Quality of life and adherence in liver transplant recipients.
Authors: Burra P, Ferrarese A, Feltrin G Abstract The assessment of Quality of Life (QoL) after transplantation represents an ancillary tool to evaluate the efficacy of solid organ transplantation, in addition to graft and patient survival rates and complication. The global assessment of QoL after transplantation usually confirms the improvement when compared to pre-transplant setting. However, whereas significant improvement in physical activity has been widely reported, several studies have demonstrated that social domain and cognitive function do not reach the level of general population after surgery. ...

Liver allocation.
Authors: Neuberger J Abstract Liver allocation policies determine how those livers that are offered for transplantation are allocated to those who have met the criteria for liver transplantation and are fit enough to undergo the procedure. The goals of allocation policies are based on varying combinations of need (reducing mortality awaiting a graft), benefit (maximising the outcomes of candidates on the list) or utility (maximising the life-years gained). Improvement in quality of life is rarely included in the setting of allocation policies. Policies need to be complaint with legal and ethical standards,...

Hepatitis B and liver transplantation.
Authors: Ferrarese A, Zanetto A, Russo FP Abstract Liver transplantation (LT) is the only effective treatment for hepatitis B-virus (HBV) related end stage liver disease, even if the outcome of these patients, has significantly improved after introduction of effective and well tolerated nucleos/tide analogues (NUC). Pre-transplant therapy has been initially based on lamivudine, but entecavir and tenofovir represent the currently recommended first-line therapeutic option in patients with HBV decompensated cirrhosis. After LT, the development of hepatitis B immunoglobulin (HBIG) in the early 1990s change dra...

Sorbitol malabsorption in patients with abdominal discomfort.
CONCLUSIONS: Sorbitol-malabsorption is a common finding in patients with functional abdominal discomfort. Dietary intervention positively affects abdominal symptoms in these patients. PMID: 28994566 [PubMed - as supplied by publisher] (Source: Minerva Gastroenterologica e Dietologica)

Underwater endoscopic mucosal resection (UEMR): an alternative for removal of large sessile colorectal polyps.
CONCLUSIONS: In this largest series on UEMR, we report the feasibility and safety of this procedure. Future randomized trials comparing this technique versus standard lift polypectomy technique will further elucidate the benefit of one over other. Histopathology, grading and staging of nonalcoholic fatty liver disease.
Authors: Kleiner D Abstract Nonalcoholic fatty liver disease (NAFLD) is the liver disease associated with obesity, diabetes and the metabolic syndrome . Although steatosis is a key histological feature, liver biopsies of patients with NAFLD can show a wide range of other findings, including portal and lobular inflammation, ballooning and apoptotic hepatocellular injury, Mallory-Denk bodies, megamitochondria and fibrosis. Nonalcoholic steatohepatitis (NASH) is a progressive subtype of NAFLD first defined by analogy to alcoholic hepatitis. The characteristic finding in NASH is ballooning hepatocellular injur...

Hepatitis C and liver transplantation.
Authors: Martini S Abstract Hepatitis C virus (HCV)-related liver disease represents the leading indication for liver transplantation (LT) in the United States and Europe and HCV recurrence is universal in recipients who are viremic at LT. Until a few years ago, pegylated-interferon in association with ribavirin was the only therapeutic strategy, usable only in compensated cirrhotic patients, in order to prevent post-LT viral recurrence. The recent advent of direct-acting antiviral agents (DAAs) has dramatically increased the chances of curative treatment for the transplant population and the debate about ...

Relationship between sleeping difficulties and airway symptoms severity with the health-related quality of life in patients with GERD.
CONCLUSIONS: The severity of these reflux-related disorders is well correlated with a significant negative impact on the normal daily activity. The cases with advanced severity of sleeping difficulties had the worst well-being. PMID: 28927247 [PubMed - in process] (Source: Minerva Gastroenterologica e Dietologica)

Comparison of two methods for the in-vivo diagnosis of Helicobacter pylori infection using a tablet of 13C-urea.
CONCLUSIONS: Correlation between the two methods was excellent. Moreover, the patient satisfaction, measured by VAS, showed a significantly greater acceptance of BreathID® method. This study support the use of a product containing 13C-urea 100 mg and citric acid for immediate determination of HP infection by urea breath test performed with BreathID®. Gut microbiota, obesity and metabolic disorders.
Authors: Federico A, Dallio M, DI Sarno R, Giorgio V, Miele L Abstract Obesity, diabetes and metabolic disorders represent hugely significant problems concerning the health in Western countries and the study of gut microbiota in metabolic pathologies is part of this framework. Diet effects on intestinal microbial composition and its role in pathogenetic mechanisms responsible for both obesity and systemic, hepatic and adipose tissue inflammation, represent at the moment one of this mostpromising topic in gastroenterology research. Gut health safety is essential, but it needs to be further explored in order...

Gut microbiota and the liver.
Authors: Federico A, Dallio M, Caprio GG, Ormando VM, Loguercio C Abstract Nowadays liver diseases represent one of major healthy problems in the world in terms of morbidity and mortality. The high prevalence of liver pathologies represents the key point to understand the necessity to identify the pathogenetic mechanisms that support these disorders in order to nurse them. Alterations of intestinal microbiota seem to play an important role in induction and promotion of liver damage progression, in addition to direct injury resulting from different causal agents. Gut microbiota is considered both as a promo...

Fecal microbiota transplantation: past, present and future perspectives.
Authors: Bibbò S, Ianiro G, Gasbarrini A, Cammarota G Abstract Gut microbiota is known to play a main role in regulating both health and disease in humans. Strategies for the therapeutic modulation of gut microbiota are therefore expected to give a relevant contribution in the management of disorders associated with its impairment. Among these options, one of the most renowned is fecal microbiota transplantation (FMT). A growing body of evidence showed clearly that FMT is an effective treatment against recurrent Clostridium difficile infection. Moreover, it was shown to be a promising therapy for the mana...

Microangiopathic hemolytic anemia caused by a signet-ring cell carcinoma of the intrahepatic bile duct.
Authors: Younes R, Ponzo P, Marzano A Abstract Microangiopathic hemolytic anemia (MAHA) originates from a mechanical injury of red cells, caused by vascular thrombosis or stenosis. Cancer is a cause of MAHA as a consequence of both chemotherapy and disseminated disease itself. Here we describe the case of a 60-year-old man who developed a signet-ring cell carcinoma originated from the intrahepatic bile ducts, complicated by bone marrow metastasis and MAHA.
