Difficult airway management patterns among anesthesiologists practicing in Cairo University Hospitals
Conclusion The practice of anesthesiologists in Cairo university hospitals is close to the recommendations of the ASA guidelines for management of difficult airway. There is increased skills in fiberoptic bronchoscopes and SGA with increased frequency of difficult airway managements training courses; however, they need to improve their skills in awake fiberoptic intubations technique and they need to be trained on invasive airway management access to close the discrepancy between their theoretical choices in different situations and their actual skills. Vagal reflex stimulation complicating retrieval of an unusual foreign body from the laryngotracheal lumen: Case report
We report a case of a 4year old boy with an unusual foreign body (FB) impacted in the laryngotracheal causing difficult intubation and precluding tracheostomy with attendant vasovagal reflex stimulation and cardiac arrest. Clinical presentation and radiological evaluation of the patient were highlighted with a review of pertinent literature. We conclude that dis-impacting a foreign body in the trachea could potentiate bradycardia and cardiac arrest; co-existing hypercarbia and/or sepsis increase the risk and worsen the prognosis. A complicated case of antepartum eclamptic fit with HELLP syndrome, acute renal failure and multiple intracranial hemorrhages: A mortality report
Publication date: Available online 28 November 2015 Source:Egyptian Journal of Anaesthesia Author(s): Ahmed Samy El-agwany HELLP is an acronym for hemolysis, elevated liver enzymes and low platelets count, affecting 0.2–12% of all pregnancies or 4–12% of those with preeclampsia. The maternal mortality reported from the literature is up 4% due to disseminated intravascular coagulation, placental abruption, acute renal failure, eclampsia, and cerebral hemorrhage. A 20year old, G2P1, at 36weeks of gestation, was referred to our hospital because of postictal coma state with bilateral mydriasis and epistaxis due to re...

Hemodynamic changes and stress response during BIS-guided TCI anesthesia with propofol-fentanyl in laparoscopic versus open cholecystectomy
Conclusion BIS-guided TCI anesthesia with propofol-fentanyl offers a good and safe anesthesia technique for patients undergoing either laparoscopic or open cholecystectomy. The hemodynamic stability guided by esophageal Doppler monitor makes it a very appealing choice. (Source: Egyptian Journal of Anaesthesia)

Postoperative cognitive function and controlled hypotensive anesthesia in patients undergoing septoplasty
Conclusion Increasing isoflurane MAC and using controlled hypotension to get bloodless field can affect the cognitive function appear in MMSE score but light volatile anesthetic combined with regional anesthesia with local vasoconstrictors is better regime to get better postoperative cognitive function. Dexmedetomidine versus fentanyl in anesthesia of cochlear implantation in pediatric patients
Conclusion Dexmedetomidine infusion in cochlear implantation in pediatric patients was better in inducing deliberate hypotension and providing better quality scale of surgical field compared to fentanyl infusion. It allowed rapid recovery from anesthesia and reduced need for pain medication in the PACU. (Source: Egyptian Journal of Anaesthesia)

Ultrasound guided pectoral nerve blockade versus thoracic spinal blockade for conservative breast surgery in cancer breast: A randomized controlled trial
Conclusion In conclusion, both Pecs and TSB provide effective intraoperative anesthesia and prolonged postoperative pain relief after breast surgery, but the Pecs block is technically simple and easy to learn with few contraindications, provides hemodynamic stability, and has a low complication rate and it is therefore a safe and effective technique in performing intraoperative anesthesia and controlling postoperative pain after unilateral conservative breast surgery. Different drugs for prevention of post subarachnoid block shivering. Randomized, controlled, double blind study
Conclusion Preoperative administration of granisetron, dexmedetomidine, and tramadol was effective in decreasing the incidence and intensity of post subarachnoid shivering without increasing the incidence of the side effects. (Source: Egyptian Journal of Anaesthesia)

Comparative study of clonidine versus esmolol on hemodynamic responses during laparoscopic cholecystectomy
Conclusion This study concluded that clonidine and esmolol provide hemodynamic stability in laparoscopic cholecystectomy but clonidine provides more stability with postoperative sedation. (Source: Egyptian Journal of Anaesthesia)

Comparative evaluation between ascorbic acid and N-acetyl cysteine for preventing tourniquet induced ischaemic reperfusion injury during lower limb surgery, a randomized controlled trial
Conclusion Both N-acetyl cysteine and ascorbic acid reduce post-deflation increase in blood levels of markers of oxidative stress and markers of systemic inflammation and thus both are beneficial in preventing post-tourniquet deflation ischemic reperfusion injury in lower limb surgery. Comparative study between effect of carbetocin and oxytocin on isoflurane-induced uterine hypotonia in twin pregnancy patients undergoing cesarean section
Conclusion Single dose of carbetocin appears to be more effective than oxytocin for several hours on uterine contraction and though preventing postpartum hemorrhage in multiple pregnancy patients undergoing elective caesarian section. (Source: Egyptian Journal of Anaesthesia)

Synergistic effects of Paracetamol and Dexamethasone with Lidocaine in Intravenous regional anesthesia (IVRA) of upper limbs: A randomized clinical trial
Conclusion A combination of Paracetamol and Dexamethasone significantly enhances the analgesic effect of Lidocaine in IVRA by accelerating the establishment of both the sensory and motor nerve blocks and prolonging the period of analgesia as well as improving the quality of analgesia and reducing the need for analgesic medications during and after the operation. Iron alone or iron and erythropoietin added to acute normovolemic hemodilution in myomectomy patients: A randomized controlled trial
Conclusion Preoperative iron alone or iron and erythropoietin added to acute normovolemic hemodilution help to reduce the need for allogenic blood transfusion in myomectomy patients. Further studies are needed to evaluate their use in older patients with higher risk for thrombosis and hypertension. (Source: Egyptian Journal of Anaesthesia)

Edward’s syndrome: A rare cause of difficult intubation-utility of left molar approach
Publication date: Available online 31 October 2015 Source:Egyptian Journal of Anaesthesia Author(s): Teena Bansal, Savita Saini Edward’s syndrome (trisomy 18) is an autosomal abnormality with dysmorphic face, visceral deformities and delayed mental and motor development including congenital heart disease. Challenges may arise during mask ventilation, laryngoscopy and/or intubation of the trachea due to dysmorphic face. Difficult airway cart should be kept ready. Left molar approach using a standard Macintosh blade improves the laryngoscopic view in patients with difficult midline laryngoscopy. We hereby present a...
