Factors Associated With Blood Transfusion During Intracranial Aneurysm Surgery
(Abstracted from J Clin Anesth, 36:164–167, 2017) This single-center retrospective observational study aimed to identify risk factors associated with intraoperative blood transfusions in patients undergoing intracranial aneurysm surgery. Considering known risks associated with blood product transfusion, several organizations now have guidelines that advise a more restrictive approach to product administration than in the past. Anesthetic Considerations for Patients With Congenital Central Hypoventilation Syndrome: A Systematic Review of the Literature
This study’s aim was to review current medical literature on congenital central hypoventilation syndrome (CCHS), a sleep-disordered breathing, especially with regard to perioperative anesthetic techniques. This systematic review of 45 relevant articles sourced from the Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, EMBASE, and MEDLINE was conducted until October 2015. Time for a Breath of Fresh Air: Rethinking Training in Airway Management
(Abstracted from Anaesthesia, 71(11):1259–1264, 2016) This editorial accompanied and provides commentary on the article by Lindkær-Jensen et al. It recommends potential solutions related to the conclusions of their research. (Source: Survey of Anesthesiology)

A National Survey of Practical Airway Training in UK Anaesthetic Departments. Time for a National Policy?
This article outlines the results of a 2013 survey that provides a snapshot of current practices regarding airway training workshops in the UK National Health Service hospitals. In the Fourth National Audit Project (NAP4) of the Royal College of Anesthetists and Difficult Airway Society, it was found that the most common reasons for complications were due to a lack of training, education, and judgment. Preprocedural Ultrasound Assessment Does Not Improve Trainee Performance of Spinal Anesthesia for Obstetrical Patients: A Randomized Controlled Trial
(Abstracted from J Clin Anesth, 37:21–24, 2017) The purpose of this trial was to assess the effectiveness of preprocedural ultrasound examination in facilitating spinal placement by junior anesthesia residents on patients undergoing elective cesarean delivery. Between February 2011 and October 2012, 17 trainee residents participated and performed spinal anesthesia 1 to 8 times on 80 healthy obstetric patients. (Source: Survey of Anesthesiology)

Hypocapnia Measured by End-Tidal Carbon Dioxide Tension During Anesthesia Is Associated With Increased 30-Day Mortality Rate
(Abstracted from J Clin Anesth, 36:123–126, 2017) The negative effects of hypocapnia are well documented, including a decrease in cerebral blood and cognitive function, an increase in airway resistance and pulmonary cellular dysfunction, vasoconstriction and an increase in myocardial oxygen demand, hypercoagulopathy, and dysrhythmias. However, there has been no study documenting an association between hypocapnia during anesthesia and postoperative mortality. (Source: Survey of Anesthesiology)

Detection of Inherited and Acquired Hemostatic Disorders in Surgical Patients
(Abstracted from Can J Anaesth, 63(9):1003–1006, 2016) This editorial accompanied the article by Bonhomme et al,1 which proposed a 7-point structured questionnaire HEMSTOP (Hematoma, hEmorrhage, Menorrhagia, Surgery, Tooth extraction, Obstetrics, Parents) to facilitate preoperative identification of surgical patients with hemostatic disorders requiring specific measures to reduce perioperative bleeding. The author outlines commonly used techniques to detect and evaluate the more common acquired hemostatic disorders, as well as rarer inherited ones in surgical patients. Preoperative Hemostatic Assessment: A New and Simple Bleeding Questionnaire
(Abstracted from Can J Anaesth, 63(9):1007–1015, 2016) The purpose of this investigation is to assess the effectiveness of the 7-item HEMSTOP (Hematoma, hEmorrhage, Menorrhagia, Surgery, Tooth extraction, Obstetrics, Parents) questionnaire in identifying bleeding disorders preoperatively. Unlike acquired blood disorders, inherited coagulation disorders and platelet dysfunctions are rare (1 in 40,000 patients without hemorrhagic symptoms). (Source: Survey of Anesthesiology)

Association Between Arterial Carbon Dioxide Tension and Outcome in Patients Admitted to the Intensive Care Unit After Coronary Artery Bypass Surgery
(Abstracted from J Cardiothorac Vasc Anesth, 31:61–68, 2017) Hypercapnia and hypocapnia can cause adverse physiologic changes. Acidemia due to hypercapnia can lead to myocardial depression and arrhythmia. (Source: Survey of Anesthesiology)

Effect of Continuous Paravertebral Dexmedetomidine Administration on Intraoperative Anesthetic Drug Requirement and Post-thoracotomy Pain Syndrome After Thoracotomy: A Randomized Controlled Trial
(Abstracted from J Cardiothorac Vasc Anesth, 31:159–165, 2017) The purpose of this prospective, randomized, controlled, double-blind trial was to evaluate the effectiveness of the administration of dexmedetomidine paravertebrally as an adjuvant to local anesthetic in limiting the intraoperative anesthetic drug requirement and the occurrence of postthoracotomy pain syndrome. Postthoracotomy pain increases the risk of delayed recovery due to atelectasis, hypoxemia, ventilation/perfusion mismatch, and infection. (Source: Survey of Anesthesiology)

Use of Postoperative Peak Arterial Lactate Level to Predict Outcome After Cardiac Surgery
(Abstracted from J Cardiothorac Vasc Anesth, 31:45–53, 2017) The purpose of this single-center retrospective analysis of prospectively collected data was to determine the role of lactate levels in predicting the severity of perioperative circulatory failure in patients recovering from cardiac surgery. Many reports support the link between elevated perioperative lactate levels and postoperative complications including mortality. (Source: Survey of Anesthesiology)
