Perceptions of a Continuing Professional Development Portfolio Model to Enhance the Scholarship of Teaching and Learning
Objective: To obtain feedback about the potential usefulness of a continuing professional development (CPD) portfolio for enhancing a faculty or practitioner’s scholarship of teaching and learning (SoTL). Method: A CPD portfolio approach to the SoTL was distributed in advance to registrants of the 2011 Annual AACP Teacher’s Seminar. In an interactive workshop, faculty facilitators described a model for a CPD process applied to the development of an individual’s SoTL. During the workshop, participants were asked to complete the initial sections of the portfolio to develop a personal plan for success in th...

Combination of Inhaled Corticosteroid and Bronchodilator-Induced Delirium in an Elderly Patient With Lung Disease
We describe the occurrence of delirium with the combination of an inhaled glucocorticoid and bronchodilator. An elderly male described confusion and hallucinations within 1 week after initiation of budesonide/formoterol for chronic obstructive pulmonary disease. The combination inhaler was discontinued with resolution of symptoms. Several weeks later, the patient was hospitalized and restarted on the combination inhaler. The patient was alert and oriented on admission, however, confusion and hallucinations progressed throughout his hospital stay. The combination inhaler was discontinued and his confusion and hallucinations...

Serotonin Syndrome: Is It a Reason to Avoid the Use of Tramadol With Antidepressants?
Conclusions: Review of the available case reports of tramadol combined with antidepressant drugs in therapeutic doses indicates caution in regard to the potential for SS but does not constitute a contraindication to their use. Tramadol is only contraindicated in combination with MAOIs but not other antidepressants in common use today. These case reports do suggest several factors associated with a greater risk of SS, including increased age, higher dosages, and use of concomitant potent cytochrome P450 2D6 inhibitors. Tramadol can be safely combined with antidepressants; however, monitoring and counseling patients are prud...

Safety of Varenicline in Patients With Cardiovascular Disease
Smoking cessation lowers the risk of death substantially in patients with cardiovascular disease. Although varenicline is an effective medication for smoking cessation, its safety in this population has been questioned and evaluated in several studies. In 2 randomized controlled trials of patients with cardiovascular disease, the rates of serious cardiovascular events were up to 2% higher in patients receiving varenicline than placebo, though the differences were not statistically significant. In the first meta-analysis of mostly trials involving patients with a history of cardiovascular disease, varenicline was found to s...
