Spontaneously resolved gigantic retroperitoneal haematoma following oophorectomy: A case report in a cardiac patient on anticoagulant therapy
Conclusion Delay in diagnosis is potentially fatal and high clinical suspicion remains crucial. Finally, it is a matter of controversy whether retroperitoneal haematomas should be surgically evacuated or conservatively treated and the final decision should be made after taking into consideration patient’s general condition and the possibility of permanent femoral or sciatic neuropathy due to compression syndrome. Continuous spinal anesthesia versus single small dose bupivacaine–fentanyl spinal anesthesia in high risk elderly patients: A randomized controlled trial
Conclusion CSA provided fewer episodes of hypotension and no severe hypotension versus SD 7.5mg bupivacaine. CSA offers the added advantage of the ability to titrate dose of local anesthetic as needed while maintaining hemodynamic stability. (Source: Egyptian Journal of Anaesthesia)

Reactive thrombocytosis and pseudohyperkalemia-entities we come across occasionally
Publication date: Available online 11 April 2015 Source:Egyptian Journal of Anaesthesia Author(s): R. Subha , Rachel Cherian Koshy Thrombocytosis and hyperkalemia are entities which can have sinister complications in the perioperative period and can be worrisome to the alert anesthesiologist. Hence it is good to be aware of variants which are quite benign and need not lead to delaying of anaesthesia administration. One such case is reported to highlight this fact. Postendovascular thoracic aortic repair subclavian steal syndrome revealed by severe headache
We present a case of severe headache occurring after a TEVAR with intentional coverage of the origin of the left SCA. This headache was the only symptom from which the patient complained, and which disappeared immediately after carotid-SCA bypass. Other devastating complications can happen, which gave as a concern about the management of SCA when decision to practice a TEVAR is taken. (Source: Egyptian Journal of Anaesthesia)

Effect of low tidal volume during general anesthesia for urological procedures on lung functions
Conclusion In comparison with conventional mechanical ventilation using high tidal volume with zero PEEP and no RM: a lung protective strategy using low tidal volume with 5–10cm H2O PEEP and RM did improved lung functions in the first post-operative 24hours. The overall postoperative follow up did not show significant difference between the two groups. (Source: Egyptian Journal of Anaesthesia)

Ultrasound guided TAP block versus ultrasound guided caudal block for pain relief in children undergoing lower abdominal surgeries
Conclusion TAP block and caudal block under ultrasound guidance proved to be safe with no recorded complications either intra or postoperatively. Patient and parent satisfaction was markedly observed in case of TAP block. (Source: Egyptian Journal of Anaesthesia)

Nebulized lidocaine and fentanyl before sevoflurane induction of anesthesia in congenital diaphragmatic hernia repair: Prospective double blind randomized study
Conclusions Premedication of infants undergoing CDH repair with nebulized solution containing 4mgkg− 1 lidocaine 1% plus 2μgkg− 1 fentanyl improves the intubating conditions under inhalational sevoflurane induction without muscle relaxation. The studied combination can suppress patients’ hemodynamic changes to intubation. (Source: Egyptian Journal of Anaesthesia)

Superior laryngeal nerve block as an adjuvant to General Anesthesia during endoscopic laryngeal surgeries A randomized controlled trial
Conclusion During endoscopic laryngeal surgeries, using bilateral block of the internal branch of superior laryngeal nerve as an adjuvant to general anesthesia was associated with better intubation conditions, better intraoperative hemodynamic response to intubation and surgical procedure and better recovery profile in the form of improved postoperative cough and sore throat. Efficacy of intrathecal dexmedetomidine in prevention of shivering in patients undergoing transurethral prostatectomy: A randomized controlled trial
Conclusion Our results suggest that the use of 10μg intrathecal dexmedetomidine in patients undergoing TURP has a noticeable control of shivering even in occurrence of tolerable side effects. (Source: Egyptian Journal of Anaesthesia)

Complications and outcomes following central neuraxial anesthesia in a sub-Saharan Tertiary Hospital: The legal implication
Conclusion We have demonstrated that complications, which may result in litigation and claim following central neuraxial anesthesia is not a rare occurrence in our institution. However, there was no record of litigation or claim in our review. Anesthetist in Nigeria need to be aware of the legal implication of such complications. When performing blocks, well recognized complications should be discussed before obtaining consent. If any untoward effect occurs, a detailed note of the findings and treatment should be documented for future reference. (Source: Egyptian Journal of Anaesthesia)

Preoperative gabapentin alone or in combination with dexamethasone on postoperative pain relief after abdominal hysterectomies. A randomized controlled trial
Conclusion Gabapentin alone reduced the intraoperative and postoperative opioid requirement as well as postoperative pain and PONV which was significant in comparison with the placebo effect in the control. Obviously these effects were more prominent and highly significant when dexamethasone was added to gabapentin. (Source: Egyptian Journal of Anaesthesia)
