Helicobacter pylori infection in Africa: 2018 literature update.
Authors: Smith SI, Seriki A, Ndip R, Pellicano R Abstract Helicobacter pylori (H. pylori) relationship with humans is believed to have originated in Africa and three African H. pylori populations have been identified: hpAfrica1, hpAfrica2 and hpNEAfrica. Actually, H. pylori infection is a ubiquitous infection in developing countries, particularly Africa. In this continent, the prevalence of H. pylori is higher and the bacterium is acquired at an early age compared to developed countries. In fact, while it affects over 50% of humans in developed countries its prevalence reaches 80% or more among adults in A...

Gastroenterology today: between certainties and news: a meeting held in Turin on May 27, 2017.
Authors: Pellicano R, Ribaldone DG, Astegiano M, Dughera L, Battaglia E, Morgando A, Arrigoni A, Spandre M, Grassini M, Saracco GM, Mégraud F Abstract This special article reports on two crucial issues discussed during a meeting. The first was the updated management of Helicobacter pylori infection. This was approached taking into account the recent European Guidelines, with a focus on novelties in treatment. In particular, considering the increasing H. pylori antibiotic resistance to clarithromycin, in countries with a high clarithromycin resistance rate the bismuth-containing quadruple therapies should ...

Chronic kidney disease in patients with chronic hepatitis C virus infection.
Authors: Shahab O, Golabi P, Younossi ZM Abstract Hepatitis C virus (HCV) infection affects many organs in the body, including the liver, kidneys, skin, joints and others. Although the hepatic manifestation of HCV has been widely studied, the extrahepatic manifestaions of HCV have not been fully appreciated. Studies have shown that patients with HCV have a higher risk of chronic kidney disease and end-stage renal disease, as well as poorer outcomes after kidney transplantation. Given these findings, it is important to screen HCV patients for presence of renal impairement in a timely manner. Current guideli...

Prevalent use of combined prophylaxis of hepatitis B after liver transplantation in Italy: results of a national survey in a large cohort.
CONCLUSIONS: Data from this survey confirm: the excellent results of combined prophylaxis; the past and persistent use of Hepatitis B Immunoglobulin-on and only rare -off prophylactic regimens, in contrast with the newest reports; the increasing use of anti-HBc-positive grafts; the past and present high prevalence of HBsAg-positive recipients, due to an increase in candidates with either hepatocellular carcinoma and Hepatitis Delta Virus coinfection in the last years. Internist, anesthesiologist and surgeon use of ketogenic diet.
Authors: Cenci L, Paoli A, Omar HR, Dalvi P, Camporesi EM, Mangar D, Quartesan S, Fiorito A, Bosco G Abstract Ketogenic diet is being increasingly utilized in recent decades because of its success as an effective tool for short and intermediate-term weight loss. Promoting physiological ketosis from a drastically low carbohydrate diet is the fundamental basis for this diet regime. Though debated, these diets have been demonstrated to be effective, at least in the short- to medium terms, to manage excess weight, hyperlipidemia, and other cardiovascular risk factors. We reviewed the cardiovascular, metabolic,...

Liver transplantation for non-alcoholic fatty liver disease.
Authors: Germani G, Becchetti C Abstract Non-alcoholic fatty liver disease (NAFLD) is considered the hepatic manifestation of metabolic syndrome (MS) [1], and it is defined by the presence of steatosis in >5% of hepatocytes, according to histological analysis or detected by specific proton density fat fraction obtained MRI. The term NAFLD covers two pathologically distinct conditions: non-alcoholic fatty liver (NAFL) and non-alcoholic steatohepatitis (NASH), which can have similar clinical presentation, but with different prognosis. Both conditions can lastly lead to the development of cirrhosis. End st...

Indications for and outcomes of liver transplantation in patients with alcohol-associated liver disease (AUD).
Authors: Phillips PK, Caldis MW, Hristov AD, Lee AS, Lucey MR Abstract The manufacture and consumption of alcohol are ubiquitous and as old as time. Alcohol use disorder (AUD) is defined as a problematic pattern of alcohol use leading to clinically significant impairment. The physical injuries that result from excessive alcohol consumption are varied including end-organ damage to liver (alcohol-associated liver disease or ALD), pancreas, brain and heart, and systemic consequences including several forms of cancer. Liver transplantation (LT) remains the treatment of last resort for life-threatening liver di...
