Hypertension in pregnancy: An unresectable mediastinal pheochromocytoma
Hypertension is a relatively common occurrence during pregnancy, which usually has a benign course with an excellent prognosis. However, physicians caring for pregnant women should have a high index of suspicion for underlying medical conditions that could lead to a more perilous outcome. Herein, we present the case of a pregnant woman who was found to have uncontrollable hypertension late in her pregnancy, secondary to a mediastinal pheochromocytoma, which was deemed unresectable at the time of exploration after her delivery. Video-assisted thoracic surgery for cancer after thoracic aortic aneurysm repair
We report the successful use of video-assisted thoracoscopic surgery in 3 patients with history of open thoracic aortic aneurysm repair who presented with left lower lobe stage I non-small-cell lung cancer. While this approach is feasible, it is technically demanding and thus, at the present time, we recommend that its use be limited to high-volume video-assisted thoracoscopic surgery centers. Endotracheal tube as tracheal stent in vascular ring
Management of vascular ring is always a matter of great concern. There is limited literature on management of carbon dioxide retention in mechanically ventilated patients with vascular rings due to double aortic arch, while awaiting corrective cardiac surgery. A 29-day-old girl with complete duplicated double aortic arch, left descending aorta, and a left patent ductus arteriosus, had severe upper respiratory tract obstruction with carbon dioxide retention. Pulmonary valvuloplasty for pulmonary atresia-restrictive ventricular septal defect
We present our experience of utilizing both transesophageal and epicardial echocardiography during surgical pulmonary valvuloplasty in a 22-day-old neonate with pulmonary atresia with restrictive ventricular septal defect. Subclavian artery aneurysm in a patient with vascular Ehlers-Danlos syndrome
We describe our experience of surgical treatment in a 28-year-old woman with vascular Ehlers-Danlos syndrome. A right subclavian artery aneurysm was detected. The right vertebral artery arose from the aneurysm. Digital subtraction angiography showed interruption of the left vertebral artery. The aneurysm was excised and the right vertebral artery was anastomosed end-to-side to the right common carotid artery under deep hypothermia and circulatory arrest. Huge aneurysm and coronary-cameral fistula from right coronary branch: First case
We report the case of middle-aged woman who presented with a huge extracardiac aneurysmal mass and a coronary-cameral fistula from a right coronary artery branch. It was successfully repaired by ligation and excision plus marsupialization of the aneurysm. We believe this is the first such a case reported in literature. Acute coronary artery bypass graft failure in a patient with polycythemia vera
Polycythemia vera in patients undergoing cardiac surgery is clinically rare. A 65-year-old man with polycythemia vera was admitted with effort-related chest discomfort. We planned coronary artery bypass grafting for left anterior descending artery and obtuse marginal branch stenosis, using bilateral internal thoracic arteries, with perioperative prophylactic management to avoid thromboembolism. Successful repair of mesenteric ischemia in acute type A aortic dissection
A 64-year-old man with acute type A aortic dissection had superior mesenteric artery occlusion and marked metabolic acidosis. By an emergency laparotomy, bypass grafting from the left external iliac artery to the superior mesenteric artery was performed with great saphenous vein. After deep sedation and antihypertensive management in the intensive care unit, the acidosis resolved, and central repair was carried out. Two-stage approach for mixed total anomalous pulmonary venous connection
A 4-day-old boy underwent an urgent operation for mixed total anomalous pulmonary venous connection with the left upper pulmonary vein draining into the innominate vein and the other pulmonary veins draining into the coronary sinus. Endovascular repair for retrograde type A aortic dissection with malperfusion
An 81-year-old man became semiconscious and suffered back pain followed by chest pain. Computed tomography showed retrograde acute type A aortic dissection with entries in the proximal and middle descending aorta, and an occluded true lumen of the right carotid artery. A case of right superior vena cava draining into the left atrial roof
We report the multidisciplinary consideration in a case of right superior vena cava draining into the left atrial roof. A 12-year-old boy was diagnosed with a sinus venosus-type atrial septal defect associated with anomalous right upper pulmonary venous return. However, intraoperative inspection showed the right superior vena cava draining into the left atrial roof, associated with anomalous right upper pulmonary venous return.
