Humeral Head Fracture with Intrathoracic Migration
Publication date: June 2018 Source:Journal of Orthopaedics, Trauma and Rehabilitation, Volume 24 Author(s): Gianfilippo Caggiari, Giulia Raffaella Mosele, Leonardo Puddu, Mauro Spiga, Carlo Doria Fracture-dislocation of the humeral head with intrathoracic migration is extremely rare. In our study we describe the case of a 23-year-old man who was admitted to the emergency clinic of our hospital, after being injured in a high-speed motor vehicle accident. The patient presented in a state of hemorrhagic shock and severe respiratory disease. Chest radiography showed fracture of the right humeral head and the presence of...

Clinical, Magnetic Resonance Imaging, and Arthroscopic Correlation in Anterior Cruciate Ligament and Meniscal Injuries of the Knee
Conclusion In conclusion, the present study supports that clinical diagnosis is of primary necessity, as the positive predictive value is high for all the lesions. MRI is an additional diagnosing tool for ligament and meniscal injuries of the knee. Routine use of MRI to confirm the diagnosis is not indicated, as the positive predictive value is low, but can be used to exclude pathology, as the negative predictive value is high for all the lesions. Pathological Fracture of the Proximal Tibia from an Intraosseous Gouty Tophus: A Rare Presentation of Gout
We present a rare case of a pathological fracture due to an intraosseous gouty tophus as the presentation of gout, which imposed a diagnostic challenge, and illustrates the importance of multidisciplinary management in such conditions. (Source: Journal of Orthopaedics, Trauma and Rehabilitation)

Adverse Effects of Smoking on Outcomes of Orthopaedic Surgery
Publication date: December 2017 Source:Journal of Orthopaedics, Trauma and Rehabilitation, Volume 23 Author(s): Sheung-tung Ho Smoking has many adverse effects on the musculoskeletal system, particularly on the outcomes after orthopaedic surgery. Smoking is associated with surgical site infection and postoperative wound complications after spine surgery, total joint arthroplasty, and fracture fixation; nonunion after spinal fusion, ankle fusion, osteotomy, and internal fixation and bone grafting for scaphoid nonunion; worse outcomes after lumbar disc prolapse, spinal stenosis, and cervical myelopathy surgery; periprosth...

Does Conservative Rehabilitation Program Lead to Long-Term Stiffness after Arthroscopic Rotator Cuff Repair?
Conclusions Immobilization for 4 weeks to 6 weeks after arthroscopic rotator cuff repair does not result in long-term stiffness and may even be beneficial to cuff tendon healing. (Source: Journal of Orthopaedics, Trauma and Rehabilitation)

Surgical Techniques of Using Direct Anterior Approach and Bridging Plate Fixation of Periprosthetic Fracture of an Arthrodesed Hip: A Case Report
We present a case of a 65-year-old woman with traumatic periprosthetic fracture of left proximal femur with ipsilateral arthrodesed hip and cobra plate in-situ. It imposes challenges on achievement of stable fixation for fracture management and preservation of blood supply for fracture healing. Minimally invasive plate osteosynthesis through direct anterior approach was executed with intraoperative templating. Anterior bridging plating using pre-bent reverse distal femoral locking compression plate (less invasive stabilisation system) was performed successfully. The patient had a fracture union in 10 months and returned t...

Spinal Implants Can Be Retained in Patients with Deep Spine Infection: A Cohort Study
Conclusion Implant removal is not always necessary in cases of deep spine infection as retention of implants is not associated with higher mortality, relapse, or reoperation rates. (Source: Journal of Orthopaedics, Trauma and Rehabilitation)

Non-Hodgkin Lymphoma Mimics Infected Total Knee Arthroplasty
We report a case of non-Hodgkin lymphoma mimicking an infected total knee arthroplasty. A 73-year-old woman who received left total knee replacement presented with fever and a discharging sinus over her left knee 12 weeks after surgery. The diagnosis of prosthetic joint infection was made and a two-stage revision arthroplasty planned. After implant removal and insertion of an antibiotic spacer in the first-stage operation, culture results of all intraoperative specimens were negative but the pathology report showed diffuse large B-cell lymphoma. She then received chemotherapy and a second-stage reconstruction operation. T...

Distribution of Sesamoid Bones in the Hand —A Study in the Chinese Population
Conclusion This is the first study in Hong Kong to report prevalence of sesamoid bones in the Chinese Population and identify the symmetry of sesamoid bones in bilateral hands. The result revealed that the Chinese ethnic group had a higher prevalence of sesamoid bones at the 2nd and 3rd MCPJ when compared with other ethnic groups. The distribution of sesamoid bones was symmetrical in bilateral hands. (Source: Journal of Orthopaedics, Trauma and Rehabilitation)
