Epidemiology and natural history of gastro-esophageal reflux disease.
Authors: Savarino E, Marabotto E, Bodini G, Pellegatta G, Coppo C, Giambruno E, Brunacci M, Zentilin P, Savarino V Abstract Gastroesophageal reflux disease (GERD) is highly prevalent in Western countries, particularly when considering its most classic symptom, that is heartburn. This symptom is very frequent in the community and ranges from 10% to more than 30%, according to the various population-based studies. This disease is much more represented in Europe and US than in Asiatic countries. It has been shown that GERD prevalence increases in parallel with the remarkable growth of obesity, as this conditi...

Gut microbiota and gastric disease.
Authors: Sgambato D, Miranda A, Romano L, Romano M Abstract The gut microbiota may be considered a crucial "organ" of human body because of its role in the maintenance of the balance between health as well as disease. It is mainly located in the small bowel and colon, while, the stomach was long thought to be sterile in particular for its high acid production. In particular, stomach was considered "an hostile place" for bacterial growth until the identification of Helicobacter pylori (HP). Now, the stomach and its microbiota can be considered as two different "organs" that share the same place and they hav...

Effect of Cis-palmitoleic acid (an omega-7 fatty acid) supplementation on inflammation and expression of HNF4 γ, HNF4α and IL-6 in patients with ulcerative colitis: a double-blind, randomized pilot study.
CONCLUSIONS: Cis-palmitoleic acid as co- adjuvant therapy for 8 weeks seems to decrease the inflammatory activity through the increased expression of HNF4α and HNF4γ in patients with UC. PMID: 28185444 [PubMed - as supplied by publisher] (Source: Minerva Gastroenterologica e Dietologica)

The intestinal mycobiota: a year of observation about the incidence of yeast's isolation in fecal samples.
CONCLUSIONS: These data testify the constant presence in the human gut of a fungal community that, according to us, could be defined as intestinal "mycobiota". This study wishes to be a contribution to discover the relationship established between human and yeasts and to evaluate the colonizing or pathogenetic role performed by these microorganisms. Adequate dietary fiber supplement along with TONE concept can help avoid surgery in most patients with advanced hemorrhoids.
CONCLUSIONS: Adequate fiber supplement coupled with TONE concept can correct deranged defecation habits and can thus prevent progression of hemorrhoids and bleeding and the need for surgery in most patients with advanced hemorrhoids. Life style and other risk factors for diverticulitis.
CONCLUSIONS: The recognition of risk factors for the development of diverticular disease or even complicated disease like life style habits or medication is crucial for patient management. PMID: 28150481 [PubMed - as supplied by publisher] (Source: Minerva Gastroenterologica e Dietologica)

Towards reducing the risk of new onset diabetes after pancreatitis.
Authors: Jivanji CJ, Soo DH, Petrov MS Abstract Metabolic abnormalities due to acute and chronic pancreatitis are common. In particular, post-pancreatitis diabetes mellitus (PPDM) is increasingly recognised as a distinct clinical entity. However, most of research in this field is focused on unravelling the pathogenesis of PPDM whereas research frontiers are rarely extended to reducing the risk of new onset diabetes after pancreatitis (NODAP). The recent development and validation of the PERSEUS score paved the way to identifying individuals after acute pancreatitis who are at high risk of developing NODAP....

The diagnostic challenge of symptomatic uncomplicated diverticular disease.
Authors: Cremon C, Bellacosa L, Barbaro MR, Cogliandro RF, Stanghellini V, Barbara G Abstract Colonic diverticulosis is a common condition in Western industrialized countries occurring in up to 65% of people over the age of 60 years. Only a minority of these subjects (about 10- 25%) experience symptoms, fulfilling Rome criteria for irritable bowel syndrome (IBS) diagnosis (IBS-like symptoms) in 10 to 66% of cases. Symptomatic uncomplicated diverticular disease (SUDD) is a syndrome characterized by recurrent abdominal symptoms attributed to diverticula in the absence of macroscopically evident alterations o...

World epidemiology of non-celiac gluten sensitivity.
Authors: Vasagar B, Cox J, Herion JT, Ivanoff E Abstract While the term "gluten" has become commonplace, the disorders associated with gluten still remain poorly understood. Non-celiac gluten sensitivity (NCGS), the most recently recognized of the gluten-related disorders, is arguably the most unknown. While celiac disease and wheat allergy have diagnostic algorithm, NCGS remains a diagnosis of exclusion. With no evidence-based objective diagnostic criteria or serological tests, it is difficult to diagnose and even more difficult to study epidemiologically. Studies often use varied definitions of NCGS and ...

Therapeutic management of acute uncomplicated diverticulitis.
Authors: Fan YC, Peery AF Abstract Acute diverticulitis is a painful disease of the colon characterized by peridiverticular inflammation and/or infection. The spectrum of acute diverticulitis ranges from isolated peridiverticular inflammation all the way to free perforation with purulent or feculent peritonitis. Among gastrointestinal disorders, acute diverticulitis is one of the most common diagnoses in the ambulatory setting and principal hospital discharge diagnoses. Acute diverticulitis accounts for substantial utilization of healthcare resources and is a detriment to patients' quality of life. In the ...

Rationale and evidences for treatment of symptomatic uncomplicated diverticular disease.
CONCLUSIONS: The results of the studies available in literature are controversial and debatable, for this reasons a clear and defined algorithm for treatment of SUDD has not yet been defined. Further randomized, double-blind, placebo controlled study are necessary. PMID: 27973463 [PubMed - as supplied by publisher] (Source: Minerva Gastroenterologica e Dietologica)

Pathogenesis of diverticulosis and diverticular disease.
Authors: Walker MM, Harris AK Abstract Diverticulosis is defined by the presence of diverticula due to herniation of mucosa and muscularis mucosa through the muscularis propria at sites of vascular penetration in the colon and is asymptomatic in the vast majority affected. There are global differences of distribution, in Western industrialised societies, the most common site is in the right colon, but in Asia left sided diverticulosis predominates. Whilst present in 17.5% of a general population and 42% of all comers at endoscopy it is seen in 71% of those aged ≥80 years. Diverticular disease is defined ...
