Fentanyl versus magnesium sulphate as adjuvant to peribulbar anesthesia in cataract surgery
Conclusions Addition of fentanyl (2μg/ml) or magnesium sulphate (50mg) to peribulbar block in patients undergoing cataract surgery equally prolongs the duration of postoperative analgesia. In addition to this effect, fentanyl fastens the onset lid and globe akinesia and provides better akinesia score. (Source: Egyptian Journal of Anaesthesia)

Evaluation of dexmedetomidine as a sole agent in sedation of cancer patients undergoing radiological interventional procedures
Conclusion Dexmedetomidine can be used alone for sedation of interventional procedures when minimal to mild pain is in prospect thus provides an alternative for anesthesiologists for high risk patients but cannot be used alone when intense pain is anticipated. Intubation stress responses: Pre-anesthetic dexmedetomidine versus fentanyl in pre-eclamptic patients undergoing caesarean delivery: A prospective double blind randomized study
Conclusions Dexmedetomidine given over 10min before induction of general anesthesia significantly reduced the measured hemodynamic variables compared to baseline values. Dexmedetomidine successfully attenuated the intubation stress response and provided a significant hemodynamic stability more than fentanyl which given 3min before the induction of anesthesia in sever pre-eclamptic patients. Neither drug was associated with any harmful neonatal outcome. Pan African Clinical Trials Registry (PACTR201508001198128). (Source: Egyptian Journal of Anaesthesia)

Effect of low dose ketamine versus dexmedetomidine on gag reflex during propofol based sedation during upper gastrointestinal endoscopy. A randomized controlled study
Conclusion Dexmedetomidine with propofol in patients undergoing UGIE was safe and effectively, can reduce the incidence of gag reflex better than ketamine when added to propofol, with less propofol consumption and better in recovery time. (Source: Egyptian Journal of Anaesthesia)

Superficial parotidectomy under local anesthesia, case report
Publication date: Available online 11 January 2017 Source:Egyptian Journal of Anaesthesia Author(s): Mohamed Ibrahim For superficial parotidectomy general anesthesia is ideal, but certain patients may be unfit. The present case had uncontrolled hypertension together with difficult airway. To avoid any suspected complications, we decided to do the operation under local anesthesia. We planned to block the maxillary nerve and superficial cervical plexus with greater auricular nerve together with incision site infiltration. A mixture of bupivacaine 0.25% and lignocaine 1% with adrenaline 1: 200,000 was prepared. The block w...

Airway management of a patient with severe post burn mento-sternal contracture: A novel approach
Publication date: Available online 7 January 2017 Source:Egyptian Journal of Anaesthesia Author(s): Poonam Gupta, Charu Bamba Airway management in patients with severe post burn mentosternal contracture is always a challenge for the anaesthesiologist. Difficulty in intubation occurs due to contracture and fixed flexion deformity of the neck resulting in nonalignment of oropharyngeal and laryngeal axis. We are reporting a case with severe postburn mentosternal contracture in whom endotracheal intubation was done successfully by using a Pro seal Laryngeal Mask Airway without its introducer along with a tube exchanger aft...

The role of ondansetron in prevention of post-spinal shivering (PSS) in obstetric patients: A double-blind randomized controlled trial
Conclusion Ondansetron (8mg) was effective in reducing post-spinal shivering in parturients underwent elective cesarean delivery and decreasing the requirement to meperidine together with lower incidence of post-spinal hypotension and nausea when compared to placebo (saline). Immediate post-discectomy percutaneous facet nerve continuous and nerve root pulsed radiofrequency and intraluminal injection of steroid with hyaluronidase improved outcome of surgery for lumbar disk herniation
Conclusion Open discectomy provided significant improvement of LBP and disability secondary to LDH. Discectomy with adjuvant therapy including PRF, CTRFN and triamcinolone and hyaluronidase intraluminar injection significantly improved outcome compared to discectomy alone. Giant paraganglioma in a child: Medical and anaesthetic concerns
We present a 12year old girl with giant paraganglioma with severe hypertension and end organ damage. Diagnosis was confirmed with 24h urinary Vanillymandelic Acid (VMA) and CT scan. Preoperative blood pressure was controlled with intravenous nitroprusside, and oral prazosin, amlodepine, labetalol and metoprolol. General anaesthesia with epidural analgesia was given. Intra operative blood pressure rise was managed with infusion of nitriglycerine (NTG), esmolol, nitroprusside and propofol. (Source: Egyptian Journal of Anaesthesia)

Efficacy of postsurgical ultrasound guided serratus intercostal plane block and wound infiltration on postoperative analgesia after female breast surgeries. A comparative study
Conclusion There is considerable evidence that serratus intercostal plane block in addition to general anesthesia provide better postoperative pain control with little adverse effects compared with wound infiltration, indicating that a perioperative serratus intercostal plane block is a feasible and effective method for an improved postoperative pain treatment after breast surgery.
