Review: gut microbiota and functional diseases of the gastrointestinal tract.
CONCLUSIONS: Despite the exciting and growing research on the role of gut microbiota in FGIDs, our knowledge remains fairly limited. Further studies are needed to measure the diversity, function and resistance to antibiotics of the intestinal microbiota in FGIDs. May radiofrequency be the best choice for III-IV degree hemorrhoids? Results of a large series of patients.
CONCLUSIONS: Radiofrequency haemorrhoidectomy is a valid alternative to the conventional diathermy technique, due to the reduction of operative time, postoperative pain, early and late complication rate. Anti-gliadin antibodies in NCGS.
Authors: Infantino M, Meacci F, Grossi V, Macchia D, Manfredi M Abstract Although no biomarker has been identified to date, previous studies have reported that about 50% of patients with suspected Non Celiac Gluten Sensitivity (NCGS) had positive first generation anti- gliadin antibodies (AGAs), expecially of the IgG class. These antibodies are not specific for NCGS, being also found in CD (80-90%), autoimmune liver disorders (21.5%), connective tissue disease (9%) and IBS (20%), as well as in healthy controls (2-8%), but their finding in patients with a clinical phenotype consistent with NCGS has been reg...

A large multicenter study of recurrence after surgical resection of branch-duct intraductal papillary mucinous neoplasm of the pancreas.
CONCLUSION: The risk of BD-IPMN recurrence after resection depends upon the histological type, with the highest-risk groups being those with severe dysplasia/intraductal carcinoma in situ and invasive IPMN. Even after negative resection margins, the pancreatic remnant still harbors a risk of recurrence which requires long-term surveillance. Liver transplantation from HCV RNA-positive donors in the era of interferon-free HCV therapeutics: a reexamination of the situation.
Authors: Willuweit K, Canbay A, Gerken G, Timm J, Paul A, Treckmann J, Herzer K Abstract Although the availability of donor organs is limited, liver grafts from HCV-positive donors remained yet an obstacle, primarily because of limited therapeutic options for HCV reinfection and lower rates of graft and patient survival. However, new interferon-free regimens containing direct-acting antiviral agents have fewer adverse effects and better effectiveness, making HCV treatment feasible early after transplant. In 2014, we successfully used sofosbuvir and ribavirin to treat a patient with HCV genotype 3 cirrhosis...

Pathophysiology of non-celiac gluten sensitivity: where are we now in 2016?
Authors: Caio G, Riegler G, Patturelli M, Facchiano A, DE Magistris L, Sapone A Abstract Non-celiac gluten sensitivity (NCGS) is the term used to describe individuals complaining of intestinal and extra-intestinal symptoms related to gluten ingestion and rapidly improving after its withdrawal, and in which both celiac disease (CD) and wheat allergy (WA) were properly ruled out. The prevalence of this condition remains unknown and a lot of questions about the possible pathogenetic mechanisms are still unclarified. It is believed that NCGS represents a heterogeneous condition with different subgroups potenti...

Animal models to study non-celiac gluten sensitivity.
Authors: Amnuaycheewa P, Murray JA, Marietta EV Abstract Non-celiac gluten sensitivity (NCGS), sometimes known as non-celiac wheat sensitivity (NCWS), has recently received much attention, both scientific as well as from the alternative medical community. Over the past 5 years, there are over 200 publications on NCGS indexed on PubMed database, the gluten-free market has been growing bigger, and it is clear that the number of consumers who are on a gluten-free diet (GFD) possibly because of a suspicion for NCGS appears to grow even faster. Nevertheless, despite these three rising events, many questions abo...

Update on Hepatitis C treatment: Systematic review of clinical trials.
Authors: Jhaveri M, Procaccini N, Kowdley KV Abstract Chronic Hepatitis C is a major public health problem. The chronicity of the Hepatitis C can lead to advanced liver disease, cirrhosis and even hepatocellular carcinoma. Chronic hepatitis C is the leading indication of for liver transplantation in the United States. Since the introduction of directly acting antiviral agents (DAAs), there have been there have been dramatic advances in treatment of hepatitis C in terms of tolerability, duration of therapy with significant increases in the rates of sustained virology response (SVR). This review summarizes t...

Optimal treatment of hepatic encephalopathy: an update.
Authors: Allampati S, Mullen KD Abstract Hepatic Encephalopathy (HE) is a complex neuropsychiatric syndrome associated with decompensated liver disease. The spectrum of disease ranges from trivial abnormalities in complex decision making and prolonged reaction time to coma in its most severe form1. The very initial stages, recently termed covert Hepatic Encephalopathy (CHE), can only be diagnosed with the help of neuropsychiatric testing while the later and more severe forms, termed overt Hepatic Encephalopathy (OHE), can be diagnosed clinically. Severity of HE is graded based on West Haven Criteria and pl...

Stereotactic radiation therapy for hepatic malignancies.
Authors: Meyer J Abstract Management of primary and secondary tumors of the liver is a complex undertaking involving multi- disciplinary input. Radiation therapy has held a minor role in the treatment of liver tumors, but with technological innovations and the growth of the field of stereotactic body radiation therapy (SBRT), this role is evolving. In this review, I discuss the issues of radiation-induced hepatic toxicity, the planning of liver SBRT treatments, clinical results with SBRT, its place amongst other non-surgical ablative treatments, and potential future avenues of investigation. Endoscopic resection for patients with mucosal adenocarcinoma of the esophagus.
Authors: Knabe M, May A, Ell C Abstract Due to its rapidly increasing incidence in the last decade, esophageal adenocarcinoma has been attracting widespread interest among gastroenterologists all over the world. Advanced adenocarcinomas arising from intestinal metaplasia in the esophagus (Barrett's esophagus) are associated with a poor prognosis when esophagectomy is required. In addition to high mortality rates, there is a significant morbidity rate among patients who undergo surgery. However, if cancer can be diagnosed at an early stage, the endoscopic treatment options that are available significantly r...

Mood disorders and non celiac gluten sensitivity.
Authors: Casella G, Pozzi R, Cicognetti M, Bachetti F, Torti G, Cadei M, Villanacci V, Baldini V, Bassotti G Abstract The association between gluten related disorders and psychiatric diseases has been firmly demonstrated. Non celiac gluten sensitivity (NCGS) is a syndrome diagnosed in patients responsive to gluten free diet after ruling out celiac disease and wheat allergy. The pathogenesis of neuro-psychiatric disorders in NCGS is unclear. An association between gluten and schizophrenia was described for the first time in 1950 by Bender et al. In the 50', Dicke noted that gluten free diet improved mood in...

Glutathione in the treatment of liver diseases: insights from clinical practice.
Authors: Sacco R, Eggenhoffner R, Giacomelli L Abstract Liver diseases markedly contribute to the global burden of mortality and morbidity. The pathogenesis of alcohol and non-alcohol induced liver diseases is complex and many factors have been described to contribute to the progressive loss of liver functions, including the over- generation of reactive oxygen species. The tripeptide glutathione (GSH) is the most important low molecular weight antioxidant synthesized in cells. In fact, GSH is a reducing molecule and it can react with oxygen species by neutralizing the unpaired electrons that make them high...
