Demographics, tumor characteristics, treatment, and clinical outcomes of patients with ampullary cancer: a surveillance, epidemiology, and end results (SEER) cohort study.
CONCLUSIONS: The annual incidence of ampullary cancer has been fairly constant, though males are more likely to be affected. While its incidence increases with age, patients who are treated by surgical intervention have significantly better outcomes. Additionally, through the use of endoscopic techniques, ampullary cancer can be detected and treated much earlier. Over-the-scope-clips as primary and rescue therapy for non-variceal gastrointestinal bleeding: a systematic review and meta-analysis.
CONCLUSIONS: This meta-analysis demonstrates success on the use of OTSC as primary and rescue therapy in the management of NVGIB. Further trials should clarify the ideal setting for the use of OTSC and assess the cost of these devices as compared to standard therapy. Liver stiffness based model for portal hypertension and hepatocellular cancer risk in HBV responsive to antivirals.
CONCLUSIONS: LSPS is useful to identify patients with regression of PH and EVs, avoiding endoscopy. LSPS ≤0.62 at baseline or due to antiviral therapy is associated with a lower risk of HCC. Early antiviral treatment is recommended in order to maintain or to induce LSPS ≤0.62. PMID: 30356037 [PubMed - as supplied by publisher] (Source: Minerva Gastroenterologica e Dietologica)

Challenges and future developments in liver transplantation.
Authors: Toniutto P, Bitetto D, Fornasiere E, Fumolo E Abstract Liver transplantation (LT) has become the treatment of choice for a wide range of liver diseases in both adult and pediatric patients. Until recently, the largest proportion of LT in adults, were performed in patients with hepatitis C (HCV) related cirrhosis. The recent availability of safe and effective direct antiviral agents to cure HCV infection in almost all patients whatever the HCV genotype and severity of liver disease, will reduce the need for LT in this category of recipients. Thus, it is presumed that in the next 1 to 2 decades HCV ...

Negative payoffs of upper gastrointestinal endoscopy in patients admitted under observation status.
CONCLUSIONS: Upper gastrointestinal endoscopy in patients with atypical chest pain of GI origin as an initial step is a significant healthcare burden and contradicts the currently recommended High Value Care recommendations. Our study delineates this large negative financial impact when performing upper endoscopies under observation status. Such patients should be started on an empirical trial of PPI, and endoscopy should be reserved for patients whose symptoms are unresponsive to PPIs or have alarm features. Symptoms and dietary practices of irritable bowel syndrome patients compared to controls: results of a United States National survey.
CONCLUSIONS: In this US survey, IBS was reported by 9% of the population, most often females over 45 years. IBS patients were more likely to implement therapeutic strategies and cite food triggers, yet few were aware of the low-FODMAP diet. Outreach programs could improve awareness of this substantiated intervention. PMID: 30293415 [PubMed - as supplied by publisher] (Source: Minerva Gastroenterologica e Dietologica)

Diverticulosis today.
Authors: Elisei W, Brandimarte G, Tursi A Abstract The incidence of diverticulosis and diverticular disease of the colon is increasing worldwide. Although the majority of patients remains asymptomatic long-life, the prevalence of diverticular disease of the colon, including acute diverticulitis, is substantial and is becoming a significant burden on National Health Systems in terms of direct and indirect costs. Fiber, non-absorbable antibiotics and probiotics seem to be effective in treating symptomatic and uncomplicated patients, and 5-aminosalicylic acid might help prevent acute diverticulitis. Unfortuna...

Prevalence of Helicobacter pylori infection among dyspeptic patients with and without type 2 diabetes mellitus in South-West and South-South regions of Nigeria.

Skeletal muscle-gut axis: emerging mechanisms of sarcopenia for intestinal and extra intestinal diseases.
Authors: de Sire R, Rizzatti G, Ingravalle F, Pizzoferrato M, Petito V, Lopetuso L, Graziani C, de Sire A, Mentella MC, Mele MC, Gasbarrini A, Scaldaferri F Abstract In recent years, there has been an increasing interest on muscle wasting, considering the reduction of quality of life and the increase of morbidity and mortality associated. Sarcopenia and cachexia represent two conditions of reduction of muscle mass, sharing several elements involved in their pathogenesis, such as systemic inflammation, impaired muscle protein synthesis, increased muscle apoptosis, mitochondrial dysfunction in skeletal muscl...

Is CO2 insufflation an amelioration of routine colonoscopy?
CONCLUSIONS: The study revealed only moderate positive effects for use of CO2 during colonoscopy. We found only modest improvement of patient's comfort during and after colonoscopy. No reduced doses of drugs for sedation could be recorded. PMID: 30008194 [PubMed - in process] (Source: Minerva Gastroenterologica e Dietologica)

Post-pancreatitis diabetes mellitus and gastroparesis: driven together?
This article overviews studies on epidemiology and impact of gastroparesis in the setting of type 1 and type 2 diabetes, with a view to estimating the possible burden of gastrointestinal dysfunction in PPDM. Further, the usefulness of quantifying gastrointestinal dysmotility is discussed. The gastroparesis cardinal symptom index has emerged as a standardised gastrointestinal dysmotility endpoint and its use in individuals after pancreatitis is justified.
