Role of thyroid ultrasound combined with thyroglobulin in the diagnosis of postoperative recurrence of thyroid cancer.
CONCLUSIONS: Thyroid ultrasound combined with Tg is of high clinical value in the diagnosis of postoperative recurrence of thyroid cancer, which can significantly improve the sensitivity, specificity and accuracy of clinical diagnosis. The TgAb level can be detected when Tg is used to monitor the postoperative recurrence and metastasis of thyroid cancer, providing a reference for the clinical prevention and treatment. A clinical Audit: diagnostic and epidemiological evaluation of the Adrenal Incidentaloma (AI).
Authors: Nuzzo V, Attardo T, Augello G, Brancato D, Camerlingo S, Canale C, Coretti F, Franco A, Giacometti F, Gambacorta M, Loreno M, Maffettone A, Provenzano V, Zuccoli A Abstract Audit is a clinical instrument of government characterized by a whole process of evaluation inter pares to improve medical behaviors in the clinical practice. Different endocrinopathies are underestimated in the clinical and diagnostics practice but they can be a real problem in patients admitted in the departments of Internal Medicine. The Adrenal Incidentaloma (AI) is an accidental discovery with an incidence equal to 4% in r...

Cushing's disease: major difficulties in diagnosis and management during pregnancy.
Authors: Caimari F, Corcoy R, Webb SM Abstract Pregnancy in women with a diagnosis of Cushing' syndrome (CS) is an extremely rare event and its diagnosis and treatment are a real medical challenge. During pregnancy, the hypothalamus-pituitary-adrenal axis undergoes major changes leading to a significant increase in plasma cortisol levels throughout gestation. The difficulties in diagnosis are related to the resemblance of symptoms of CS and those of pregnancy, and to the complex interpretation of the screening tests. Moreover, the diagnostic work up in the postnatal period may be difficult in the first wee...

Quality of semen: a 6-year single experience study on 5680 patients.
CONCLUSIONS: Our patients had spermiogram's results quite better than the standard guidelines. Our study showed that the country of origin could be a factor influencing several parameters of the spermiogram in healthy population and through Robust regression confirmed a strict correlation between age and these parameters. Non-functioning pituitary tumors: any special considerations during pregnancy?
Authors: Flitsch J, Burkhardt T Abstract New clinical symptoms from pituitary tumors during pregnancy are rare. Either the patient becomes symptomatic from a known lesion prior to conception during pregnancy, or it is a de-novo diagnosis due to acute symptoms. Non-functioning pituitary tumors can be subdivided into non-functioning pituitary adenomas and other pathologies. This review aims at giving a general overview as well as therapeutical considerations, taking into account that no specific recommendations/guidelines are available for this rare clinical problem. TSH-receptor autoantibodies: pathophysiology, assay methods, and clinical applications.
Authors: Ehlers M, Allelein S, Schott M Abstract TSH receptor antibodies (TRAbs) are characteristic indicators for the common autoimmune thyroid disease Graves' disease (GD). In almost all patients stimulating TRAbs are found leading to hyperthyroidism as these antibodies act in an agonistic manner to TSH. Besides stimulating TRAbs, other TRAbs are also frequently found leading to inhibition of TSH receptor signaling (blocking TRAbs) or to the activation of different signaling cascades resulting in e.g. thyrocyte apoptosis (cleavage antibodies). Patients' sera may contain all three types of TRAbs. Dependen...

Prolactinomas: how to handle prior to and during pregnancy?
Authors: Glezer A, Bronstein MD Abstract Prolactinomas are the most common cause of pathological hyperprolactinemia, leading to central hypogonadism and, therefore, a frequent etiology of infertility. Treatment, usually with dopamine agonist (DA), can reverse hyperprolactinemia and hypogonadism, allowing pregnancy in the majority of cases. Bromocriptine is still the DA of choice for such purpose. Important issues in DA-induced pregnancies include fetal exposition, both malformations and neuropsychological development and tumor size increase. Regarding microprolactinomas and intrasellar macroprolactinomas, ...

Study on regulation of adipokines on body fat distribution and its correlation with metabolic syndrome in type 2 diabetes mellitus.
CONCLUSIONS: The adipokines DPP-4 and leptin in the serum can influence the body fat distribution of patients with T2MD; there is an important association of DPP-4, leptin and ADPN levels with MS, which may be used as therapeutic targets for multiple metabolism disorders of T2MD.
