Microbiological and immunological characteristics of a lethal pulmonary Aspergillus niger infection in a non-neutropenic patient
Publication date: September 2018Source: Medical Mycology Case Reports, Volume 21Author(s): Jessica D. Workum, Suzanne W. de Jong, Mark S. Gresnigt, Katharina L. Becker, Peter Pickkers, Frank L. van de Veerdonk, Yvonne F. Heijdra, Eva KolwijckAbstractInvasive pulmonary aspergillosis is increasingly described in non-neutropenic patients, such as patients with COPD receiving corticosteroids and the critically ill. Here, we present a case of a lethal pulmonary Aspergillus niger infection in a COPD patient. Immunological tests showed an impaired innate and adaptive immune response to Aspergillus. A history of COPD, unresponsive...

Two cases of fungal keratitis caused by Metarhizium anisopliae
We present two cases of keratitis due to Metarhizium anisopliae in geographically separated areas of the United States. The isolates were microscopically similar but morphologically different and were identified by ribosomal DNA sequencing. Both isolates had low minimum inhibitory concentration (MIC) values to caspofungin and micafungin, but high MIC values to amphotericin B. The morphologic and antifungal susceptibility differences between the two isolates indicate possible polyphylogeny of the group. (Source: Medical Mycology Case Reports)

Subcutaneous abscesses caused by Trichophyton rubrum in the unilateral groin of an immunocompromised patient: A case report
Publication date: September 2018Source: Medical Mycology Case Reports, Volume 21Author(s): Utako Okata-Karigane, Yasuki Hata, Emiko Watanabe-Okada, Shunichi Miyakawa, Michi Ota, Yutaka Uzawa, Shigekazu Iguchi, Atsushi Yoshida, Ken KikuchiAbstractA 60-year-old Japanese man presented with multiple subcutaneous nodules in his left groin. Histologically, the nodules consisted of suppurative granulomas and abscesses not involving the hair follicles. Trichophyton rubrum TWCC57922 was detected by fungal culture and polymerase chain reaction (PCR) sequencing of the rDNA genes. We diagnosed these nodules as deeper dermal dermatophy...

Blastomycosis in a renal transplant recipient: Case of immune reconstitution inflammatory syndrome
Publication date: September 2018Source: Medical Mycology Case Reports, Volume 21Author(s): Alexis Guenette, Shahid Husain, Ana Konvalinka, William Geddie, Coleman RotsteinAbstractThere are limited data on blastomycosis in solid organ transplant recipients with the subsequent development of the immune reconstitution inflammatory syndrome (IRIS). Herein we describe a case of pulmonary blastomycosis in a renal transplant recipient with the development of concomitant IRIS. (Source: Medical Mycology Case Reports)

A case of pulmonary cryptococcoma due to Cryptococcus gattii in the United Kingdom
We report a case of Cryptococcus gattii infection in the UK in a 76-year-old woman on biologic therapy for intra-abdominal non-Hodgkin lymphoma. An incidental nodular lung lesion was found on a chest imaging and histology, culture and molecular mycology studies of the lobectomy specimen revealed the presence of C. gattii. Lomentospora prolificans vertebral osteomyelitis with spinal epidural abscess in an immunocompetent woman: Case report and literature review
We report a case of L. prolificans thoracic vertebral osteomyelitis with spinal epidural abscess in a patient without apparent immunodeficiency. Clinical manifestations and radiographic findings could not distinguish from other etiologic agents. Treatment is also challenging because L. prolificans is usually resistant to antifungal agents. The patient underwent surgical debridement and has been receiving a prolonged combination of antifungal therapy to prevent an infection relapse. (Source: Medical Mycology Case Reports)

Near-fatal mucormycosis post-double lung transplant presenting as uncontrolled upper gastrointestinal haemorrhage
We present a case of splenic mucormycosis post-double lung transplant, presenting as uncontrolled near-fatal upper gastrointestinal haemorrhage, to remind clinicians of the need to consider pre-transplant invasive fungal infection risk factors if an unexpected fungal infection arises in the post-transplant period. This case also highlights the valuable contribution of molecular technology for fungal identification but also the need for clinical correlation. (Source: Medical Mycology Case Reports)

Local extensive granulomatous inflammation of the neck region and lymphangitis caused by Lichtheimia corymbifera infection in a Japanese Black calf
Publication date: September 2018Source: Medical Mycology Case Reports, Volume 21Author(s): A.P.P. Teh, T. Hirai, S. Ito, Y. Hidaka, Y. Goto, H. Furukawa, J. Sawada, R. YamaguchiAbstractA 7-month-old female Japanese Black calf developed elongated, nodular mass measuring 30 × 16 cm extended from the retropharyngeal region to mid lateral neck region. Histological examination revealed granulomatous lymphangitis with non-septate fungal hyphae recognized throughout the lesions. Fungal culture, DNA sequencing and molecular phylogenetic tree analysis confirmed the sequence of Lichtheimia corymbifera. The lymphogenous route ...

Multiple Organ Dysfunction Syndrome (MODS) induced by Candida krusei in an Aldabra giant tortoise (Aldabrachelys gigantea) and confirmed by electron microscopy analysis
Publication date: September 2018Source: Medical Mycology Case Reports, Volume 21Author(s): Maddalena Iannaccone, Petra Rita Basso, Terenzio Congiu, Paolo Cavicchio, Valentina Ulivi, Marco CampoloAbstractA young female Aldabra giant tortoise (Adabrachelys gigantea) was presented with anorexia, ataxia, severe constipation and bloating. Analysis revealed liver disease and collected biopsy diagnosed Candida krusei infection. Despite Itraconazole treatment, the tortoise got worse and died. Aspergillus endocarditis of the mitral valve with ventricular myocardial invasion, cerebral vasculitis, and intracranial mycotic aneurysm formation in a patient with hemophagocytic lymphohistiocytosis
We report a case of mitral valve aspergillus endocarditis with ventricular myocardial invasion, cerebral vasculitis and intracranial fungal aneurysm formation in a patient with hemophagocytic lymphohistiocytosis (HLH). This case illustrates the importance of prompt investigation and treatment of masses seen on an echocardiogram for rare infections such as Aspergillus endocarditis in immunocompromised patients. Candida auris colonization in an immunocompetent patient: A new threat in medical ICU
Publication date: September 2018Source: Medical Mycology Case Reports, Volume 21Author(s): Shukla Das, Richa Tigga, Gargi Rai, Praveen Kumar Singh, Shyama Datt, Asha Tyagi, Narendra Pal SinghAbstractCandida auris has become a great challenge in diagnostic, therapeutic and hospital environmental adaptation. With a prevalence of 5.3% in intensive care unit (ICU) acquired candidemia in India, its colonization is very rapid which hastens hospital transmission. Strict surveillance and preventive measures need to be adopted in ICU as it can persist on dry, inanimate object, prompt adaptation and antifungal resistance can pose a ...
