Analgesic efficacy and safety of peri-operative pregabalin following radical cystectomy: A dose grading study
Conclusion Peri-operative pregabalin at doses of 300mg and 600mg reduced postoperative opioid consumption and prolonged time to first request of analgesia in patients who underwent radical cystectomy, and a single preoperative dose of 600mg is superior in analgesia to others, without serious side effects. Remifentanil vs dexmedetomidine for severely preeclamptic parturients scheduled for cesarean section under general anesthesia: A randomized controlled trial
Conclusion Both remifentanil and dexmedetomidine were effective on blunting the pressor response to noxious stimuli in severely preeclamptic parturients. While remifentanil was marginally more effective in suppressing the pressor response, dexmedetomidine was safer for the neonates. (Source: Egyptian Journal of Anaesthesia)

Bupivacaine constant continuous surgical wound infusion versus continuous epidural infusion for post cesarean section pain, randomized placebo-controlled study
Conclusion The current study demonstrated that bupivacaine administered by continuous epidural infusion provided a significantly lower pain scores with mobilization, and hence better analgesia for post cesarean section pain in the first postoperative day compared to continuous bupivacaine wound infusion through fenestrated catheter using the constant flow PainFusor system. The effect of adding dexmedetomidine to local anesthetic mixture for peribulbar block in vitreoretinal surgeries
Conclusions Adding dexmedetomidine to a mixture of lidocaine 2% and bupivacaine in peribulbar block shortened sensory and motor block onset, extended the analgesia period and the motor block duration. It also significantly decreased the intraocular pressure and enabled better operating conditions. In this study, the addition of 25μ dexmedetomidine to the local anesthetic mixture showed the best outcome. (Source: Egyptian Journal of Anaesthesia)

Ivabradine versus propranolol given orally in microlaryngoscopic surgeries in attenuating stress response: A comparative prospective double blind randomized study
Conclusion Premedication with 5mg of oral ivabradine or 10mg of oral propranolol before microlaryngoscopic surgeries was effective in achieving a good degree of hemodynamic stability but ivabradine was more effective. Both drugs didn’t show an obvious effect on blood glucose level perioperatively. No complications were recorded. (Source: Egyptian Journal of Anaesthesia)

Anaesthetic management in a case of large plunging ranula with difficult airway: A case report
Publication date: Available online 4 October 2016 Source:Egyptian Journal of Anaesthesia Author(s): Surendra Kumar Sethi, Neena Jain, Arvind Khare, Veena Patodi Plunging ranula is a mucous retention cyst found on the floor of mouth which arises from the submandibular and sublingual salivary glands extending to lateral aspect of neck, which may often cause potential airway obstruction leading to difficulty in airway management. A forty year old female patient was admitted to our hospital with large, painless swelling in the floor of mouth extending to the lateral part of body of mandible and neck. This intraoral swell...

The anesthetic management for a special needs patient with trisomy 18 accompanying untreated tetralogy of Fallot
Publication date: Available online 30 September 2016 Source:Egyptian Journal of Anaesthesia Author(s): Masanori Tsukamoto, Takashi Hitosugi, Kanako Esaki, Takeshi Yokoyama Special needs patients with mental retardation are recognized to have poorer oral health condition. Oral health related quality of life reflects daily activity and well-being. Dental treatment under general anesthesia is often an option for such patients. Trisomy 18 is characterized by congenital heart disease, craniofacial abnormality and mental retardation. Congenital heart disease can be greater risk during anesthesia. In the case of trisomy 18 ...

Comparative study between continuous epidural anaesthesia and continuous Wiley Spinal ® anaesthesia in elderly patients undergoing TURP
Conclusion In elderly patients undergoing TURP, continuous Wiley Spinal® anaesthesia showed nearly comparable haemodynamics as continuous epidural anaesthesia with minimal adverse effects. This technique also provided good anaesthetic profile as well as fast sensory and motor block onset and recovery. (Source: Egyptian Journal of Anaesthesia)

Anesthesia for an elderly female with a rare congenital heart disease – A case report
We present a case of female patient 78 year old with long standing Eisenmenger syndrome. Her ECHO cardiography showed ejection fraction 57%, dextrocardia, large VSD 1.4cm with right to left shunt with systolic pressure gradient 70mmHg, moderate to severe TR, moderate MR, dilated and hypertrophied RV with preserved systolic function, hugely dilated right atrium, and severe pulmonary hypertension with pulmonary artery systolic pressure 125mmHg. The patient had future neck femur, arthroplasty under hemi spinal anesthesia. After one month she presented to us with dislocated joint, and hemiarthroplasty under hemispinal anesthes...

Anesthesia management of Struge-Weber syndrome
We report a case of 12year old boy having port wine stain on his left side of face with poor left sided vision. Proper examination and anesthetic management of these patients are very important for an anticipated difficult intubation if angiomas involve the airway. Smooth induction and extubation are necessary to avoid increase in intraocular and intracranial pressure. (Source: Egyptian Journal of Anaesthesia)

Safety and efficacy of dexmedetomidine sedation for elective fiberoptic bronchoscopy: A comparative study with propofol
Conclusion Dexmedetomidine and propofol-fentanyl are effective sedatives for patients undergoing flexible fiberoptic bronchoscopy. The sympatholytic and respiratory stability effects of dexmedetomidine make it an attractive and safe alternative for sedation during FOB. (Source: Egyptian Journal of Anaesthesia)
