Saccharomyces cerevisiae fungemia in a critically ill patient with acute cholangitis and long term probiotic use

Palate ulcer, uvular destruction and nasal septal perforation caused by Sporothrix brasiliensis in an HIV-infected patient
We report a dramatic presentation of Sporothrix brasiliensis infection, with destruction of the nasal septum, soft palate, and uvula of an HIV-infected woman. She was successfully treated with amphotericin B deoxycholate followed by itraconazole. Chronic pulmonary aspergillosis following pulmonary embolism
We describe a case of CPA in a 79-year-old female following bilateral pulmonary emboli. The clinical implications are numerous, including the dilemma of anticoagulation. Treatment of cerebral mucormycosis with drug therapy alone: A case report
We report on an elderly male patient with headache and right-side weakness. Imaging studies revealed multiple space-occupying lesions in the parietal and occipital cerebral regions. Biopsy revealed broad aseptate ribbon-like structures branching at right angles, suggestive of mucormycosis. Improvement was observed after medical therapy with 20 weeks of liposomal amphotericin B (5 mg/kg/day) combined with posaconazole, followed by posaconazole (400 mg twice a day) alone for 1 month. Aspergillus fumigatus endocarditis in an Indigenous Australian male without classic risk factors

Utilization of Voriconazole Drug Monitoring in the Treatment of Cutaneous Scedosporium apiospermum Infection
We report the use of voriconazole troughs to achieve appropriate therapeutic levels in treatment of a cutaneous Scedosporium apiospermum infection. Following heart transplantation, a 63-year-old immunocompromised patient presented with post-traumatic nodular lesions on his right shin. Pathology showed fungal yeasts with culture revealing Scedosporium apiospermum. According to therapeutic drug monitoring, initial voriconazole treatment was subtherapeutic requiring increased dosing until appropriate therapeutic trough levels were attained, and resolution of the fungal infection was achieved.
