A Retrospective Evaluation of Response to Vitamin D Supplementation in Obese Versus Nonobese Patients
Conclusion: Our study demonstrated an overall moderate response rate to replacement therapy with ergocalciferol and considerable variability in vitamin D replacement strategies initiated by primary care providers. Based on our findings, elevated BMI ≥30 kg/m2 may impact the likelihood of attaining vitamin D sufficiency with ergocalciferol. (Source: Journal of Pharmacy Practice)

Evaluation of a Pharmacy-Managed Pharmacokinetic Dosing Program
Conclusion: Implementation of a pharmacy-managed pharmacokinetic dosing program significantly improved appropriate dosing of famotidine, enoxaparin, and ketorolac. These findings could justify expansion of pharmacist autonomy through institution–approved, pharmacy-managed programs for other medications to improve appropriate dosing. Conclusion: This study demonstrates gaps in patients’ understanding of warfarin therapy. Relying solely on the information in the FDA medication guide is insufficient to guarantee adequate understanding. Evaluation of Treatment Courses When Vancomycin Is Given Every 8 Hours in Adult Patients
Conclusion: We observed an increase in the percentage of trough levels >20 mg/L later during treatment courses of vancomycin IV Q8H with a relatively small corresponding increase in vancomycin dose. Pilot and Revision of a Basal-Bolus Dosing Guideline for the Management of Hyperglycemia in Noncritically Ill Adult Patients
The use of basal-bolus insulin (BBI) regimens for the treatment of inpatient hyperglycemia has become standard of care. The purpose of this study was to develop and evaluate a newly piloted dosing guideline and utilize the results to adjust it prior to its implementation hospital-wide. This was an institutional review board approved, prospective, and multiphase study.
