Video-assisted thoracic surgery for pulmonary sequestration
Bronchopulmonary sequestration is a rare developmental abnormality. Most cases are asymptomatic and found incidentally. The definitive treatment for bronchopulmonary sequestration is surgical excision. An 18-year-old man was admitted to our clinic with longstanding cough, fever, and dense sputum. Chest computed tomography identified cystic bronchiectasis in common areas of the left lower lobe, and parenchymal destruction with air-fluid levels. Multiple ruptures of left sinus of Valsalva aneurysm into main pulmonary artery
We report an extremely rare case of left sinus of Valsalva aneurysm compressing and rupturing into the main pulmonary artery with 2 communications. The 34-year-old man also had severe aortic regurgitation and severe tricuspid regurgitation. He underwent successful polytetrafluoroethylene patch repair and aortic valve replacement. The rarity of this case highlights the need for a precise preoperative diagnosis, the roles of transthoracic echocardiography, transesophageal echocardiography, and 3-dimensional echocardiography, and the importance of prompt surgical management. Acute pericarditis with cardiac tamponade induced by pacemaker implantation
An 87-year-old woman was diagnosed with third-degree atrioventricular block and underwent pacemaker implantation. On postoperative day 12, she experienced cardiac tamponade that was suspected on computed tomography to be caused by lead perforation; therefore, we performed open-heart surgery. Perigraft seroma with minute observations after Gore-Tex graft implantation
Perigraft seroma is a very rare postoperative complication following abdominal aortic aneurysm repair. A 74-year-old man with history of esophageal cancer surgery, underwent Gore-Tex graft replacement for an abdominal aortic aneurysm. A 50-mm perigraft seroma was noted one year later, increasing to over 70 mm in the next 6 months. We resected the Gore-Tex graft and replaced it with a Dacron graft via a repeat laparotomy. "Killer coronary artery" and aortic valve stenosis: A tricky case
We report such an anomaly in a 75-year-old lady presenting with chest pain and syncope. Preoperative screening revealed that her aortic valve was moderately stenotic. The patient underwent a successful unroofing procedure combined with aortic valve replacement. The outcome was uncomplicated and the patient remained asymptomatic at one year postoperatively. Polytrauma to right diaphragm, descending thoracic aorta, and innominate artery
We report a case of combined traumatic diaphragmatic, aortic and innominate artery injuries, which was managed successfully. To have a fruitful outcome, management of this injury complex should preferably be staged depending on the hemodynamic stability of the patient. Bilateral axillary artery aneurysms after Bentall procedure in Marfan syndrome
A man with Marfan syndrome underwent a Bentall procedure for annuloaortic ectasia and severe aortic regurgitation at 43 years of age. Twenty-eight years after the Bentall procedure, he developed bilateral axillary artery aneurysms (length x diameter: right: 80 x 39 mm; left: 103 x 45 mm). Aneurysmectomy and reconstruction of the axillary artery were performed using an artificial vascular graft. Histological examination revealed cystic medial necrosis. Symptomatic stenosis of left subclavian artery from Kommerell's diverticulum
We report an extremely rare case of symptomatic stenotic origin of an aberrant left subclavian artery arising from a Kommerell’s diverticulum in a 39-year-old man with right-sided aortic arch who had left upper limb pain and numbness on exercise. Congenital coronary artery anomaly simulating a ventricular septal defect
Anomalous origin of the circumflex artery of the right sinus of Valsalva is a rare finding which may be present with other cardiac malformations. A 19-year-old man presented with syncope. A transthoracic echocardiogram revealed discrete subaortic stenosis with a small defect just below the aortic valve, suggesting a ventricular septal defect. Transesophageal echocardiography showed anomalous origin of the circumflex artery from the right sinus of Valsalva. This was confirmed by coronary angiography. Bioprosthetic aortic valve replacement on a mechanical valve ring
Mechanical valve failure is a known but rare occurrence in cardiac surgery. A 71-year-old lady underwent surgery for failure of her mechanical aortic valve. It was replaced with another mechanical valve. However, during operative manipulation, both leaflets fractured, resulting in a prolonged operative time. Under these circumstances, a novel approach was adopted and a bioprosthetic valve was implanted on the mechanical valve sewing ring as a final resort. The intervention was successful. Rapidly progressing left atrial hemangiopericytoma
We describe a case of hemangiopericytoma in the left atrium, which was diagnosed as myxoma preoperatively. A 70-year-old woman was admitted with heart failure. An echocardiogram showed a large myxoma-like mass in the left atrium, herniating into the left ventricle; therefore, an emergency operation was performed. Histological examination revealed a malignant hemangiopericytoma. The patient’s postoperative course was uneventful, but she died due to a local recurrence 4 months after the operation. Spontaneous pneumomediastinum: Experience in 13 adult patients
Conclusions Spontaneous pneumomediastinum is a benign disorder, but underlying lung diseases and concomitant pneumothorax are likely to complicate the disease course. Exacerbation of post-tubercular obstructive airway disease is a common risk factor for spontaneous pneumomediastinum in a tuberculosis endemic country.
