Onychomycosis with greenish-black discolorations and recurrent onycholysis caused by Candida parapsilosis
Publication date: Available online 16 April 2019Source: Medical Mycology Case ReportsAuthor(s): Gai Ge, Zhiya Yang, Dongmei Li, G. Sybren de Hoog, Dongmei ShiAbstractCandida parapsilosis (C. parapsilosis) has become a common pathogen, especially in immunocompromised hosts. Here, we present an immunocompetent adult with greenish-black discoloration of the right first finger nail in combination with recurrent onycholysis. C. parapsilosis was isolated from the right first finger nail and was confirmed by morphological characteristics as well as by DNA molecular analysis. Patient was successfully treated with oral itraconazole...

Cryptococcal meningitis after ART: Need for proper baseline evaluation in the era of ‘Test & Treat’
We report a case of severe cryptococcal meningitis presenting following the commencement of ART. There is clear benefit in the early commencement of ART among HIV infected patients and to prevent patients lost to follow-up as aimed with the ‘Test & Treat’ approach. However, this approach needs to be balanced against the risk of IRIS and its associated morbidity and mortality when those patients are not being properly evaluated for opportunistic infections being present without overt symptoms. Rasamsonia sp: An emerging infection amongst chronic granulomatous disease patients. A case of disseminated infection by a putatively novel Rasamsonia argillacea species complex involving the heart
Publication date: Available online 16 April 2019Source: Medical Mycology Case ReportsAuthor(s): Ahmed Babiker, Nupur Gupta, Connie F.C. Gibas, Nathan P. Wiederhold, Carmita Sanders, James Mele, Hongxin Fan, Alina Iovleva, Ghady Haidar, Carolyn FernandesAbstractChronic granulomatous disease (CGD) is a heterogeneous condition due to defects in NADPH oxidase characterized by granuloma formation and increased susceptibility to invasive infections, in particular moulds. The use of broad-spectrum, mould-active antifungal prophylaxis has improved mortality. However rare resistant moulds have emerged as important pathogens. Diagno...

Hypersensitivity pneumonitis of a bagpipe player: Fungal antigens as trigger?
Publication date: Available online 26 March 2019Source: Medical Mycology Case ReportsAuthor(s): Katharina Ziegler, Marcus Joest, Nesrin Turan, Dirk Schmidt, Peter-Michael Rath, Joerg SteinmannAbstractHere we present a 79-year old man with chronic hypersensitivity pneumonitis probably caused by fungal contamination of a bagpipe. Several samples were taken from the patient's bagpipe. Four potential fungal antigens (Exophiala phaeomuriformis, Kwoniella europaea, Pyrenochaeta unguis-hominis and Aureobasidium melanogenum) as potential trigger of hypersensitivity pneumonitis were identified. A serum ELISA test with Exophiala pha...

Chronic Candida dubliniensis meningitis in a lung transplant recipient
We report a case of a possible donor derived Candida dubliniensis infection in a lung transplant recipient, who initially presented with empyema that was treated successfully, but subsequently developed chronic meningitis. Diagnosis was delayed due to the low yield of CSF cultures, and was confirmed with fungal PCR and BD glucan assay. (Source: Medical Mycology Case Reports)

Infection of burn wound by Aspergillus fumigatus with gross appearance of fungal colonies
Publication date: Available online 15 March 2019Source: Medical Mycology Case ReportsAuthor(s): Anh – Tram Que, Ngoc - Minh Thai Nguyen, Ngoc – Anh Do, Nhu-Lam Nguyen, Ngoc – Dung Tran, Tran – Anh LeAbstractThe morbidity of invasive aspergillosis in burn patients is low but the diagnosis is difficult and the mortality rate is high. A severe burned patient at the Vietnam National Institute of Burn was suspected of fungal wound infection (FWI) with fungal growth on the wound. The diagnosis of FWI caused by Aspergillus fumigatus was made by isolation and histological examination. This may be the first reported case of...
