The gut microbiota: its anatomy and physiology during all life.
Authors: Gibiino G, Ianiro G, Cammarota G, Gasbarrini A
Abstract
The wide assemblage of microorganisms inhabiting our gastrointestinal tract is an attractive field for overwhelming contemporary research in several aspects of the human health. Most recent molecular techniques, as the use of rRNAs or whole genome sequencing, definitely overcame traditional culture techniques in giving outline of the intestinal bacterial richness. Moreover, the crosstalk between host and intestinal microbes is considered a cornerstone affecting the host's metabolism, immunity, autoimmunity and autoinflammation and elaborate n...

Gut microbiota and inflammatory bowel disease: an update.
Authors: Holleran G, Lopetuso LR, Ianiro G, Pecere S, Pizzoferrato M, Petito V, Graziani C, Mc Namara D, Gasbarrini A, Scaldaferri F
Abstract
Major advances have occurred in the knowledge of the pathogenesis of Inflammatory Bowel Disease (IBD) over the last decade, and perhaps the most major, and clinically advantageous of these advances has been the discovery of the microbiome as a key multifaceted component of inflammation. The Gut microbiome is the largest known group of cells in the body, and is now recognised as an organ in its own right. Initial studies looking at a possible role of bacterial manipul...

Complexity and diversity of GERD phenotypes.
Authors: Zentilin P, Marabotto E, Pellegatta G, Coppo C, Furnari M, Savarino E, Savarino V
Abstract
Gastroesophageal reflux disease (GERD) is defined as a condition which develops when the reflux of gastric contents causes troublesome symptoms, impairs quality of life, or leads to mucosal damage or complications. There are two main phenotypic presentations of GERD, the erosive (ERD) and non-erosive reflux disease (NERD), with the latter one representing up to 70% of GERD spectrum. Moreover, patients with GERD can be clinically subdivided into two distinct syndromes: patinets with esophageal and extraesopha...

Extra-esophageal presentation of GERD: new understanding in a new era.
Authors: Martinucci I, Albano E, Marchi S, Blandizzi C
Abstract
Associations of gastroesophageal reflux disease (GERD) with extraesophageal manifestations, such as chronic cough, asthma, and laryngitis, are reported frequently, and there is a strong evidence of biological plausibility in support of this relationship. On the other hand, extraesophageal reflux disease (EERD) is usually multifactorial in nature with reflux being just one of the several potential contributing cofactors. Moreover, the accuracy of currently available diagnostic tests for EERD is suboptimal, and therefore the causal relationship ...

Is there a role for high resolution manometry in GERD diagnosis?
Authors: Tolone S, Savarino E, Docimo L
Abstract
Gastroesophageal reflux disease (GERD) is a condition, which develops when the reflux of stomach contents causes troublesome symptoms and/or complications. In patients with alarm symptoms or signs or in those refractory to antiacid therapy, it is indicated to perform instrumental tests to correctly diagnose GERD presence. Esophageal manometry is currently considered the gold standard test for the diagnosis of esophageal dysmotility. However, it has shown limited capability in diagnosing GERD. With the advent of high resolution manometry (HRM), more accurate ...

Overlap of GERD and gastrointestinal functional disorders.
Authors: de Bortoli N, Natali V, Melissari S, Simonetti N, Tapete G, Marchi S
Abstract
The functional gastrointestinal disorders (FGIDs) are common conditions with an overall prevalence burden estimated at approximately one-third of the population. These represent a heterogeneous group of conditions which may include both abnormal symptom perception and functional causes and seems to share similar trigger factors as food, lifestyle and psychological factor. GERD develops when the reflux of gastric contents into the esophagus leads to troublesome symptoms, with or without mucosal involvement. Functional hea...

Overview of pathophysiological features of GERD.
Authors: Usai Satta P, Oppia F, Cabras F
Abstract
Gastro-esophageal reflux disease (GERD) is a very prevalent condition and has a high impact on the quality of life. Nevertheless, pathophysiology is complex and multi-factorial. Several mechanisms have been proposed: decreased salivation, decreased lower esophageal sphincter pressure resting tone, presence of hiatal hernia, increased number of transient lower esophageal sphincter relaxations, increased acid, and pepsin secretion, duodeno-gastro-esophageal reflux of bile acids and trypsin. Other factors contributing to the pathophysiology of GERD include poo...

PPI-refractory GERD: current diagnosis and management.
Authors: Frazzoni L, Fuccio L, Frazzoni M
Abstract
Proton pump inhibitor (PPI) therapy is the mainstay of treatment for gastroesophageal reflux disease (GERD). However, up to 30% of patients with reflux symptoms report inadequate symptom control with PPI therapy. PPI-refractory GERD should be suspected when troublesome reflux symptoms persist after 4-week standard dosage and 8-week high-dosage PPI therapy. Impedance-pH monitoring represents the gold standard for investigating the mechanism(s) of PPI refractoriness and for distinguishing patients with reflux-related from those with reflux- unrelated PPI-ref...

Natural history of bleeding risk in colonic diverticulosis. Review.
Authors: Segev L, Ozuner G
Abstract
Colonic diverticulosis prevalence is gradually increasing worldwide. 3-15% of the patients with diverticulosis will bleed at some point. The risk for rebleeding after the first episode is substantial but varies in the literature between 13-48%. We analyze risk factors for diverticular bleed and management options for initial and recurrent diverticular bleeding. A review of the literature on this subject is presented.

Gut microbiota and pancreatic diseases.
Authors: Signoretti M, Roggiolani R, Stornello C, Delle Fave G, Capurso G
Abstract
Changes in diet, lifestyle and exposition to environmental risk factors account for the increased incidence of pancreatic disorders, including acute and chronic pancreatitis, and pancreatic cancer. The role of the microbiome in the development of pancreatic disorders is increasingly recognized. The translocation of gut bacteria and endotoxins following gut barrier failure is a key event contributing to the severity of acute pancreatitis, while small intestine bacterial overgrowth is common in patients with chronic pancreatit...

Surgery for diverticulitis in the 21st century: recent evidence.
Authors: Morris AM, Kin C
Abstract
Sigmoid diverticulitis is an increasingly common disease in Western countries. As technology has led to increased knowledge about the disease and options for treatment, management strategies have become less certain. In previous years, the rationale for early elective surgery was largely preventive, due to concern that diverticulitis recurrence would result in increased risk of sepsis or the need for a colostomy. New technology has enabled diagnosis, through computed tomography scans, predictive information through clinical and administrative databases, and less invasive ...

How to modulate gut microbiota: diet, pre-probiotics or antibiotics?
Authors: Ponziani FR, Binda C, Gasbarrini A
Abstract
The gut microbiota is involved in the maintenance of humans' health and its alterations have been associated to several intestinal and extra intestinal disorders. For this reason, there is increasing interest on gut microbiota modulation as both a preventive strategy or as a therapeutic option in different gastrointestinal, hepatic and systemic diseases. Many studies have tried to find out the impact of diet, prebiotic, probiotic and antibiotics on gut microbiota composition and function, obtaining very heterogeneous and sometimes conflicting results, es...
