An unusual presentation of pneumomediastinum

A young gentleman presented to A&E with an acute history of neck swelling and a transient episode of chest pain. He was normally fit and well. On examination, he had a visibly distended neck with palpable crepitus extending from neck to torso. He was systemically well. ECG showed widespread concave ST elevation resembling pericarditis. CT neck and chest revealed extensive surgical emphysema along the thoracic wall, extending through the mediastinum, upwards into the neck. No pneumothorax was seen.
