‐worker support is highly valued, organisations should train renal nurses specifically to support one another. (Source: Journal of Renal Care)

Outcomes of buttonhole and rope‐ladder cannulation techniques in a tropical renal service
Conclusions and applications to practiceThis study confirms that in this setting there are few negative outcomes of either technique of fistula cannulation. Specifically, buttonhole cannulation appears to be a safe alternative means of fistula access to the gold‐standard cannulation technique. The Service is examining strategies to improve attendance. Are haemoglobin levels adequate in patients with chronic kidney disease (ckd) stage 2–4?
Conclusion and Application to PracticeMore emphasis should be placed on detection and treatment of anaemia in earlier stages of kidney disease. Poor response to erythropoiesis stimulating agent therapy should be investigated in this group and any identified causes of poor response treated. (Source: Journal of Renal Care)

Vascular access graft and native arterial branch fistulous formation
SUMMARY This is the first reported case of an unusual complication of upper extremity swelling of the arm in a patient with an arterio‐venous (AV) graft for haemodialysis. A graftogram demonstrated an aberrant fistula formation between the native arterial branch and the AV graft (aberrant arterio‐graft fistula). Inadvertent back wall laceration of the AV graft during haemodialysis cannulation was postulated to be the aetiology. Aberrant arterio‐graft fistula formation may be hard to diagnose because of the presence of thrill or bruit at the level of the anastomoses and rarity of the complication. Progression of extre...

Exploring the views of patients not on the transplant waiting list: a qualitative study
ConclusionsPatients trust their carers and support an efficacy argument when considering scarce resources. Communication should be improved to ensure clarity and understanding of clinical decisions. (Source: Journal of Renal Care)

Patient independence in chronic kidney disease and anaemia: implications of the 2012 kdigo guideline
ConclusionsAlthough the KDIGO Guideline has the potential to reduce patient independence, by acknowledging the needs of the individual early on and employing a multi‐disciplinary approach, a balance can be found between utilising the most recent guidelines and meeting the needs of the individual to ensure their on‐going independence. Role of components of frailty on quality of life in dialysis patients: a cross‐sectional study
ConclusionsDependence, malnutrition, disability, poor social and economic conditions have a significant effect on life quality. The role of comorbidities appears to be less important. Screening of patients, nutritional and functional rehabilitation and prevention of social isolation appear to be indispensable in guaranteeing a satisfactory life quality.
