Dietary adherence in children with chronic kidney disease: a review of the evidence
SUMMARY BackgroundIn children with chronic kidney disease (CKD), a nutrition support plan is important to ensure optimal macro‐ and micro‐nutrient intake in order to avoid malnutrition, disease‐related complications and growth rate reduction. Children with CKD and their families encounter many difficulties in adjusting to the renal diet. Even though adherence to the recommended dietary plan is important in CKD, it is rarely measured partly due to the lack of robust, unbiased assessment methods. MethodsIn this review of 22 papers, the techniques used to assess adherence in children with CKD are reviewed, alongside the...

Management of hyperphosphataemia: practices and perspectives amongst the renal care community
ConclusionThis renal care community followed professionally accepted guidelines for patient nutrition and management of hyperphosphataemia. There was disparity in the perceptions and recommendations between nurses and dietitians, highlighting the need to standardise management practices amongst renal care professionals. COMPARISON OF Kt/V AND UREA REDUCTION RATIO IN MEASURING DIALYSIS ADEQUACY IN PAEDIATRIC HAEMODIALYSIS IN ENGLAND
ConclusionOne standard measure should be used to assess dialysis adequacy in paediatric centres in England. KDOQI guidelines were not consistently followed in obtaining a post‐urea blood sample and this procedure should be standardised. (Source: Journal of Renal Care)

Medicine non‐adherence in kidney transplantation
ConclusionTransplantation is the preferred treatment option for people with end‐stage kidney disease. Medicine non‐adherence in kidney transplantation increases the risk of rejection, kidney loss and costly treatments. Interventions are needed to help the transplant recipient take all their medicines as prescribed to improve general well‐being, medicine safety and reduce healthcare costs. Saliva composition and upper gastrointestinal symptoms in chronic kidney disease
ConclusionAltered saliva composition in patients with Stages 4 and 5 CKD may be associated with uraemic upper GI symptoms. In particular, lower saliva concentrations of bicarbonate are associated with dry mouth and retching. Higher saliva calcium levels are also related to a dry mouth whilst higher saliva sodium levels and a greater sodium/potassium ratio are associated with nausea. Further studies investigating strategies to improve uraemic symptoms via changes in saliva are required. (Source: Journal of Renal Care)

A multi‐centre, randomised trial to assess whether increased dietary fibre intake (using a fibre supplement or high‐fibre foods) produces healthy bowel performance and reduces laxative requirement in free living patients on peritoneal dialysis
ConclusionThis study has confirmed the prevalence of laxative use amongst patients on PD and shown that fibre use can confer improvements in bowel function without affecting biochemistry. (Source: Journal of Renal Care)

Nutrition care for renal transplant recipients: an evaluation of service delivery and outcomes
ConclusionNutrition care provided did not meet guideline recommendations, highlighting difficulty in implementing evidence to practice. Significant weight gain was evident particularly in patients classified as ‘healthy weight’ at the time of transplant. Long‐term, prospective studies are needed to determine the effectiveness of implementing nutrition care to attenuate weight gain and improve clinical outcomes. (Source: Journal of Renal Care)

Living kidney donation: considerations and decision‐making
ConclusionThe study provides insight and knowledge for the health care professionals to meet and involve donors' narratives in reflections about and modifications to clinical nursing practice. It is essential that health care professionals have an understanding and appreciation of the experiences and concerns among LKDs, and this can help in planning and providing individual nursing care and support to donors.
