Right upper lobe lung cancer: Resection through left anterior mediastinotomy
We describe the case of a 55-year-old woman who underwent a left pneumonectomy for lung carcinoma. Four years later, a radiological examination revealed a hypermetabolic nodule in the right upper lobe, which was located in the left hemithorax because of right lung hyperinflation and a mediastinal shift to the left. Wedge resection was carried out through a left anterior mediastinotomy. We believe that an anterior mediastinotomy represents a valuable option for the management of recurrent lung cancer after previous surgery. (Source: Asian Cardiovascular and Thoracic Annals)

Superior vena cava obstruction after heart transplantation
Superior vena cava obstruction can be a serious complication after heart transplantation. A 58-year-old man with ischemic cardiomyopathy underwent orthotopic bicaval heart transplantation. On the 12th postoperative day, one hour after removing the central venous line, he developed sudden onset of facial edema, cyanosis, and tachycardia. Embolized ductus device extraction and associated iliofemoral artery repair
Less-invasive methods for nonsurgical closure of a patent ductus arteriosus have gained popularity as first-line therapy. An 11-year-old girl underwent percutaneous patent ductus arteriosus closure with a device. The device embolized into the descending aorta. An attempt at catheter retrieval was complicated by iliofemoral arterial injury. Subsequent device extraction was performed with repair of the arterial injury using a tube graft. Surgical retrieval of entrapped coronary angioplasty guidewire from the aorta
An entrapped or retained coronary angioplasty guidewire is a rare but serious complication of coronary interventions. A failed percutaneous transluminal coronary angioplasty attempt on the left anterior descending artery in a 35-year-old man was complicated by entrapment of the guidewire. Under cardiopulmonary bypass and cardioplegic arrest, the whole length of the entrapped guidewire was retrieved successfully from the left anterior descending artery and the aorta through an aortotomy following revascularization with left internal mammary artery. Repair of unbalanced atrioventricular septal defect with small right ventricle
A 2-month-old boy with trisomy 21, a small right ventricle, and an unbalanced atrioventricular septal defect underwent successful biventricular repair without atrial fenestration. We decided to perform leftward partitioning of the atrioventricular valve to widen the right ventricle inflow tract. Pheochromocytoma and tetralogy of Fallot: Coincidence or a rare association?
We report the case of a 25-year-old woman with tetralogy of Fallot who presented with chest pain and hypertension, and on further investigation, was diagnosed with a pheochromocytoma in the right adrenal gland. She underwent surgical excision of the tumor. While the simultaneous occurrence of these two diseases is extremely rare, the suspicion of a possible relationship has been raised in the past. Leiomyosarcoma of the inferior vena cava
Inferior vena cava leiomyosarcoma is a rare tumor with a variety of symptoms. A 41-year-old woman was admitted with nonspecific epigastric pain. Computed tomography revealed a dense mass between the inferior vena cava and the liver. The patient underwent successful resection of the mass. The pathologic study confirmed leiomyosarcoma. Late cardiac metastasis from colorectal carcinoma 15 years after surgery
Late recurrence of malignant tumors in the heart more than 10 years after surgery is quite rare, especially for colorectal carcinoma. Here, we report a case of late cardiac metastasis from a primary colorectal carcinoma, which occurred more than 15 years after the initial surgery. Finite element analysis to model complex mitral valve repair
Although finite element analysis has been used to model simple mitral repair, it has not been used to model complex repair. A virtual mitral valve model was successful in simulating normal and abnormal valve function. Models were then developed to simulate an edge-to-edge repair and repair employing quadrangular resection. Stress contour plots demonstrated increased stresses along the mitral annulus, corresponding to the annuloplasty. Unusual redo mitral valve replacement for bleeding in Glanzmann thrombasthenia
We report the case of 23-year-old man with mitral valve regurgitation and Glanzmann thrombasthenia, who underwent mechanical mitral valve replacement. Warfarin therapy was devastating, causing bilateral hemothorax, pericardial effusion, gastrointestinal bleeding, and hematuria. Redo mitral valve replacement with a biological prosthesis was required to resolve this critical situation. To our knowledge, this is the first report of mitral valve replacement in Glanzmann thrombasthenia, highlighting the danger of oral anticoagulation in this pathology. Sarcoidosis or acute coronary syndrome: Easily distinguishable?
We describe such a case which resolved with thrombolysis, and on further investigation, the patient was found to have a large sinus of Valsalva aneurysm. Abnormal gross pathology at the time of surgery was suspected to be aortitis which was only confirmed on postmortem examination 6 months later. We highlight the importance of working closely with a pathologist when aortitis is suspected, because failure to treat it medically results in a poor outcome.
