Eosinophilic esophagitis: current evidence-based diagnosis and treatment in children and adults.
Authors: Lucendo AJ Abstract Eosinophilic esophagitis (EoE) is a disease caused by an immune response to food antigens in contact with the esophageal mucosa that has arisen as a common disorder in current clinical practice. Its diagnosis is defined by the combination of symptoms of esophageal dysfunction and inflammation of the esophageal mucosa predominantly by eosinophils. Its chronic course and frequent progression to subepithelial fibrosis leading to strictures and narrow-caliber esophagus indicate the need for treatment. Information provided by recent clinical trials and systematic reviews allowed for...

Performance of the Over The Scope Clip system in the endoscopic closure of iatrogenic gastrointestinal perforations and post surgical leaks and fistulas.
Authors: Iabichino G, Eusebi LH, Palamara MA, Arena M, Pellicano R, Consolo P, Fagoonee S, Amato L, Opocher E, Barabino M, Luigiano C Abstract The increased invasiveness of endoscopic procedures and complex surgical interventions has resulted in an increased number of gastrointestinal iatrogenic defects, such as perforations, leak and fistulas. The conventional treatment for these gastrointestinal defects is surgery, with considerable risks especially in emergency situations and in patients with comorbidities. The Ovesco over-the- scope clip (OTSC) system (Ovesco Endoscopy AG, Tübingen, Germany) and more ...

"Tipping" extracellular matrix remodelling towards regression of liver fibrosis: novel concepts.
Authors: Magdaleno F, Schierwagen R, Uschner FE, Trebicka J Abstract Fibrosis development was initially conceived as an incessant progressive condition. Nowadays, it has become evident that fibrotic tissue undergoes a continuous two-way process: fibrogenesis and fibrinolysis, characterizing the remodeling of extracellular matrix (ECM). However, in established fibrosis, this two-way process is tipped towards fibrogenesis and this leads to a self-perpetuating accumulation of ECM, a distinct metabolic unit, as well as other cells and processes promoting fibrosis deposition. Several mechanisms promote fibrosis...

Clinical utility of the patency capsule: a large volume center experience.
CONCLUSIONS: PC is a safe and effective exam. Occlusive symptoms and imaging strictures were significantly associated with the retention. The retention rate was 25%, a similar result to that described in other series but higher than expected for the various indications, suggesting the importance of new research studies. Liver fibrosis in the context of nonalcoholic steatohepatitis: the role of adipokines.
Authors: DI Maira G, Pastore M, Marra F Abstract Liver fibrosis is a multifaceted process that occurs as a consequence of chronic liver injury. This process is characterized by inflammation, activation of matrix-producing cells, matrix deposition and remodeling, and epithelial cell regeneration. In subjects with chronic liver damage, fibrogenesis is favored by the presence of obesity and insulin resistance, which are also relevant risk factors for the development and progression of nonalcoholic steatohepatitis (NASH). It is now well-known that adipose tissue is not only involved in energy storage but also ...

Use of fully covered self expanding metal stents for benign biliary etiologies: a large multi-center experience.
CONCLUSIONS: FCSEMS are safe and effective for treating biliary strictures and leaks. We report decreased rates of stent migration compared to previous studies. Prospective studies are needed to compare plastic stents with FCSEMS, determine optimal stent in-dwell times and cost effectiveness of FCSEMS. Lactase non-persistent genotype distribution in Italy.
CONCLUSIONS: For the first time we analyzed the distribution of the LCT-13910 CC genotype in a big population of Italian subjects. Our data did not validate the presence of a North-South gradient for adult hypolactasia along the Italian peninsula. Grading health claims by weight of evidence would make for a better-informed public.
Authors: Allaert FA, Vandermander J, Herpin F, Ventura G Abstract In the European Regulation 1924/2006 (1) and especially its first recital; the evaluation of health claims (HC) by European Food Safety Agency (EFSA) was introduced so as "to ensure a high level of consumer protection, [and] give the consumer the necessary information to make choices in full knowledge of the facts…" Now, with 10 years of hindsight since the Regulation was adopted, it can be asked whether EFSA HC process of evaluation that led to a marginal number of accepted claims is consistent with this objective, not just for protecting...

Hepatocarcinoma: genetic and epigenetic features.
Authors: Lombardi A, Grimaldi A, Zappavigna S, Misso G, Caraglia M Abstract HCC is the third leading cause of cancer-related deaths worldwide, accounting for about 1 million deaths annually. The incidence of HCC is highest in Asia and Africa, where the endemic high prevalence of hepatitis B and hepatitis C strongly predisposes to the development of chronic liver disease and subsequent development of HCC. Patients with HCC generally present at an advanced stage due to compensated cirrhosis defined by the absence of pathognomonic symptoms, resulting in death within 6 to 20 months, suggesting an urgent need i...
