A rare case of parotid acinic cell carcinoma presenting during pregnancy
Introduction: Acinic cell carcinomas (AcCCs) are rare, histologically-diverse salivary gland malignancies. More common in women and those of white race, AcCCs usually present as slowly-enlarging solitary mobile masses, most often in the parotid tail. There are few established risk factors. An unintended consequence of artificial feeding
Aim: Re-feeding syndrome is an under-recognised complication associated with nutritional supplementation following a period of starvation or malnourishment. Prolonged malnutrition leads to protein catabolism and phosphate depletion; introduction of carbohydrate leads to an anabolic state causing large electrolyte shifts, salt and fluid retention which untreated can lead to death. An unusual presentation of pneumomediastinum
A young gentleman presented to A&E with an acute history of neck swelling and a transient episode of chest pain. He was normally fit and well. On examination, he had a visibly distended neck with palpable crepitus extending from neck to torso. He was systemically well. ECG showed widespread concave ST elevation resembling pericarditis. CT neck and chest revealed extensive surgical emphysema along the thoracic wall, extending through the mediastinum, upwards into the neck. No pneumothorax was seen. Benign osseous metaplasia of the breast: Case report
Aim: Osseous metaplasia of the breast usually refers to the presence of bone tissue in benign and malignant breast neoplasms. It is a rare, with only a few cases reported in the literature, especially in association with benign lesions. Management of a large CBD stone, beyond the scope of ERCP
We present the case of a 77-year-old lady admitted with obstructive jaundice. Magnetic resonance cholangiopancreatography (MRCP) performed identified the presence of a large common bile duct (CBD) stone measuring 50.22 mm by 20.62 mm. Due to the size of the stone it was not possible for removal via endoscopic retrograde cholangiopancreatography (ERCP) or use lithotripsy to aid its removal. Attempts to insert a stent past the stone to drain bile had failed. Thus the stone had to be removed surgically by an open common bile duct exploration. (Source: International Journal of Surgery)

Oesophageal stent causing small bowel obstruction
Description: A 59 year old woman presented with abdominal pain and vomiting. Her past medical history included a T4 N2 M0 infiltrative squamous cell carcinoma of the right upper lobe invading the upper mediastinum, and causing extrinsic compression of the oesophagus. Due to worsening dysphagia she had undergone metallic stent placement 5 months prior to admission. CT scans to monitor response to chemotherapy had shown gradual migration of her stent into the stomach. Plain abdominal x-ray showed the stent to be lying in the pelvis. Caecal volvulus, a rare complication of abdominoplasty
Introduction: Caecal volvulus is a rare complication of abdominoplasty with only one published report in 2014. Incomplete rotation of the caecum during embryogenesis predisposes patients to caecal volvulus as there is potential for the mesentery to twist. Additionally, intra-abdominal pressure rises following abdominoplasty, reducing the likelihood of volvulus resolving spontaneously. (Source: International Journal of Surgery)

Supracondylar fractures with a pink hand: Beware the tear!
Aims: Supracondylar fractures of the humerus are common in children (17.9% of all paediatric fractures). A 10-year study demonstrated 10% of these result in vascular injury. Intimal tear leading to brachial artery thrombosis can initially be clinically occult as, at presentation, the vessel may not have occluded. The hand can be pink and well perfused, with or without absent pulses. We aim to demonstrate that close neurovascular observation is necessary even if the hand is pink and well-perfused. Salmonella infected total hip replacement complicated by a Vancouver C peri-prosthetic fracture; Management, surgical technique, trials and tribulations
We report a sixty-five year old who developed an infected total hip replacement with Salmonella Typhimurium. The patient was managed for ten years with suppressive antibiotics before necessitating a two-stage revision for worsening pain.
